Usefulness of transient and persistent no reflow to predict adverse clinical outcomes following percutaneous coronary intervention.

The no reflow phenomenon is reported to occur in >2% of all percutaneous coronary interventions (PCIs) and portends a poor prognosis. We analyzed data from 5,286 consecutive patients who underwent PCI from the Melbourne Interventional Group (MIG) registry from April 2004 through January 2008 who had 30-day follow-up completed. Patients without no reflow (normal reflow, n = 5,031) were compared to 255 (4.8%) with no reflow (n = 217 for transient no reflow, n = 38 for persistent no reflow). Patients with transient or persistent no reflow were more likely to present with ST-elevation myocardial i

Bridge to decision using the Levitronix CentriMag short-term ventricular assist device.

BACKGROUND: Ventricular assist devices are effective in severe end-stage cardiac failure but outcomes remain poor in critically ill patients. Cheaper and less invasive short-term devices have been used in this setting. We report our experience with the Levitronix CentriMag short-term ventricular assist device as a potential bridge prior to deciding whether a more expensive device should be used or whether transplantation should be undertaken. METHODS: Since August 2003, 16 moribund patients (14 males; age 32.7 +/- 14.9 [range 16 to 62] years) have been supported with the CentriMag device as a

Risk index for perioperative renal dysfunction/failure: critical dependence on pulse pressure hypertension.

BACKGROUND: An acute renal event after coronary bypass graft surgery is associated with high mortality and substantial additive cost. METHODS AND RESULTS: This prospective and descriptive study of 4801 patients having coronary bypass graft surgery with cardiopulmonary bypass from November 1996 to June 2000 at 70 centers in 16 countries established associations between predictor variables and postoperative renal composite (renal dysfunction and/or renal failure) from a cohort of 2381 patients and developed a risk index assessed in a validation cohort of 2420 patients. Postoperative renal compo

Left main coronary artery stenosis: factors predicting cardiac events in patients awaiting coronary surgery.

Although most patients with left main coronary artery stenosis undergo urgent coronary artery bypass grafting, limited information is available regarding the risk factors that might lead to cardiac events between angiographic diagnosis and surgery. We retrospectively reviewed 1,731 cases of coronary artery bypass grafting at our institution, 97 of which were performed in patients with significant (> or = 50%) left main coronary artery stenosis. These patients were placed in 1 of 2 groups: eventful waiting or uneventful waiting. We analyzed multiple preoperative variables, and the incidence of

Predictors of length of stay after coronary stenting.

BACKGROUND: Postprocedure length of stay (LOS) remains an important determinant of medical costs after coronary stenting. Variables that predict LOS in this setting have not been well characterized. METHODS: We evaluated 359 consecutive patients who underwent coronary stenting with antiplatelet therapy. Sequential multiple linear regression (MLR) models were constructed with use of 4 types of variables to predict log-transformed LOS: preprocedure, intraprocedure, and postprocedure factors and adverse outcomes. RESULTS: Preprocedure factors alone explained more than one third of the variabili

Complete arterial revascularization in multivessel coronary artery disease with 2 conduits (skeletonized grafts and T grafts).

BACKGROUND: Complete arterial CABG is a surgical option to improve long-term results in the treatment of coronary artery disease (CAD). Harvesting of multiple arterial grafts is commonly associated with prolonged operating times and increased trauma. By use of new operative techniques (skeletonized grafts and the T-graft approach), CABG in multivessel CAD is now possible with only 2 grafts. We present our experience in the use of these techniques on a routine basis. METHODS AND RESULTS: Between March 1996 and September 1999, 490 patients (aged 61+/-9 years, 20% female) underwent complete arte

Kidney Allograft Outcomes of Dual-Organ Heart Kidney Transplant Recipients Following Donation After Circulatory Determination of Death.

INTRODUCTION: Simultaneous Heart-Kidney Transplantation (SHKT) following donation after circulatory determination of death (DCD) is increasing. Whether kidney allograft outcomes differ from donation after brain death (DBD) SHKT recipients has not been well studied. METHODS: We identified adults in the UNOS database who were listed for SHKT and transplanted after 2018. Baseline characteristics of DBD and DCD recipients and donors were compared. Outcomes of interest included primary nonreceipt of a kidney allograft, acute post-transplant dialysis, and chronic dialysis or renal transplantation.

Management of Post-Myocardial Infarction Ventricular Septal Defects.

Post-myocardial infarction ventricular septal defect (post-MI VSD) is a rare but life-threatening complication, with an incidence of approximately 0.3% and high early mortality despite advances in reperfusion therapy. Management remains challenging and requires multidisciplinary decision-making. We conducted a narrative review of the literature through May 2024 using PubMed, Scopus, and Google Scholar, focusing on surgical, percutaneous, and mechanical circulatory support (MCS) strategies for post-MI VSD. A narrative approach was adopted due to the rarity of the condition, the heterogeneity of

Percutaneous closure of ventricular septal rupture after myocardial infarction: a case report of competing timelines in multifactorial shock.

