Radiological characteristics of myelin oligodendrocyte glycoprotein antibody disease.

BACKGROUND: MOG antibody disease is an autoimmune disease of the central nervous system (CNS) characterized by the presence of a serological antibody against myelin oligodendrocyte glycoprotein (MOG). MRI is instrumental in distinguishing neuromyelitis optica spectrum disorder (NMOSD) from multiple sclerosis (MS), but MRI features of MOG disease appear to overlap with NMOSD and MS. OBJECTIVES: In this study we aim to characterize the radiological features of MOG antibody disease and compare the findings with those previously described. METHODS: This is a retrospective study of 26 MOG positiv

Myelin oligodendrocyte glycoprotein antibody-associated demyelination: comparison between onset phenotypes.

BACKGROUND AND PURPOSE: The aim of this study was to analyse the clinical and prognostic features of myelin oligodendrocyte glycoprotein (MOG) antibody-associated demyelination with different onset phenotypes. METHODS: A total of 52 MOG-IgG-seropositive patients were divided into four groups: (i) optic neuritis (ON) at onset (MOG-ON+ , n = 23), (ii) transverse myelitis (TM) at onset (MOG-TM+ , n = 12), (iii) pure brain symptoms at onset (MOG-ON- -TM- , n = 14) and (iv) both ON and TM at onset (n = 3). This final group was not included in further analyses. Data were collected through medical r

Etiologic spectrum and prognosis of longitudinally extensive transverse myelopathies.

BACKGROUND: Patients with a first episode of longitudinal extensive transverse myelopathy (LETM) were reviewed with two objectives: to evaluate the clinical spectrum of LETM and to analyze the related clinical and laboratory variables that can be used as functional prognostic markers. METHODS: A retrospective review was conducted of clinical, radiologic and biochemical data of patients admitted for LETM between 1993 and 2011. RESULTS: Our cohort included 72 patients [median age 41 years, interquartile range (IQR) 29-61.5]. Median follow-up was 34 months (IQR 17.2-63). The modified Rankin Sca

Neuromyelitis optica with CSF examination mimicking bacterial meningomyelitis.

Neuromyelitis optica (NMO) is a rare inflammatory demyelinating disease of the central nervous system that causes severe attacks of optic neuritis and myelitis. Abnormal CSF findings are one of the characteristics of NMO that help to distinguish it from classical multiple sclerosis. Here we describe two cases of Devic's syndrome with CSF findings suggestive of bacterial meningomyelitis.

MOG-IgG is rare in AQP4-IgG seronegative NMO phenotype in Brazil.

BACKGROUND: Neuromyelitis optica spectrum disorder (NMOSD) is a rare autoimmune disease most frequently characterized by a neuromyelitis optica (NMO) phenotype, comprising both simultaneous or sequential optic neuritis (ON) and longitudinally extensive transverse myelitis (LETM). Symptoms of brainstem, diencephalic and cerebral involvement may also occur. While most NMOSD patients test positive for serum aquaporin-4 (AQP4) antibodies, some seronegative patients test positive for oligodendrocyte glycoprotein-IgG (MOG-IgG). Early identification of seropositive MOG-IgG seropositive patients among

Is spinal angiography useful in the differential diagnosis of myelopathies of unknown origin? Analysis of a series of 93 patients.

BACKGROUND: Myelopathies of unknown origin (MUO) constitute a challenge for the clinician, due to the absence of image findings that explain the symptoms of the patients. The role of spinal angiography (SA) in MUO has not been studied before in the medical literature. METHODS: Retrospective study of 93 SA performed in a neurointerventional reference center in a ten-year period (January 2013-December 2022). Suspected, angiopraphic and final diagnosis for each patient were studied and compared. RESULTS: SA constituted an 0.8% of the total neurointerventional procedures performed (93 out of 109

Neurobrucellosis: A Narrative Review.

