Neurogenic pulmonary edema associated with autonomic dysreflexia.

Neurogenic pulmonary edema is associated with a variety of central nervous system injuries and results from a massive centrally mediated sympathetic discharge. Another syndrome characterized by sympathetic overactivity is autonomic dysreflexia, which occurs in patients with spinal cord injury above T4 to T6. We describe a patient with a cervical spinal cord injury who had intense, prolonged autonomic dysreflexia following external sphincterotomy, which culminated in pulmonary edema. Traction on an over-inflated Foley catheter balloon was the stimulus for autonomic dysreflexia and deflation of …

[Sclerosis of the anal sphincter from the point of view of mechanical ileus (author's transl)].

Anal sphincter sclerosis, which leads to recurrent subileus and finally to the complete picture of ileus, is presented in relation to 6 of o … subileus or ileus condition can be eliminated in all cases by stretching the sphincter and partial sphincterotomy, so that the patients could be spared a laparotomy.

Intermittent catheterization and bladder rehabilitation in spinal cord injury patients.

A review was made of 111 spinal cord injury patients who underwent bladder rehabilitation. The essential mode of treatment was intermittent catheterization. About 91 per cent of the patients were free of the catheter after 3 to 168 days of intermittent catheterization. Acute patients given an initial dosage of 30 to 40 mg. bethanechol chloride 3 times daily for 2 to 3 weeks recovered reflex activity in almost half the period (average of 32 days compared to 79 days). Modified sphincterotomy at the bladder neck was performed in 10 patients. This procedure improved the results to 98 per cent cath…

Pancreaticopleural Fistula: A Rare Complication of Alcoholic Pancreatitis.

Pancreaticopleural fistula (PPF) is an uncommon complication of chronic pancreatitis. The authors describe a case of a 41-year-old male with … ct; however, there was disruption of the pancreatic duct with leaking beyond the pancreatic neck. A sphincterotomy was performed, and a double-flanged stent was placed, which resulted in the resolution of the dyspnea and the right-sided pleural effusion.

Current endoscopic strategies for managing large bile duct stones.

Common bile duct stones are a relatively common occurrence and can often lead to devastating complications. Endoscopic retrograde cholangiopancreatography was introduced in the 1970s for management of common bile duct stones. Most common bile duct stones can be removed with simple techniques such as endoscopic sphincterotomy and balloon trawling. However, large bile duct stones continue to pose some difficulty in achieving complete extraction. In this article, we will review some of the established techniques such as the use of endoscopic papillary large balloon dilatation, mechanical lithotri…

[Acute pancreatitis after endoscopic evacuation of the gall stone, presenting as duodenal perforation: a case report].

We present the case of a 25-year-old patient who had sphincterotomy performed for the retrieval of gall stones form the common bile duct, and in whom, immediately after the procedure, signs and symptoms of the retroperitoneal, iatrogenic perforation of the duodenum had developed. Additionally, the patient showed clinical and biochemical symptoms of acute pancreatitis. The patient was operated on, and, intraoperatively, the duodenal perforation was not found, but excessive inflammatory infiltration of the retroperitoneal space, without bile leakage, and typical features for acute pancreatitis. …

Biliogastric diversion for the management of high-output duodenal fistula: report of two cases and literature review.

High-output duodenal fistula occurs as a result of a duodenal wall defect caused by gastroduodenal surgery, endoscopic sphincterotomy, duodenal injury, and tumors with high morbidity and mortality rate. A new technique for its management is reported along with literature review. This procedure consists of transection of the duodenum 2 cm distally to the pylorus, transection of the common bile duct, and end duodenostomy with or without suturing the duodenal wall defect. The continuity of the alimentary tract is reinstated by an end-to-end duodenojejunostomy, end-to-side choledochojejunostomy, a…

Surgical management of a hydatid cyst of the hepatic dome ruptured into the biliary tree.

Hydatid disease of the liver remains an important and challenging medical problem. Although surgery is considered the treatment of choice for hydatid disease of the liver, controversies still exist about the preferred operative technique. We report the case of a patient with obstructive jaundice caused by rupture of a hydatid cyst of the hepatic dome into the biliary tract. He was managed by a new surgical approach of myoplasty of the right hemidiaphragm combined with preoperative decompression of the bile duct by an endoscopic sphincterotomy. The postoperative course of the patient was uneven…

Aetiology and treatment of anal fissure.

Anal fissure is a common problem that causes significant morbidity in a young and otherwise healthy population. Treatment has remained largely unchanged for over 150 years and the pathogenesis of this condition is not yet fully explained. Acute fissure should be treated conservatively with dietary modification. Chronic fissures do not respond to conservative treatment. The current recommended surgical treatment for chronic fissure is lateral internal sphincterotomy. However, there is a disturbance of continence in a sizeable proportion of those undergoing this procedure. As yet there is no pro…

[Endoscopic papillosphincterotomy in early childhood].

Cholelithiasis may be complicated by choledocholithiasis that is quite rare in children. Surgical correction in children with distal block of the common bile duct is determined by efficacy of therapy, concomitant duodenopancreatobiliary anomalies and diseases. A 2-year-old girl (body weight 9 kg) admitted with complaints of abdominal pain, gray stool and jaundice of skin. Clinical, laboratory and instrumental examination revealed choledocholithiasis. Endoscopic retrograde typical papillosphincterotomy was performed without adverse events. The child was discharged in 3 postoperative days with p…

Evidence-based clinical practice guidelines for cholelithiasis 2021.

The Japanese Society of Gastroenterology first published evidence-based clinical practice guidelines for cholelithiasis in 2010, followed by a revision in 2016. Currently, the revised third edition was published to reflect recent evidence on the diagnosis, treatment, and prognosis of cholelithiasis conforming to the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) system. Following this revision, the present English version of the guidelines was updated and published herein. The clinical questions (CQ) in the previous version were reviewed and rearranged into three ne

OBJECTIVE: To evaluate the effect of the treatment of congenital hypertrophic pyloric stenosis (CHPS) with endoscopic pyloromyotomy. METHOD: Nine consecutive infants (7 boys, 2 girls; age range 26 - 70 days; weight range 2.65 - 6.10 kg), with a diagnosis of CHPS according to typical clinical manifestations, transabdominal ultrasound (US), gastroenterography and gastroscope. All the cases had accompanying malnutrition, anaemia, metabolic alkalosis, and some were complicated with congenital heart disease. In gastroscope operating room, all the patients were given pentobarbital and midazolam int

[Cholestasis: interventional therapy].

Treatment of cholestasis by bile duct obstruction is nowadays dominated by endoscopy and other interventional measures. Intraductal concrements can be removed almost always without open surgery. This is as a rule achieved by a transpapillary approach with the additional use of lithothripsy. Percutaneous transhepatic treatment of concrements is restricted to patients in which a transpapillary approach is impossible. Stenoses of biliary ducts by malignancies are treated with plastic or metallic endoprosthesis if curative resection is precluded. Percutaneous drainage is a valid option in reserve.
