[A case of central pontine and extra-pontine myelinolysis demonstrated by magnetic resonance imaging].

A 74-year-old woman was admitted because of disturbed consciousness due to iatrogenic hyponatremia. At admission, her serum level of sodium … ith Tl and Gd enhanced images. On Tl images areas of low signal intensity were found in the central pons and left putamen, which were more clearly demonstrated by Gd enhanced MRI images. These lesions were considered to be consistent with central pontine and extra-pontine myelinolysis (CPEM). In addition, a part of the right temporal cortex was enhance …

[Lesions of the brain stem and cerebellum of alcoholic and nutritional deficiency origin].

Lesions of the brain stem and cerebellum due to toxic substances or nutritional deficiencies are mostly seen in chronic alcohol abuse and, m … o malnourished subjects who receive glucose infusions. Cerebellar atrophy of alcoholic origin is responsible for subacute ataxia which, as a rule, is definitive. Central pontine myelinolysis is rare and presents as sudden quadriplegia with pseudobulbar palsy and sometimes "locked-in" syndrome. Beside alcohol, a frequent cause is major ionic disorders, …

[Olivopontocerebellar atrophy in the context of progressive cerebellar atrophies (a clinico-anatomical study)].

The study of 15 cases of progressive cerebellar atrophies, and especially of the olivopontocerebellar atrophy, that was investigated both clinically and anatomically, has attempted to evidence particularities and correlations existing between these two types of atrophy. Olivopontocerebellar atrophy appears to be an abiotrophy of the cerebellum, considered to be spontaneous, sporadic, and sometimes with a hereditary familial background. It is a systemic disease, predominantly of the neocerebellum and involving the cerebellopedal system. It is at the opposite end of Holmes-type atrophies, and of

Bilateral horizontal gaze paralysis due to pontine hemorrhage. A case report.

In a case of bilateral horizontal gaze paralysis, vertical gaze was clinically intact but eye movement recordings demonstrated a transient reduction of vertical saccadic velocities. Horizontal caloric vestibulo-ocular responses were absent. CT scanning and NMR imaging showed a hematoma in the median pontine tegmentum. This case provides additional evidence that vertical and horizontal saccade genesis may be independent of caudal paramedian pontine reticular formation lesions.

Primary pontine hematoma presenting as a cerebellopontine angle mass.

A case of hemipontine hematoma from the rupture of a cryptic vascular malformation presenting as a cerebellopontine angle tumor is reported. The clinical features of and surgical approaches to partial pontine hematomas are briefly reviewed.

Thrombosis of a saccular microaneurysm causing cerebral (pontine) lacunae.

The authors present the case of a hypertensive patient with a thrombosis of a saccular microaneurysm (SMA) and underlying cerebral (pontine) lacunae. This SMA lay at the junction between the main vessel and two small feeding arteries penetrating into the necrotic territory. Microhemorrhages were observed in and outside the aneurysmal wall. Very sinuous narrow channels of recanalization crossed the thrombosis between the upstream and downstream vessels. The anatomical relationship between the SMA and the lacunae has been histologically demonstrated.

Lacunar syndrome due to intracerebral hemorrhage.

It has been recognized that small intracerebral hemorrhage not uncommonly produced lacunar syndromes. In this study, we examined cases of intracerebral hemorrhage presenting as lacunar syndromes. Of 174 cases with recent intracerebral hemorrhage, 19 presented with a lacunar syndrome: 4 presented with pure motor hemiparesis, 5, ataxic hemiparesis, 3, dysarthria-clumsy hand syndrome, 7, sensorimotor stroke, and, none, pure sensory stroke. The sites of hemorrhage were capsular in 11, putamenal in 6, and pontine in 2. In these 19 patients, 17 were hypertensive, and the signs characteristic of pare

[Microtopography of cerebellopontine angle structures in acoustic neuroma].

The structures of the cerebellopontine angle in neurinoma of the auditory nerve were studied in autopsy material and during surgical intervention with the use of an operative microscope. Principally different variants of the relationships between the cerebellopontine angle structures and the neurinoma capsule in intra- and extracisternal localization of the tumor were revealed. The greatest difficulties occur in exposure of the capsule of an intracisternal neurinoma which is tightly connected with the structures of the cerebellopontine angle by means of strong connective-tissue trabeculae. In

Pontine infarction with diffuse myelin destruction.

A 57-year-old man with clinical symptoms of dementia and spastic paraplegia revealed pathologically scattered cerebral arteriosclerotic changes and diffuse myelin destruction in the basis pontis. The latter was characterized by comcomitances with an eminent fibrous gliosis and a neuronal as well as an axonal loss. Pontine infarction and the following fibrous gliosis possibly caused a wide area of myelin destruction resembling cpm.

[Internuclear ophthalmoplegia in infectious encephalitis: observation on a case of listeriosis].

An internuclear ophthalmoplegia was diagnosed and recorded electro-oculographically in a patient with pontomedullary listeriosis. The etiological differential diagnosis in case of internuclear ophthalmoplegia is discussed. It is emphasized that listeria monocytogenes encephalitis should be considered because this pathology may be electively located in the pontomedullary area and because of the encreasing frequency of human listeriosis.

The pontocerebellar projection onto the paramedian lobule in the cat: an experimental study with the use of horseradish peroxidase as a tracer.

Horseradish peroxidase (HRP) was injected into cerebellar cortex of the paramedian lobule in 12 cats, and the ensuing distribution of labele … s projecting onto the paramedian lobule with the terminal areas of pontine afferents shows that the pons may be a relay station in mediating influences from other parts of the cortex (MsI, visual and acoustic), the cerebellar nuclei and the colliculi to the paramedian lobule.

Central pontine myelinolysis and its arterial blood supply.

A case of central pontine myelinolysis is described. The clinical, angiographic and post mortem findings suggest that among the associated disorders the oligemic shock combined with a local decreased flow in the basilar artery was the most important cause of this patholigical entity. The angioarchitecture findings in the brain stem, compared to these in 26 normal brains, confirm this hypothesis.

Responses of respiratory neurons of the rabbit to some excitatory and inhibitory stimuli.

The effects of an afferent stimulation of the vagus nerve, hypercapnia and hyperventilation on the discharges of pontine, medullary and phrenic respiratory neurons were studied. The responses were investigated in spontaneously breathing, lightly anaesthetized rabbits with intact vagi, and subsequently during artificial ventilation, after vagotomy and under surgical anaesthesia. We found that the patterns of activity of respiratory neurons depend upon the integrity of vagal conduction, pattern of ventilation and depth of anaesthesia, and that vagotomy and deep anaesthesia markedly modify, and s…