BACKGROUND: Neurobrucellosis is a rare but serious complication of systemic Brucella infection, occurring in approximately 3-15% of cases and accounting for about 0.5% of all community-acquired central nervous system (CNS) infections. Although uncommon, it poses significant diagnostic challenges and carries the risk of long-term neurological sequelae. OBJECTIVE: This review provides a comprehensive overview of the epidemiology, pathogenesis, clinical manifestations, diagnostic strategies, and management of neurobrucellosis, with an emphasis on current evidence and diagnostic difficulties. ME

Systemic lupus erythematosus with acute ischemic optic neuropathy complicated with neuromyelitis optica: a case report.

BACKGROUND: Neuromyelitis optica is a relapsing-remitting disease characterized by a recurrent attack of optic neuritis and transverse myelitis; sometimes associated with acute brainstem syndrome. Systemic lupus erythematosus is an autoimmune multisystem disorder in which ocular involvement such as acute ischemic optic neuropathy is a rare manifestation. However, neuromyelitis optica can be associated with systemic lupus erythematosus. CASE PRESENTATION: A 24-year-old Bangladeshi woman was admitted to the hospital with complaints of sudden, progressive, painless vision loss in both eyes, and

Serum and Cerebrospinal Fluid Biomarkers in Neuromyelitis Optica Spectrum Disorder and Myelin Oligodendrocyte Glycoprotein Associated Disease.

The term neuromyelitis optica spectrum disorder (NMOSD) describes a group of clinical-MRI syndromes characterized by longitudinally extensive transverse myelitis, optic neuritis, brainstem dysfunction and/or, less commonly, encephalopathy. About 80% of patients harbor antibodies directed against the water channel aquaporin-4 (AQP4-IgG), expressed on astrocytes, which was found to be both a biomarker and a pathogenic cause of NMOSD. More recently, antibodies against myelin oligodendrocyte glycoprotein (MOG-IgG), have been found to be a biomarker of a different entity, termed MOG antibody-associ

Central nervous system adverse events after ChAdOx1 vaccination.

INTRODUCTION: Post-ChAdOx1 vaccine (AZD1222) adverse events following immunization (AEFI) are uncommon. Recently described neurological events include thrombocytopenia with thrombosis syndrome (TTS) with cerebral venous thrombosis and Guillain-Barré syndrome. There are very few AEFI reports following COVID vaccination from India, because of underreporting or other factors. A few cases of acute transverse myelitis (ATM) and post-vaccinal encephalitis have also been reported. MATERIALS AND METHODS: Over 11 months, in 2 districts of Kerala, India, 8.19 million people were vaccinated with the ChA

Refractory hyponatremia in neuromyelitis optica in a pediatric patient: A case report.

RATIONALE: Neuromyelitis optica spectrum disorders (NMOSD) is a rare autoimmune disease predominantly involving optic nerves and spinal cord, and possible comorbidities including syndrome of inappropriate antidiuretic hormone secretion or urinary complication. We reported a young girl diagnosed with NMOSD presented with refractory hyponatremia, acute urine retention, and general weakness. Clinical symptoms improved gradually after receiving intravenous immunoglobulin, high-dose methylprednisolone, and plasmapheresis. NMOSD should be kept in mind in adolescence with acute urine retention, inter

Patients with neuromyelitis optica spectrum disorder (NMOSD) are associated with adverse outcome after total hip arthroplasty: a matched case-control study.

BACKGROUND: Neuromyelitis Optica Spectrum Disorders (NMOSD) are rare inflammatory diseases of the central nervous system that cause transverse myelitis and optic neuritis. Steroids are commonly administered in NMOSD patients. The use of steroids may lead to osteonecrosis, which makes some of the NMOSD patients candidate for total hip arthroplasty (THA). To date, the clinical outcome of THA in NMOSD patients have not been investigated. AIM: Investigate the patient reported outcome measures (PROM), radiographic outcome and complication in NMOSD patients after THA, compared with that of non-NMOS
