The diagnosis and treatment of schistosomiasis among migrants in nonendemic countries are still challenging. An online questionnaire was sent to adult and pediatric infectious disease specialists through the French infectious disease societies' mailing lists to assess attitudes and practices toward the disease. We included all individuals who responded to the online questionnaire but excluded from analysis residents and doctors with dual practices (adults and children) from the analysis. The questionnaire consisted of 19 questions about professional status, screening practices, diagnostic meth

INTRODUCTION: Exercise is beneficial for improving general health, wellbeing, and specific medical conditions. In musculoskeletal conditions such as chronic low back and neck pain, prescribed exercise has been found to be moderately effective in decreasing pain and improving function. Osteopaths are primary contact health professionals who manage predominantly musculoskeletal complaints. This work presents a secondary data analysis of the Australian osteopathy practice-based research network and profiles the characteristics of osteopaths who often use exercise prescription in patient care. OBJECTIVE: To provide contemporary estimates of internists' perceptions of adverse effects associated with proton pump inhibitors (PPIs) and self-reported clinical use.
METHODS: We invited 799 internists, including specialists and postgraduate trainees, to complete an online survey. Topics included perceptions of PPI adverse effects (AEs) and effectiveness for upper gastrointestinal bleeding (UGIB) prevention, changes in prescribing, and management recommendations for patients using PPIs for gastroesophageal reflux disease or UGIB prevention. We used logistic regression to identify factors as

OBJECTIVES: Choosing Wisely Canada (CWC) is an initiative to encourage patient-physician discussions about the appropriate, evidence based use of medical tests, procedures and treatments. We present the Canadian Association of Emergency Physicians' (CAEP) top five list of recommendations, and the process undertaken to generate them.
METHODS: The CAEP Expert Working Group (EWG) generated a candidate list of 52 tests, procedures, and treatments in emergency medicine whose value to care was questioned. This list was distributed to CAEP committee chairs, revised, and then divided and randomly all

BACKGROUND: This survey aimed to describe the current use of laryngeal mask (LM) and to define the difference of use according to a group defined like experts.
METHODS: A self-questionnaire available on Internet was sent by e-mail to French anesthesiologists. Results were compared by a Chi(2) test between anesthesiologists according to their expertise defined by a >750 LM use.
RESULTS: Nine hundred and fifty-three anesthesiologists answered to the survey. Only 19 reported never using LM and 37% of responders were defined as expert. More than half of the MAR used the ideal weight to set the s

AIM: Children with learning difficulties are commonly seen in Australian paediatric outpatient settings. The practice patterns of paediatricians in assessing, referring and managing these children are unknown, and there is no consensus on best practice. We thus aimed to examine the consistency between Australian paediatricians': (i) assessment; (ii) referral; and (iii) management of children presenting with learning difficulties.
METHODS: All 373 paediatrician members of the Australian Paediatric Research Network were invited to participate in an online survey in 2010. Paediatricians who saw

CONTEXT: Upper respiratory conditions are common and have a significant impact on patient quality of life, medical resource expenditure, and antibiotic use. Saline nasal irrigation (SNI) is an adjunctive therapy for upper respiratory conditions; clinical studies suggest that use of SNI may be effective for symptoms of upper respiratory conditions, and its popularity seems to be growing. The prescribing patterns of physicians regarding SNI have not yet been studied.
OBJECTIVE: To assess the use among family physicians in Wisconsin of SNI, determine how and for which conditions they recommend S

"Defensive" medicine is called medical behaviour characterized by deformation of diagnostic and therapeutic activities due to fears endangering existence and work, thus some interventions are omitted or, on the contrary, superfluous examinations are proposed on account of internal uncertainty, the patient's distrust or hostile social environment. Trust relation between patient and physician is the most gravely damaged because of aggravation and distortion of some conscienceless physicians' abuses by the media; patient-physician relations may not be degraded to contractual legal relations. BACKGROUND: Despite evidence-based guidelines, aspirin prescribing for the secondary prevention of stroke is sub-optimal. Little is known about why general practitioners do not prescribe aspirin to indicated patients. We sought to identify and describe factors that lead general practitioners (GPs) not to prescribe aspirin to eligible stroke patients. This was the first stage of a study exploring the need for and means of improving levels of appropriate aspirin prescribing.
METHOD: Qualitative interviews with 15 GPs in the West Midlands.
RESULTS: Initially, many GPs did not regard their presc

