The Addition of a Temporary Intramedullary K-wire to Facilitate Plating for Metacarpal Fractures.

Open reduction internal fixation with plate fixation is commonly used in treating metacarpal fractures to facilitate early rehabilitation. However, it is sometimes challenging to maintain a satisfactory 3-dimensional alignment during the plating process. We present a method using a temporary centrally placed intramedullary k-wire to maintain a fundamental stability for reduction of metacarpal fractures. This method facilitates the plating process and simplifies the open reduction internal fixation of the metacarpal fractures.

A stepwise algorithm for surgical treatment of type II displaced pediatric phalangeal neck fractures.

PURPOSE: To evaluate a stepwise reduction algorithm including closed, percutaneous, and open reduction techniques followed by percutaneous pin fixation for displaced pediatric phalangeal neck fractures. METHODS: Sixty-one consecutive children (mean age, 9.4 y; range, 2-18 y) presenting with closed, type II displaced phalangeal neck fractures were treated using the following algorithm. If satisfactory reduction was achieved with closed reduction, percutaneous pinning (CRPP) was performed. If not achieved, then percutaneous reduction and pinning (PRPP) was performed using a temporary intrafocal

Arthroscopic Management of a Coracoid Fracture Associated With Acromioclavicular Dislocation: Technical Note.

Operative management of a coracoid process fracture is indicated in case of painful nonunion, displacement of more than 1 cm, or multiple disruptions of the superior shoulder suspensory complex. Several techniques have been described with open reduction of the fracture and internal fixation using cortical screws with or without additional fixation of the acromioclavicular joint. This Technical Note aims to introduce an alternative safe, minimally invasive method for arthroscopic fixation of a coracoid fracture with simultaneously reduction of the acromioclavicular joint. The described arthrosc

Biepicondylar fracture presenting with elbow dislocation: a case report.

INTRODUCTION: Biepicondylar fracture of the elbow is very rare, and to date there have only been three reports of this injury and its treatment in the English scientific literature. This case report evaluates the surgical internal fixation of a biepicondylar fracture of the elbow with an associated dislocation. CASE PRESENTATION: We report the case of a 15-year-old Turkish girl with a biepicondylar fracture dislocation of the left elbow. Open reduction and an internal fixation operation were applied. There were no complications. CONCLUSION: In these injuries, open reduction and internal fixa

Open radiocarpal fracture-dislocations.

Ten open radiocarpal fracture-dislocations in nine patients were treated by wound debridement, open reduction, and maintenance of reduction by casting supplemented with internal or external fixation. Eight patients had associated fractures or dislocations of the ipsilateral extremity: elbow fracture or dislocation (three patients), shoulder fracture (one patient), fractured humerus (two patients), and phalangeal fractures (three patients). In seven of ten cases the median and/or ulnar nerve was severely contused and required immediate decompression. Six patients were followed at least 15 month

Fixation of femoral head fractures using the modified heuter direct anterior approach.

Large displaced fractures of the femoral head require open reduction and internal fixation. If the displaced fragments are small, open reduction may not be possible and excision of the fragment or nonoperative care may be the treatment of choice. Surgical approaches that have been used for open reduction and internal fixation include the Smith-Petersen approach, Watson-Jones approach, Kocher-Langenbeck approach, a posterior approach with a trochanteric flip osteotomy, and surgical dislocation of the hip. Here, we describe the use of the modified Heuter direct anterior approach for open reducti

Transscaphoid perilunate fracture dislocation and pseudarthrosis of the scaphoid.

The results of conservative treatment of a perilunate fracture dislocation were retrospectively studied. In six patients, the treatment consisted of closed reduction and immobilization in a below the elbow cast for 12 to 16 weeks. Five patients had a compromised healing of the scaphoid fracture. The frail vascularization of the scaphoid, extensive ligamentous injury and possible mid-carpal instability in case of perilunate fracture dislocation as well as a non-anatomical reduction of a scaphoid fracture are causative factors in ensuing scaphoid pseudarthrosis. If anatomical reduction of a peri

Unstable ankle fractures: comparison of closed versus open treatment.

To determine what factors were important in final clinical results, we evaluated 145 patients admitted to Vanderbilt University Hospital with ankle fractures between January 1971 and December 1976. There was a strong correlation between anatomic results of reduction and final clinical results. Talar displacement of more than 5 mm before reduction, a posterior malleolus fracture involving more than 25% of the articular surface, an anterior tibial plafond fracture involving more than 25% of the articular surface, and talus fractures involving the articular surface were four variables which signi

Incision Selection in Mandibular Angle ORIF: Postoperative Complications and Neurosensory Outcomes by Approach.