BACKGROUND: Ventricular septal rupture (VSR) following acute myocardial infarction is a rare but deadly complication with mortality rates exceeding 90% when left untreated. Coronary ischaemia leads to myocardial necrosis and friability resulting in septal rupture, most commonly 3-5 days post-infarction. The sudden development of a left-to-right shunt causes volume overload in the right ventricle, pulmonary vasculature, and left atrium, precipitating biventricular failure, and cardiogenic shock. Management requires careful consideration of timing and modality of closure along with stabilization

Fatal early graft failure in a heart transplant recipient with occult multivessel coronary artery disease in a young donor: implications for donor evaluation.

BACKGROUND: Primary graft dysfunction (PGD) represents the leading cause of early mortality following heart transplantation. Among the associated factors, donor characteristics-particularly the presence of undiagnosed coronary artery disease-may play a relevant role. Donor evaluation is typically based on clinical assessment and echocardiographic findings, which may be insufficient to detect subclinical coronary disease, especially in young donors. CASE PRESENTATION: We report the case of a 52-year-old patient with advanced dilated cardiomyopathy who underwent orthotopic heart transplantation

Veno-Arterial Extracorporeal Membrane Oxygenation in High-Risk Ventricular Tachycardia Ablation: Current Evidence, Patient Selection, and Perioperative Management.

Catheter ablation is an established therapy for recurrent ventricular tachycardia (VT), but in patients with advanced structural heart disease, procedural success is often limited by acute hemodynamic decompensation during arrhythmia induction and mapping. Temporary mechanical circulatory support may improve procedural tolerance in selected high-risk cases. Among available devices, venoarterial extracorporeal membrane oxygenation (VA-ECMO) provides the most comprehensive support by combining biventricular circulatory assistance with gas exchange independent of native cardiac output, albeit at

Comparison in trends and outcomes of multiple vs. single arterial coronary bypass graft surgery.

OBJECTIVE: Multiple arterial grafting (MAG) has been suggested to confer long-term survival benefits for patients undergoing coronary artery bypass grafting (CABG), yet its short-term benefits remain uncertain. This study aims to analyze the impact of MAG on in-hospital outcomes and identify potential risk factors. METHODS: A retrospective analysis was conducted from all patients who underwent CABG surgery in our development from January 2022 to December 2024. A generalized mixed-effects model and sensitivity analysis were employed to evaluate the influence of the type of CABG bypass graft on

Differences in Care and Outcomes in Cardiogenic Shock in Cardiac Intensive Care Units in the United States and Canada: CCCTN Registry Insights.

BACKGROUND: Mortality in cardiogenic shock (CS) remains high. Significant interhospital heterogeneity in critical care therapies has been described, which reflects the lack of high-quality evidence to guide optimal treatment. We aimed to describe differences in practices and clinical outcomes among patients with CS in the United States and Canada. METHODS: The Critical Care Cardiology Trials Network (CCCTN) is a research network of tertiary cardiac intensive care units (CICUs). Data collection spanned from 2017 to 2022. The analysis included 34 American and 8 Canadian sites. The outcomes of i

Comparison of Extracorporeal Membrane Oxygenation Initiation in Cardiogenic Shock During Work-Hours Versus After-Hours.

BACKGROUND: Extracorporeal membrane oxygenation (ECMO) as rescue therapy for cardiogenic shock (CS) is highly dependent on timeliness and medical resources. OBJECTIVES: Aimed to assess ECMO management and outcomes in adult patients with CS in terms of on- and after-hour ECMO initiation from a national ECMO registry. METHODS: Adult patients diagnosed with CS and those who received ECMO were enrolled in the study. The population was divided into the work-hours and the after-hours group based on the time of ECMO initiation. In-hospital mortality and ECMO management were compared between them. E

Modelling and Simulation of the Interactions Between the Cardiovascular System and the Combined Use of VA ECMO and IABP: Comparison Between Peripheral and Central Configurations.

Veno-arterial extracorporeal membrane oxygenation (VA ECMO) for the management of refractory cardiogenic shock (CS) has been widely used in recent years. Increased left ventricular (LV) afterload induced by retrograde flow remains a limiting factor, which is particularly evident during peripheral VA ECMO support. The concomitant use of the intra-aortic balloon pump (IABP) is an established strategy to achieve LV unloading during VA ECMO support. Nevertheless, there remains controversy about the combined use of IABP during central or peripheral VA ECMO in terms of beneficial effects and outcome
