Indwelling self-retaining ureteral stents are important for the maintenance of urinary drainage in the ureter threatened by obstruction by neoplasm, edema, or calculus. Proximal migration of the stent into the ureter prevents simple cystoscopic retrieval and necessitates removal by other techniques. In this series both rigid and flexible ureteroscopes were used to remove stents from locations ranging from the renal pelvis to the distal ureter in 16 patients. Three-pronged wire graspers was the retrieval instrument most frequently used. Stents were removed in all patients without complication.

In a review of 549 cases of breast cancer in the last 16 years, 7 cases with lobular carcinoma have been studied. All of the patients appeared well and were without recurrence. Two cases had been lobular carcinomas in situ, one of which had arisen in a small fibroadenoma associated with malignant phyllodes tumor in the opposite breast, and the other had shown multicentric foci of a lobular neoplasm. Computed tomography (CT) was used in the evaluation of nine patients with nonopaque calculi in the upper urinary tract. In each case the calculus was identified as a very high density object (370-586 Hounsfield units) with calcium oxalate and cystine stones having somewhat higher attenuation values than uric acid or xanthine stones. The differentiation between calculi and other "radiolucent" filling defects was readily made since calculi had much higher attenuation values than blood clot or neoplasm. We describe a rare pattern of urothelial carcinoma (UC) invasion consisting of large, irregular to regular nests, with bland cytology. We prospectively retrieved 23 cases of large nested UC from one of the author's consult files (2001 to 2010). The mean patient age was 63.7 years (39 to 89 y), and 86% were men.