BACKGROUND: Mandibular angle fractures carry disproportionate postoperative morbidity, yet optimal incision selection remains controversial despite its clinical importance. OBJECTIVE: The aim of the study was to evaluate whether incisional approach, particularly trocar-assisted transbuccal access, is independently associated with any postoperative complication, including postoperative paresthesia, after ORIF of mandibular angle fractures. METHODS: A retrospective cohort of consecutive patients undergoing ORIF for mandibular angle fractures was analyzed with respect to incision approach, incl

Locked Metacarpophalangeal Joint of the Little Finger in Extension Position due to the Incarceration of a Chronic Fracture Fragment: A Case Report.

We managed a case of locked metacarpophalangeal joint of the little finger in the extension position. Incarceration of a chronic osteochondral fracture fragment led to this unique condition. The fracture fragment partially adhered to the volar plate and ulnar collateral ligament on the joint side, which is supposed to have resulted in manually irreducible locking of the joint. We performed open reduction and achieved release of the locked joint by excising the fracture fragment.

Fractures of the basal joint of the thumb.

The two important fractures of the trapeziometacarpal joint are Bennett's fracture and Rolando's fracture. Bennett's fracture is a fracture-dislocation of the joint. The preferred treatment is closed reduction and percutaneous pinning of the fracture with additional fixation through the trapeziometacarpal joint verified by anteroposterior (AP) and lateral roentgenograms. If the fracture is not reducible, and open reduction through the thenar approach with Herbert screw fixation is recommended. Rolando's fracture is a comminuted T- or Y-shaped fracture through the base of the first metacarpal.

[Nasal fractures--indications for open reposition].

BACKGROUND: Many septoplasties and septorhinoplasties are indicated due to previous traumatic etiologies, and the typical treatment of these septal and/or bone fractures has been closed reduction. Review of the literature reveals a success rate of only between 30-82%. One of the reasons for these poor outcomes is the influence of tension and pressure stress vectors due to hidden cartilage fractures behind intact septal mucosa. This is the main indication for open reduction of nasal trauma. The present study analyses the results of our experience with the open reduction of nasal fractures. MET

Hoffa fracture in a 14-year-old.

Coronal shear fractures of the femoral condyle (ie, Hoffa fracture) are an uncommon clinical entity typically seen in adults after higher-energy trauma. Historically poor outcomes have been reported in the literature with nonoperative treatment of these fractures. Conversely, open reduction and internal fixation of these fractures has been shown to produce good long-term clinical results in adults. These fractures appear to be even more uncommon in skeletally immature patients, with only 3 case reports documented in the literature to date. Two of the 3 cases presented as a symptomatic nonunion

Intraoperative Foot Positioning During Percutaneous Calcaneal Fixation: A Technical Note.

Percutaneous screw fixation is a good modality for operative management of extra-articular and some intra-articular fractures of the calcaneum amenable to closed reduction. Tongue-type calcaneal fractures with a dislocated posterior facet are usually treated with percutaneous fixation. When treating calcaneal fractures with substantial soft tissue compromise, particularly open fractures, percutaneous reduction techniques are crucial. They also provide patients with local or systemic contraindications to open reduction with a therapeutic option. We describe the intraoperative positioning of the

Intraoral Approach for Zygomaticomaxillary Complex and Orbital Floor Fracture With a Customized Ballooning Technique.

The zygomaticomaxillary complex and the orbital floor are commonly affected by mid-face fractures; their fractures pose challenges in reconstruction and can result in complications, such as enophthalmos. This study explores an intraoral approach with the help of a customized balloon technique and a contrast agent as an alternative to the conventional methods. A case report of a 67-year-old man with complex midfacial trauma is presented. The surgical process involved open reduction through an intraoral approach along with a transantral approach with the customized balloon technique. The procedu

Management of a 15-year-old scaphoid non-union.

Scaphoid non-union affects wrist joint function and is often associated with a predictable pattern of degenerative change. A 30-year-old man presented with a symptomatic 15-year-old right scaphoid fracture with secondary scaphoid non-union advanced collapse of his right wrist. There was no evidence of avascular necrosis. The initial injury was from a fall onto his outstretched hand. We performed a right scaphoid open reduction, internal fixation and iliac crest bone graft. One year after the operation, the patient had good wrist function and grip strength. A CT scan showed union of the fractur

Infraorbital nerve function following treatment of orbitozygomatic complex fractures. A multitest approach.

Sensory disturbance following orbitozygomatic complex fractures was studied in 65 patients in relation to type of fracture and method of treatment. The fracture-type-dependent treatments were: no surgical intervention (n = 20), closed reduction with or without wire fixation (n = 17), open reduction with miniplate fixation (n = 15) and/or reconstruction of the orbital floor (n = 13). Several methods were applied to assess sensory function, on average 6.3 months after treatment, i.e. the patient's report and tests regarding touch, two methods of two-point discrimination, and cold, all applied on
