Injury to Anatomic Structures Due to Fixation Device in Management of Mandibular Fracture.

INTRODUCTION: ORIF is the gold standard for the management of mandibular fractures. The chances of postoperative complications are due to infection, damage of the inferior alveolar nerve canal, or tooth root during the fixation of the fracture fragments. Fixation hardware placed in the vicinity of the vital structures may encroach on the anatomic structures, leading to postoperative complications. AIM: The aim of this study is to evaluate the injury to the anatomic structures in the proximity of the fracture line due to the fixation device. MATERIAL AND METHODS: This retrospective study was

Pediatric Facial Fractures: Interpersonal Violence as a Mechanism of Injury.

PURPOSE: Interpersonal violence is a relatively infrequent cause of injury to the craniofacial skeleton in the pediatric population. The presentation of fractures as a result of different causes varies dramatically and can have a direct impact on management. The current study compares facial fractures in a pediatric population as a result of interpersonal violence with other mechanisms of injury. METHODS: A retrospective review of all of the facial fractures at a level 1 trauma center in an urban environment was performed for the years 2000 to 2012. Patients ≤18 years were included. Patient d

Pediatric Facial Trauma.

Pediatric facial fracture management is often complex and demanding. The structure and topography of the pediatric craniofacial skeleton are profoundly different from the mature skull. Consequently, the pediatric facial skeleton responds differently to traumatic force. Although the incidence of pediatric facial trauma is higher than in the adult population, the incidence of facial fracture is significantly lower. The management in younger patients is often more conservative because of potential growth impairment. As the facial skeleton matures, more conventional surgical approaches become appr

Trend and Demographic Characteristics of Maxillofacial Fractures in Level I Trauma Center.

AIM: The aim of this study was to analyze the pattern and treatment of craniomaxillofacial injuries in the northern part of Israel, within a Jewish majority and large Arab minority population. PATIENTS AND METHODS: A 5-year retrospective study evaluated patients treated for craniomaxillofacial fractures. Fracture cause, type, site, and patient demographics were evaluated. RESULTS: Patient age ranged from 1 to 94 years with an average age of 36.7 years; 52% of the victims were Jews and 48% Arabs. There was male predilection in both sectors (78.3% vs 21.7%). The main site of injury was the zyg…

Functional Anatomic Computer Engineered Surgery Protocol for the Management of Self-Inflicted Gunshot Wounds to the Maxillofacial Skeleton.

PURPOSE: Virtual surgical planning (VSP) is an indispensable aid in craniomaxillofacial reconstruction, yet no protocol is established in facial gunshot wounds. We review our experience with computer-aided reconstruction of self-inflicted facial gunshot wounds (SIGSW'S) and propose a protocol for the staged repair. METHODS: A retrospective case series enrolling patients with SIGSW's managed with the Functional Anatomic Computer Engineered Surgical protocol (FACES) was implemented. Subjects were evaluated at least one month postoperatively. Outcome variables were jaw position, facial projectio

Posttraumatic mid-facial pain and Meige's syndrome relieved by pressure on the nasion and retrocollis.

A 42-year-old farmer developed persistent mid-facial segmental pain and Meige's syndrome several months after suffering facial trauma and a fracture of the nose. He was not afflicted by systemic ailments, had no family history of movement disorder and no history of exposure to neuroleptic drugs. He was capable of suppressing his facial pain by performing a ritual that included forcefully tilting his head backwards, lowering of his eyelids and applying strong pressure to his nasion. Exceptionally dystonic movements and elaborate behavioral rituals may serve as a mechanism of pain suppression.

Orthodontic treatment for oral rehabilitation after multiple maxillofacial bone fractures.

We present the orthodontic treatment of a patient with occlusal dysfunction after plastic surgery for multiple maxillofacial bone fractures caused by a traffic accident. The patient had mandibular deviation to the right because of inappropriate repositioning and fixation of the fractured bone and complete avulsion of both mandibular central incisors. The bilateral mandibular incisors, canines, and premolars were also suspected of partial avulsion or alveolar bone fracture. Several tests, including percussion and dental computed tomography, were performed on these teeth to rule out ankylosis an

Pediatric facial burns.

