Auditory brainstem response abnormalities in adductor spasmodic dysphonia.

Six patients with adductor spasmodic dysphonia were evaluated for evidence of brainstem dysfunction using the auditory brainstem response (ABR). In four, abnormally large increments in wave V latencies were found at high stimulus repetition rates. Latency increments exceeded three standard deviations above the mean in a normative adult population. A fifth patient had the prolonged interpeak latencies described previously by other investigators. In five of six patients the capacity of the auditory brainstem to conduct impulses was compromised. These results suggest a central nervous system abno

Congenital clubfoot: an electromyographic study.

Twenty-five patients with congenital idiopathic clubfoot, aged 1 day to 22 months, were studied electromyographically. No myopathic or neurogenic changes were found in tibialis anterior, peroneus, gastrocnemius, extensor digitorum brevis, and abductor hallucis brevis muscles of the leg and foot. Maximum motor nerve conduction velocities for the peroneal and tibial nerves were normal. It is concluded that conventional electromyographic techniques were unable to demonstrate abnormalities suggesting neuropathic or myopathic changes in idiopathic clubfoot.

Severe peripheral neuropathy after exposure to monosodium methyl arsonate.

A case of severe peripheral neuropathy after several days of exposure to a pesticide spray containing monosodium methyl arsonate (MSMA) is reported. The clinical manifestations of symmetrical peripheral neuropathy with stocking-glove sensory deficit, decreased position sense, decreased and absent deep tendon reflexes, and muscle wasting are consistent with those described in other cases of arsenic intoxication. The anemia, leukopenia, and bone marrow changes of dyserythropoiesis in this case are also similar to those previously described with arsenic intoxication. The authors discuss the possi

Treatment of Guillain-Barré syndrome by plasma exchange.

Plasma exchange has been used for therapy in eight patients with the Guillain-Barré syndrome. All patients were severely ill. They became tetraplegic and showed cranial nerve involvement. Five patients received assisted respiration, but the others were also at risk of ventilatory insufficiency. Recovery was abrupt in all cases after the first plasma exchanges. Improvement was more marked when plasmapheresis was done on three successive days with plasma exchanges of 2.0-3.01 each in the initial progressive stage of the disease. A considerable advantage of this therapy is the avoidance of contin

Electrodiagnostic study of carpal tunnel syndrome after Colles fracture.

Thirty-three patients with unilateral Colles fracture were evaluated by clinical and electrodiagnostic methods to determine the frequency of concomitant ipsilateral carpal tunnel syndrome. Four cases were documented, an incidence of 12.1%; this is two to four times greater than the incidence previously reported in the orthopaedic literature. Four other cases showed bilateral carpal tunnel syndrome, on of which was more more severe on the fracture side. Although electrodiagnosis was not found to be a more sensitive diagnostic indicator than was careful history and physical examination is these

On the number and nature of regenerating myelinated axons after lesions of cutaneous nerves in the cat.

1. Electrophysiological and anatomical techniques were used to investigate normal and regenerating sural and posterior femoral cutaneous nerve fibres in the cat. 2. One and a half years after transection of these nerves it was found that the regenerating neurones supported multiple sprouts in the distal stump of the nerve. The branching occurred at or beyond the level of the neuroma and some of the branched fibres innervated split receptive fields on the skin. 3. Counts of the number of axons in the proximal stumps of transected nerves showed that the whole original population of myelinated fi

[Carpal tunnel syndrome: electroneuromyographic study of 135 cases].

One hundred thirty five patients with carpal tunnel syndrome were studied trough electrophysiological techniques. Nerve sensory conductions, motor conductions and electromyography were performed in all patients and the results discussed. The authors call the attention for the technic of sensory conduction stimulating palma and picking up the potentials on the wrist as the most sensitive technic for the early detection of carpal tunnel syndrome.

Neurogenic benign fasciculations, pseudomyotonia, and pseudotetany. A disease in search of a name.

We studied two patients with abnormal spontaneous muscular activity. The first had widespread fasciculations, painful spasms, delayed muscular relaxation, and hyperhidrosis. Improvement occurred after several years. The second case had generalized paresthesia, mild stiffness, a positive result from Trusseau's test, and was relieved by administration of carbamazepine. Both patients had abnormal conduction velocity. Examination of muscle biopsy specimens disclosed fiber type grouping and increased collateral ramification of motor axons. These observations exemplify symptoms and signs that resemb

Ophthalmoplegia plus. A multisystem disorder of unknown etiopathogenesis.

Six cases of Ophthalmoplegia Plus (OP) have been studied: all the patients had palpebral ptosis, ophthalmoparesis and descending myopathy. Hypoacusis, cardiac conduction impairment, small stature, mental deficit and vestibulo-cerebellar dysfunction were present only in some cases. EMG showed a severe slowing of motor and sensory conduction velocity in one patient. Polyphasic and long duration MU action potentials, which are indicative of a neurogenic myopathy, were found in all cases. Every muscle biopsy showed many "ragged red fibers" which, in EM, appeared to contain abnormal mitochondria. A

Interactions between extraocular proprioceptive and visual signals in the superior colliculus of the cat.

