Somatization in abused women.

BACKGROUND: The association between abuse and somatization has been less systematically investigated than other abuse-related outcomes. Moreover, such studies have given inconsistent results. Therefore, the aim of the present study was to investigate the relation between somatization and lifetime exposure to physical, sexual, and psychological abuse. METHODS: A total of 800 women, 400 reporting abuse and 400 reporting no abuse in a previous randomized, population-based study, were sent two questionnaires: SOMAT, a questionnaire on somatization, and the Abuse Inventory (AI). Of 781 eligible wo

Depression, anxiety, anger, and somatic symptoms in patients with body dysmorphic disorder.

Body dysmorphic disorder (BDD) is a relatively common and impairing disorder. However, little is known about non-BDD symptoms and well-being in patients with this disorder. Seventy-five outpatients with DSM-IV BDD completed the Symptom Questionnaire, a validated self-report measure with four scales: depression, anxiety, somatic/somatization, and anger-hostility. Scores were compared to published norms for normal subjects and psychiatric outpatients. Participants in an open-label fluvoxamine trial completed the Symptom Questionnaire at baseline and endpoint. Compared to normal controls, BDD sub

Theories of prostatitis etiology.

Prostatitis reflects a broad spectrum of prostatic infections, both acute and chronic. Chronic prostatitis, known as National Institutes of Health category III or chronic pelvic pain syndrome, broadly defines a disease that is still poorly understood, and as a consequence, difficult to treat. Typical symptoms include pelvic pain and voiding dysfunction. Infection is often cited as the cause of this condition, despite frequent negative cultures. A close look at the local prostatic microenvironment may yield clues. The role of inflammatory mediators and what stimulates them can point to potentia

[Subarachnoid hemorrhage as life-threatening complication of neural therapy. Case report].

In Germany, neural therapy is a frequently used method of complementary medicine. It is based on the hypothesis, that pathologically altered regions may cause symptoms in other, distant localizations and that this process can be interrupted by the local injection of anesthetic drugs. Since the pathogenetic acting regions are supposed to occur in any part of the body, neural therapy is not limited to superficial injections, but also includes injections into deep structures and internal organs. Supporters of this therapeutic approach always claim that these procedures are free of risk if carried

A comparison of eating disorders and body dysmorphic disorder on body image and psychological adjustment.

Eating and body dysmorphic disorders are two diagnoses with body image disturbance as a central feature. No empirical study of the similarity of these disorders or any controlled study of body dysmorphic disorder were available. The present study compared 45 women with anorexia or bulimia nervosa to 51 men and women with body dysmorphic disorder (BDD) and 50 nonclinical controls. The eating disorder patients were mainly preoccupied with weight and body shape. BDD subjects had more diverse physical complaints and reported more negative self-evaluation and avoidance due to appearance. However, t

Cardiorespiratory symptoms in response to physiological arousal.

OBJECTIVE: To develop a laboratory paradigm for assessing the tendency to amplify somatic symptoms and report bodily distress. METHOD: Reports of four different cardiopulmonary symptoms were obtained during standardized, treadmill exercise, while the physiological parameters which induce these symptoms were simultaneously measured. Two indices were developed to compare symptom reporting across patients: symptom severity after reaching 80% of predicted, maximal exercise capacity; and the magnitude of physiological arousal necessary to induce an initial sensation of discomfort. RESULTS: Fifty-

Negative affect, respiratory reactivity, and somatic complaints in a CO2 enriched air inhalation paradigm.

Subjects scoring high on negative affectivity (NA) are known to report more psychosomatic complaints than subjects scoring low. According to the symptom perception hypothesis, high NA subjects attend more to somatic sensations and interpret these as more threatening. We investigated the relationship between NA and psychosomatic complaints in a group of high and low NA subjects (N = 72) in, (a) a questionnaire study, and (b) in a laboratory setting. The latter involved the inhalation of three different gas mixtures (room air, 5.5% and 7.5% CO2-enriched air) while respiratory responses were regi

[Post-traumatic headache].

Head trauma (HT) and whiplash injury (WI) is followed by a posttraumatic headache (PH) in approx. 90% of patients. The PH due to common WI is located occipitally (67%), is of dull-pressing or dragging character (77%) and lasts on average 3 weeks. Tension headache is the most frequent type of PH (85%). Besides posttraumatic cervicogenic headache or symptomatic, secondary headache due to SDH, SAB, ICB or increased ICP, migraine- or cluster-like headache can be observed in rare cases. Prolonged application of analgetics (> 4 weeks) can cause a drug induced headache. In 80% of patients PH followin

Comparison of screening proposals for somatization disorder empirical analyses.

We report a comparison of three sets of screening criteria, plus a post hoc modification of one, for the diagnosis of somatization disorder. Included in the comparison are the DSM-III-R seven-symptom screen, 11 symptoms identified by Swartz et al., and the hierarchical screen using symptom clusters that has been proposed for DSM-IV, along with a modification to the latter. Data sets used in this test were from the St Louis Epidemiologic Catchment Area (ECA) study and a family study of alcoholism. Findings of sensitivity, specificity, and positive predictive value indicate advantages and disadv

Voluntary stimulus-sensitive jerks and jumps mimicking myoclonus or pathological startle syndromes.