Despite major advances in the area of burn management, burn injury continues to be a leading cause of pediatric mortality and morbidity. Facial burns in particular are devastating to the affected child and result in numerous physical and psychosocial sequelae. Although many of the principles of adult burn management can be applied to a pediatric patient with facial burns, the surgeon must be cognizant of several important differences. Facial burns and subsequent scar formation can drastically affect the growth potential of a child's face. Structures such as the nose and teeth may become deform

Augmentation in two stages of atrophic alveolar bone prior to dental rehabilitation: a case report.

AIM: The aim of this report is to describe a significantly deficient case of alveolar bone that was managed by alveolar bone augmentation using a technique of distraction osteogensis and onlay bone grafting prior to dental implant placement. BACKGROUND: Injury to the teeth and alveolar ridge of the maxillary anterior region can cause a severe alveolar ridge deficiency resulting in ridge atrophy and maxillary retrognathism. The loss of these teeth and alveolar bone together with fibrotic scar formation can result in adverse changes of the interarch space, occlusal plane, arch relationship, and

Complications of the orbital floor and maxillary sinus 30 years after Coe-Pak misplacement in the management of pan-facial fractures.

We present the case of a woman who had sustained pan-facial fractures in a road traffic accident 30 years previously, and describe the ensuing unusual problems with the orbital floor and maxillary sinus as a consequence of unrecognised misplacement of a dental periodontal dressing material into the sinus. The subsequent management is discussed.

Computer-assisted secondary reconstruction of unilateral posttraumatic orbital deformity.

Until now, computer-assisted surgery has not been practiced as part of the surgical routine of posttraumatic orbital reconstruction. The purpose of this study was to investigate the use of a navigation system for computer-assisted preoperative planning with virtual reconstruction to obtain symmetry of the orbits and intraoperative control of virtual contours in comparison with the clinically achieved surgical results. A further objective of the computer-assisted orbital analysis was to use an ideal measurement for the two-dimensional and three-dimensional changes following orbital reconstructi

Assessment, documentation, and treatment of a developing facial asymmetry following early childhood injury.

Prepubertal trauma is often implicated as the cause of asymmetric growth of the mandible. A series of photographs taken before and after early childhood injury to the orofacial complex illustrates the development of a three-dimensional dentofacial deformity in a patient. The diagnosis and combined surgical orthodontic treatment plan to correct the facial asymmetry and malocclusion are discussed.

[Developmental disorders of the permanent dentition following fractures of the visceral cranium during the growth period. Incidence and morphological findings].

The retrospective examination of 70 patients who had suffered fractures in tooth-bearing segments of the maxilla and the mandible when they were between one and twelve years old revealed a high incidence of late pathologic changes (in 39 out of 70). Disorders in odontogeny, eruption of the teeth and occlusion were observed. Primary and secondary loss of teeth did also occur. Our results show the necessity to inform both the young patients and their parents about the high incidence of late changes after jaw fractures in childhood. An effective recall-system is required in order to diagnose and

[Computerized tomography of the facial skeleton (part II). Pathological changes (author's transl)].

The value and main task of computerized tomography in the diagnosis of benign and malignant processes in the middle and upper facial skeleton lies in its accurate demonstration of the extent of the disease; this is important in relation to the planning of treatment, prognosis and to other complementary procedures. CT is a new and potent method for the TNM classification of tumours of the nose, paransal sinuses and nasal pharynx. The ability of the CT to demonstrate deeply situated structures in the face, without disturbing overlying shadows, makes precise staging of deep tumours possible, eg.

Correlation between Malocclusion and Mandibular Fractures: An Experimental Study Comparing Dynamic Finite Element Models and Clinical Case Studies.

Mandibular fractures are very common in maxillofacial trauma surgery. While previous studies have focused on possible risk factors related to post-operative complications, none have tried to identify pre-existing conditions that may increase the risk of mandibular fractures. We hypothesized, through clinical observation, that anatomical conditions involving poor dental contacts, such as malocclusions, may increase the risk of mandibular fractures. This work was subdivided into two parts. In the first part, Digital Imaging and Communications in Medicine (DICOM) data of four healthy patients cha
