Scapholunate injuries: challenging existing dogmas in anatomy and surgical techniques.

The management of scapholunate injuries and dissociation remains debatable. Traditional methods of fixation had centred around open procedures, but advances in wrist arthroscopic techniques has redefined both major anatomical findings and operative approaches. This article challenges two main existing dogmas: first, the anatomy of the scapholunate ligament and second, the management of these injuries. For the first, we propose that scapholunate stability is in fact maintained by a whole anatomic complex, consisting of well-defined capsuloligamentous structures and extrinsic ligaments, and that

Factors Associated With Surgical Treatment of Nondisplaced or Minimally Displaced Scaphoid Waist Fractures.

PURPOSE: To determine whether there are identifiable factors associated with the surgical treatment of nondisplaced or minimally displaced scaphoid waist fractures. METHODS: We identified 50 patients who underwent nonsurgical treatment and 67 patients who underwent surgical treatment of nondisplaced or minimally displaced scaphoid waist fractures at 2 tertiary care referral centers in a single metropolitan area in the United States between January 2010 and March 2019. Bivariate analysis was used to screen for factors associated with surgical treatment. Multivariable stepwise logistic regressi

Biomechanical Effects of Radioscapholunate Fusion With Distal Scaphoidectomy and Triquetrum Excision on Dart-Throwing and Wrist Circumduction Motions.

PURPOSE: Distal scaphoid and triquetrum excisions can improve the range of wrist motion after radioscapholunate (RSL) fusion, but little is known about the kinematics of dart-throwing and global circumduction motions. We hypothesized that these excisions could increase the range of motion without causing midcarpal instability. METHODS: Seven fresh-frozen cadaver upper extremities were mounted on a testing apparatus after isolation and preloading of the tendons of the flexor carpi radialis, flexor carpi ulnaris, extensor carpi radialis, and extensor carpi ulnaris. Sequential loadings of the fl

Three-ligament tenodesis for chronic scapholunate injuries: short-term outcomes in 203 patients.

In this cohort study, we report on short-term outcomes of 203 patients (203 wrists) who underwent scapholunate ligament reconstruction using a three-ligament tenodesis technique between December 2013 and December 2017. Patients completed the Patient Rated Wrist Evaluation questionnaire and a visual analogue scale for pain online before surgery and at 3 months and 1 year after surgery. We also measured the range of movement before surgery, in 150 patients 3 months after surgery, and in 90 patients 1 year after surgery. The patients reported clinically significant improvements in both pain and f

Scaphoid dislocation associated with axial carpal dissociation during volar flexion of the wrist: a case report.

We present the first report of a patient with an isolated scaphoid dislocation with axial carpal dissociation sustained during volar flexion of the wrist. The scaphoid was dislocated to the radial side of the radial styloid process and was slightly shifted to the dorsal side. It was shown that the position of the wrist played an irrelevant role for occurring scaphoid dislocation.

Isolated volar fracture-dislocation of the scaphoid with acute carpal tunnel syndrome: a case report.

Dislocations and fracture-dislocations of the scaphoid are rare injuries. When they occur in a volar direction, they may be complicated by median nerve compression. While volar dislocations and fracture-dislocations of the scaphoid presenting late with median nerve compression have been reported, such injuries presenting with acute median nerve compression have not been reported. We describe a case of volar fracture-dislocation of the proximal half of the scaphoid causing median nerve compression and presenting with acute carpal tunnel syndrome. Urgent open reduction and internal fixation of t

Biomechanical evaluation of a new composite bioresorbable screw.

A new bioresorbable composite cannulated screw has been developed for small bone fracture fixation. The LG ("Little Grafter") screw is manufactured from Biosteon, which is a composite of poly L-lactic acid and hydroxyapatite. This study aimed to compare interfragmentary compression generated by this new screw with conventional metal screws commonly used in scaphoid fracture fixation. Four small metallic screws were compared with the LG screw, using a bone model produced from rigid polyurethane foam. The screws included the Acutrak, Asnis III, Herbert and Herbert-Whipple screws. The mean maximu

Arthroscopic evaluation of scaphoid waist fracture stability and the role of the radioscaphocapitate ligament.