BACKGROUND: Urinary tract infections (UTIs) are common in primary care. Qualitative studies suggest that the way primary care practitioners (PCPs) manage UTIs may be influenced by a complex interplay of factors.
AIM: To explore the views, perceptions, and experiences of PCPs in the management of UTIs.
DESIGN AND SETTING: Qualitative evidence synthesis of studies set in general practice internationally.
METHOD: Four databases were systematically searched from inception to 28 June 2024, and forward citation searching of included studies was undertaken. Braun and Clarke's reflexive thematic an

IMPORTANCE: Clinical trial enrollment is essential to advancing cancer care, yet participation among patients with advanced cancer remains low. While patient-level barriers are frequently cited, the role of health care delivery factors-specifically physician- and practice-level variation-has been poorly quantified.
OBJECTIVE: To quantify variation in clinical trial participation and identify associated delivery system factors among patients with advanced cancer.
DESIGN, SETTING, AND PARTICIPANTS: This cohort study included adults with advanced or metastatic cancers diagnosed and treated betw

BACKGROUND: Organization for Economic Cooperation and Development (OECD) estimates suggest that 20% of health care spending is wasteful or even harmful. Previous interventions have had limited success in discouraging low-value care in medical practice.
METHODS: We conducted a nationwide randomized controlled trial among primary care physicians (PCPs) in Switzerland (November 2020-December 2021). We randomly assigned PCPs to one of three intervention groups related to low-value care (vitamin D testing, generic prescribing, or a cost intervention) or a control group. Agitation, a behavioural and psychological symptom of dementia, is under-recognized in Japan. To describe the physician's perceptions and treatment practice for agitation in Alzheimer's dementia (AAD) in Japan, we conducted a cross-sectional web-based survey in October 2024. The survey included physicians in neurology, neurosurgery, psychiatry, or general internal medicine who were registered with the survey panel; consented to participation; affiliated with hospitals or clinics; treating ≥ 10 people with Alzheimer's dementia (AD)/month. Responses from 529 physicians showed that they treated a

BACKGROUND: Risks related to long-term opioid therapy for chronic pain are high and may increase over time with aging. Deprescribing may be a beneficial intervention for older adults prescribed chronic opioids.
METHODS: Semi-structured interviews with hypothetical clinical cases of older adults prescribed opioids for chronic pain: (1) low-risk case: a patient prescribed low-dose opioids without concerns; (2) moderate-risk case: a patient with multimorbidity and concurrent benzodiazepine use prescribed moderate opioid doses; (3) high-risk case: a patient prescribed high-dose opioids with signs

INTRODUCTION: The outpatient parenteral antibiotic therapy (OPAT) program of Vancouver General Hospital (VGH) was supervised by emergency physicians (EPs) until 2017 when infectious disease (ID) physicians began assisting in management. We designed a retrospective study to determine whether ID involvement led to improved outcomes.
METHODS: This study analyzes the impact of ID involvement by comparing the mean days patients spent on OPAT with ID involvement versus EPs alone through a retrospective chart review. Secondary research objectives were to compare patient care decisions, e.g., antibio

OBJECTIVES: We conducted a focus group to assess the attitudes of primary care physicians (PCPs) toward prostate-specific antigen (PSA)-screening algorithms, perceptions of using decision support tools, and features that would make such tools feasible to implement.
METHODS: A multidisciplinary team (primary care, urology, behavioral sciences, bioinformatics) developed the decision support tool that was presented to a focus group of 10 PCPs who also filled out a survey. Notes and audio-recorded transcripts were analyzed using Thematic Content Analysis.
RESULTS: The survey showed that PCPs fol

BACKGROUND: Opioid use and related harms have been of growing concern in North America and elsewhere. In Canada, while policies and programs intended to mitigate opioid-related harms have been introduced by public health agencies, the medical profession, and different levels of government, there remains a paucity of evidence regarding the unintended consequences of these initiatives, including their impacts on people with chronic pain, a population often treated with opioids.
METHODS: This institutional ethnography investigated how opioid-related policies and programs are developed, deployed,