Five patients who presented with stimulus-induced jerking as part of an apparent myoclonic or pathological startle syndrome are reported. Neurophysiological observations in these patients suggested the jerks were voluntary in origin. These included (a) variable latencies to the onset of stimulus induced jerks, (b) latencies were greater than that seen in reflex myoclonus of cortical or brainstem origin, and were (c) longer than the fastest voluntary reaction times of normal subjects, (d) variable patterns of muscle recruitment within each jerk and, (e) significant habituation with repeated sti

Loneliness and psychological distress.

Research on relationships between loneliness and psychological symptoms has generally shown significant positive associations across a wide spectrum of psychopathologies. However, such results may be artificial, to some extent, given the high intercorrelations of typical psychopathology measures. In the current study, we examined associations between psychological symptoms, assessed by the Symptom Check List-90 (SCL-90; Derogatis, Lipman, & Covi, 1973) and loneliness, as measured by the UCLA-R Loneliness Scale (Russell, Peplau, & Cutrona, 1980), in college students. Using partial correlations

Differences in the ICD-10 diagnostic subtype of depression in bipolar disorder compared to recurrent depressive disorder.

BACKGROUND: The aim of the study was to investigate whether patients with bipolar depression and patients with recurrent depressive disorder present with different subtypes of depressive episode as according to ICD-10. SAMPLING AND METHODS: All patients who got a diagnosis of bipolar affective disorder, current episode of depression, or a diagnosis of recurrent depressive disorder, current episode of depression, in a period from 1994 to 2002 at the first outpatient treatment or at the first discharge from psychiatric hospitalization in Denmark were identified in a nationwide register. Interpretation Bias as a Mediator Linking Childhood Maltreatment to Psychopathology: A Network Analysis.

ObjectiveChildhood maltreatment is a well-established risk factor for the development of mental disorders. Interpretation bias has been proposed as a potential mechanism mediating the relationship between childhood maltreatment and subsequent psychopathology. This study investigated differences in interpretation bias and psychopathology symptoms based on experiences of childhood abuse and neglect. It also examined whether interpretation bias mediates the relationship between childhood maltreatment and symptoms of psychopathology using network analysis. Methods: A cross-sectional sample of 123

Psychological Features of Smokers.

BACKGROUND: Smoking tobacco is believed to be common among patients with psychiatric disorders like anxiety, depression, schizophrenia, and affective disorders. The aim of this study was to investigate the psychological symptoms of people without known psychiatric diseases who have freely decided to quit smoking. METHODS: A cross-sectional study was designed, and 124 healthy active-smokers (66 female) and 127 healthy non-smokers (61 female) were recruited. The symptom check list and Fagerström Test for Nicotine Dependence (FTND) were used to assess psychological symptoms and nicotine addictio

Atypical symptoms in hospitalised patients with major depressive episode: frequency, clinical characteristics, and internal validity.

OBJECTIVE: The objective was (1) to assess the frequency of atypical depression (AD) in depressed inpatients; (2) to compare clinical features of patients with atypical and nonatypical depression (Non-AD) (3) to evaluate the meaning of single psychopathological symptoms with special respect to mood reactivity. METHOD: Diagnoses of 1073 inpatients were assessed according to DSM-IV using SCID (Structured Clinical Interview for the DSM-IV) and AMDP (Association for Methodology and Documentation). Diagnosis of atypical depression was defined according to criteria of the DSM-IV specifier for AD. A

Relationships between physical findings (GPE-78) and psychological profiles (MMPI-2) in patients with long-lasting musculoskeletal pain.

The purpose of the study was to evaluate possible relationships between physical findings, as measured with the Global Physiotherapeutic Examination (GPE-78), and psychological characteristics, as measured with the revised Minnesota Multiphasic Personality Inventory (MMPI-2), in three groups of patients with long-lasting musculoskeletal pain. A total of 177 patients (114 women, 63 men), sick-listed owing to long-lasting musculoskeletal pain, were studied. The sample was subcategorized on the basis of pain localization, as marked on a pain drawing: group 1 = pain above a horizontal line in the

Medically unexplained symptoms in primary care.

Fourteen common physical symptoms are responsible for almost half of all primary care visits. Only about 10% to 15% of these symptoms are found to be caused by an organic illness over a 1-year period. Patients with medically unexplained symptoms are frequently frustrating to primary care physicians and utilize medical visits and costs disproportionately. This paper will review the relationship between psychological distress and the decision to seek medical care for common physical symptoms such as fatigue and headache. Evidence will be presented demonstrating that an increasing number of medic

The prevalence of current major depression and dysthymia in a Norwegian general practice.

Existing studies suggest that depression is underdiagnosed and undertreated in general practice, and that the known prevalence of this mood disorder in a primary care population may represent only the "tip of the iceberg'. A total of 100 consecutive patients in an average Norwegian general practice were tested, of whom 31 patients were diagnosed as having a depressive illness in this study; 28 patients were diagnosed as having current major depression and three as having dysthymia. In total, 21 of the 28 patients with current major depression presented with other symptoms as their major compla

Depression in old age. Is there a real decrease in prevalence? A review.

The discrepancy between the constancy or increase of the prevalence of depressive symptoms and dysphoria in old age on one hand, and the decrease in the prevalence of the DSM-III diagnoses of major depression and dysthymia on the other, is discussed in light of the most frequent explanatory hypotheses such as memory defects, interpretation of depressive as somatic symptoms, higher risk of institutionalization as well as higher mortality of depressives and a mitigated course of depression in old age. We conclude that higher mortality, mitigation and the rarity of true late-onset depression are