PURPOSE: The purpose of this article is to arthroscopically evaluate the effect of forearm rotation on scaphoid fracture displacement and the impact of intra-articular ligament sectioning. TYPE OF STUDY: Controlled laboratory study. METHODS: Scaphoid fracture stability is studied arthroscopically in 10 cadaveric upper limbs. Displacement of the osteotomized scaphoid with and without forearm rotation is arthroscopically evaluated before and after radioscaphocapitate (RSC) ligament sectioning. RESULTS: No rotation at the fracture site was identified in full pronation and full supination with

The use of medical imaging-based kinematic analysis in the evaluation of wrist function and outcome.

The authors developed a 3D CT technique to analyze in vivo variations in carpal bone position based on 3D reconstruction of transverse CT data in 5 wrist positions. The subject groups analyzed consisted of 40 asymptomatic volunteers and 30 patients with various wrist disorders (fractures, instabilities). In 11 anatomic specimens, this kinematic analysis was completed by a radiographic morphologic study and an investigation of capsular ligament anatomy. Clinical applications showed that carpal bone motion in the injured wrist was not significantly different from contralateral, asymptomatic wris

A vascularized technique for bone-tissue-bone repair in scapholunate dissociation.

Several surgical options have been used for the repair of scapholunate instability over the last 50 years. The many options have included neglect, reduction with percutaneous pinning, primary repair, partial fusions, tendon weaves, and others. Recent advancements in scapholunate repair and anatomy have been aimed at a more physiological repair. Composite replacement of the entire scapholunate interval similar to other tendon repairs seen in orthopedic surgery has become popular. Currently, more common hand-based grafts are bone-retinaculum-bone, third or second metacarpal-carpal bone or hamate

High fusion rates with circular plate fixation for four-corner arthrodesis of the wrist.

BACKGROUND: Scaphoid excision and four-corner fusion is commonly performed to reconstruct advanced scapholunate collapse and scaphoid nonunion with collapse. Metallic plates were introduced for achieving fixation of the four carpal bones. Although the developer reported high rates of fusion, several other early reports of circular plate fixation suggest higher complication rates and inferior outcomes compared with traditional fixation techniques. QUESTIONS/PURPOSES: To clarify the controversy in the literature we determined the fusion rates, complications, and functional outcomes of patients

[Osseous vascular anatomy in the hand and wrist].

The design and the realization of vascularized osseous grafts at the hand and the wrist require a precise knowledge of the general and regional anatomy. This article gives first a progress report on current knowledge about the general organization of arterial and venous vascularization, of the long bones (number and localization of the nutrient foramina, communication between the epiphyseo-metaphyseal and diaphyseal networks) and of the short bones, in the adult and the child, before the closing of the growth plate. The general organization of arterial vascularization of the hand and the wrist

Comparing the Safety and Efficacy of Robot-Assisted Versus Conventional Fluoroscopy-Guided Techniques in the Treatment of Scaphoid Fractures: A Systematic Review and Meta-Analysis.

BACKGROUND: Robotic-assisted technology has increasingly been applied in orthopedic surgery; however, its safety and efficacy in the treatment of scaphoid fractures remain controversial and lack high-level evidence. This systematic review and meta-analysis aimed to compare robotic-assisted techniques with conventional fluoroscopy-guided techniques in the treatment of scaphoid fractures. METHODS: A systematic literature search was conducted in PubMed, CNKI, VIP, Cochrane Library, Web of Science, and Scopus. Randomized controlled trials and cohort studies comparing robotic-assisted and conventi

Are outcomes of proximal row carpectomy and four-corner fusion dependent on the diagnosis of scapholunate advanced collapse or scaphoid non-union advanced collapse? a systematic review.

Scapholunate advanced collapse (SLAC) and scaphoid non-union advanced collapse (SNAC) are well-recognised degenerative wrist conditions. Debate persists over optimal treatment, particularly since 4 corner fusion (4CF) emerged in the 1980s. Reviews have compared the outcomes of 4CF and PRC; however, none of these have looked at whether the initial diagnosis determines the final outcome. This systematic review hypothesises that the outcome of PRC and 4CF for the treatment of stage 2 and 3 SNAC and SLAC varies depending on the initial diagnosis.