1. The responses of units in the superficial layers of the superior colliculus to stretch of extrinsic ocular muscles (e.o.m.) and to visual stimuli, delivered singly and paired at various inter-stimulus intervals, were studied in chloralose-anaesthetized cats. 2. Most units responded to visual stimuli and about half also gave phasic excitatory responses to stretch of e.o.m. 3. Signals from the e.o.m. of each eye reach both superior colliculi; only those in the colliculus ipsilateral to the e.o.m. stretched were studied in detail. 4. A variety of control experiments provided evidence that the

[Medullar and supraspinal conduction in multiple sclerosis: localization value (author's transl)].

Spinal cord and central conduction velocities were studied in 12 patients with multiple sclerosis and in 20 healthy volunteers using an indirect method (Dorfman's) that includes the determination of the somatosensory evoked potentials by median and posterior tibial nerves stimulation, latency of the F-waves and motor and sensory conduction velocities in the wrist-elbow segment of the median nerve. Patients with symptoms and signs of demyelination showed very altered conductions in both of the segments studied, but five patients without clinical suspicion of sensory alteration showed normal spi

Distal neuropathy in experimental diabetes mellitus.

Although nerve conduction slowing is a well-accepted abnormality in rats with acute experimental diabetes, reports of neuropathological changes in diabetic rat nerves have been inconsistent. To examine this further, we studied electrophysiological and morphological features of posterior tibial nerves and their distal branches from four-week streptozotocin-induced diabetic rats and matched controls. Diabetic rat posterior tibial motor conduction was slowed (mean +/- 1 SD, 34.8 +/- 3.1 m/sec; controls, 41.2 +/- 2.5 m/sec), and evoked muscle response amplitudes were only half of control values. U

[Composition and excitability of afferent fibers of the pelvic nerve in cats].

The afferent fibers excitable by pelvic nerve stimulation were as follows: A (81 m/sec), A (59.6 m/sec), A (45.3 m/sec), A (17.9 m/sec), A (31.6 m/sec), B (11.3 m/sec). We failed to find C fibers. Analysis of the distribution of fibers showed that the majority of them entered the S2 dorsal root (DRS2). The fast-conducting fibers (Abeta1 and Abeta2) were found at DRL7--DRS2. The number of slow-conducting fibers increased in caudal direction. The most abundant in the pelvic nerve are Abeta3 fibers, and somewhat fewer in number are Abeta and Agamma fibers.

Effect of parathyroid hormone and uremia on peripheral nerve calcium and motor nerve conduction velocity.

Peripheral neuropathy is not an uncommon complication of chronic uremia. Because parathyroid hormone, by raising brain calcium, is partly responsible for central nervous system aberrations in uremia, we studied the relative role of uremia, per se, and(or) parathyroid hormone on peripheral nerve calcium and motor nerve conduction velocity (MNCV). Studies were made in six groups of six dogs each, as follows: (a) normal dogs, (b) thyroparathyroidectomized (T-PTX) animals, (c) dogs with 3 days of uremia produced by bilateral nephrectomy, (d) T-PTX before the induction of acute renal failure, (e) n

Peripheral neuropathy with vitamin B12 deficiency.

Seven patients with neuropathy associated with vitamin B12 deficiency are reported. Four of them had other signs of malabsorption aside from the abnormal Schilling test. The neuropathy was diagnosed on the basis of the whole clinical picture and the neurophysiological findings. The pathogenesis of the peripheral nerve disease is discussed in the light of the evidence in the literature.

[The Hoffmann reflex in the normal new-borns. Post-natal evolution of the conduction velocity of the alpha motor and the Ia sensitive fibres of the ulnar nerve (author's transl)].

The conduction velocities of the alpha motor fibres and the IA sensitive fibres of the ulnar nerve have been studied in a group of full term newborn babies and during the neonatal period. At birth the average conduction velocity of the alpha motor fibres is 25,7 m/sec (sigma = to 2,4 m/sec) that of the IA afferent fibres is 35 m/sec (sigma = to 3,2 m/sec). The post natal evolution of the conduction velocity as a function of age has been established for both types of fibres. The time required for nervous conduction between the olecranon process and the wrist diminishes clearly in the course of

[Antidromic sensory action potentials of the median and ulnar nerve.--Normal value and discussion of methodology (author's transl)].

The sensory conduction velocities of the median and ulnar nerve and the amplitudes of the evoked nerve action potentials were measured and evaluated statistically in 116 healthy subjects with the antidromic technique. Both conduction times and amplitudes were correlated to age and to the nerve segment studied (finger--wrist or wrist--elbow). Moreover amplitudes varied with the distance between the recording electrodes; in spite of their great interindividual differences they may be of diagnostic help. In clinical electrophysiology this method is useful in revealing especially minor injuries of

Tuning of single fibers in the cochlear nerve of the alligator lizard: relation to receptor morphology.

(1) The general anatomy of the peripheral portion of the cochlear nerve in the alligator lizard is described. (2) Spike discharges of single units were recorded with micropipets placed in the peripheral portion of the cochlear nerve of anesthetized lizards. (3) In response to tone bursts, each unit is maximally sensitive to a charactertistic frequency (CF). There are two distinct populations of units having different CFs: a low CF population (CF in the range 0.2-0.8 kHz) recorded in the portion of the nerve that enters the apical region of the basilar papilla and a high CF population (CF in th
