Role of skin tissue layers and ultra-structure in transcutaneous electrical stimulation including tDCS.

BACKGROUND: During transcranial electrical stimulation (tES), including transcranial direct current stimulation (tDCS) and transcranial alte … in model (epidermis, dermis, and fat), with or without additional ultra-structures (hair follicles, sweat glands, and blood vessels). Current flow patterns across each layer and within ultra-structures were predicted using finite element methods considering a broad range of modeled tissue parameters including 78 combinations of skin layer conductivities …

Glycopyrrolate-induced craniofacial compensatory hyperhidrosis successfully treated with oxybutynin: report of a novel adverse effect and subsequent successful treatment.

Hyperhidrosis, or abnormally increased sweating, is a condition that may have a primary or secondary cause. Usually medication- induced secondary hyperhidrosis manifests with generalized, rather than focal sweating. We report a 32-year-old woman with a history of palmoplantar hyperhidrosis for 15 years who presented for treatment and was prescribed oral glycopyrrolate. One month later, the palmoplantar hyperhidrosis had resolved, but she developed new persistent craniofacial sweating. After an unsuccessful trial of clonidine, oxybutynin resolved the craniofacial hyperhidrosis. To our knowledge…

Dyshidrotic Eczema and Its Relationship to Metal Allergy.

Dyshidrotic eczema, also known as dyshidrotic dermatitis or pompholyx, is characterized by pruritic, small tense vesicles mainly on the palmoplantar region and lateral and ventral surfaces of the fingers. While its etiology appears to be related to sweating, as dyshidrotic eczema often occurs in an individual with hyperhidrosis, and the spring allergy season, histologic examination shows an eczematous reaction around the sweat ducts which is not associated with abnormalities of the sweat ducts. More recently, the nomenclature of 'acute and recurrent vesicular hand dermatitis' has been proposed…

[Primary hyperhidrosis. Current status of surgical treatment].

Primary hyperhidrosis-PH is an excessive sweating without known etiology. The PH is more frequent in women and in palms, soles and axillae. Medical treatment is not effective. The objective of the surgery is to remove or to disconnect sympathetic ganglia T2 (craniofacial PH or facial blushing), T3 (palmar PH) and T3-T4 (axillary PH). The surgical techniques are mainly resection/transection, ablation with electrocoagulation, sympathetic block by clipping and radiofrequency. Anhidrosis is achieved in 95% of the patients. The overall rate of complications is less than 5% and these are minor compl…

Fabry disease: impaired autonomic function.

Previous reports of extensive lipid accumulation within neurons of the autonomic nervous system in Fabry disease suggest an anatomicopathologic basis for the peculiar pain, diminished sweating, and gastrointestinal symptoms experienced in this disorder. To further assess autonomic function in Fabry disease, noninvasive clinical tests were performed on 10 patients. Diminished sweating was found in each; the loss was approximately uniform proximally and distally, suggesting sweat gland dysfunction rather than autonomic neuropathy. Impaired pupillary constriction with pilocarpine, and reduced sal…

Treatment of Traumatic Frey Syndrome and Facial Contour Deformity Using a De-Epithelialized Free SCIP Flap: A Case Report.

Frey syndrome (FS), or gustatory sweating, is typically characterized by sweating and flushing in the parotid region during eating. While FS most commonly develops after parotidectomy, it has also been reported following traumatic causes such as submandibular gland surgery, radical neck dissection, mandibular condyle fracture, or forceps delivery. The authors present the case of a 58-year-old male who developed FS and a right-sided facial contour deformity 11 months following severe blunt maxillofacial trauma. On the basis of the diagnosis, a de-epithelialized superficial circumflex iliac arte…

Long-term efficiency of endoscopic thoracic sympathicotomy: survey 10 years after surgery.

Immediate and short-term results of endoscopic thoracic sympathectomy (ETS) for primary hyperhidrosis are good. Adverse effects have been identified clearly and are supposed to decrease with time. In this institutional report, the long-term results of ETS with regard to efficacy, side effects and patient satisfaction are presented. Fifteen patients were included and mean follow-up time was 12+/-2 years. ETS success rate, rate of compensatory sweating and degree of patient satisfaction were assessed. We detected 8 patients (53%) complaining about a decent to moderate recurrence of hand sweating…

Factors affecting outcome following endoscopic thoracic sympathectomy.

BACKGROUND: Endoscopic thoracic sympathectomy (ETS) is an effective treatment for palmar hyperhidrosis and has been extended to craniofacial symptoms such as blushing and facial sweating. Adverse effects, including compensatory sweating, may cause patients to regret surgery. Such adverse events are currently unpredictable. This study investigated whether age, sex or bodyweight influenced the outcome. METHODS: A total of 110 patients who had bilateral ETS for palmar hyperhidrosis or facial symptoms (blushing or sweating) were asked to complete a questionnaire before and after surgery regarding…

Clinical features of prolonged tilt-induced hypotension with an apparent vasovagal mechanism, but without syncope.

BACKGROUND: A previous study of electroencephalography (EEG) changes with syncope led to a finding that some young patients develop prolonged periods of tilt-induced hypotension, but they do not lose consciousness. The present study aim was to compare patterns of hemodynamic changes, measures of duration, and sweating between these patients and patients with tilt-induced vasovagal syncope. METHODS: In an observational study, qualitative changes in hemodynamic parameters were compared between patients with prolonged hypotension (n = 30) and with syncope (n = 30). To demonstrate that periods of…

Alterations in cardiovascular autonomic function tests in idiopathic hyperhidrosis.

We performed cardiovascular autonomic function tests to assess sympathetic and parasympathetic functions in patients with idiopathic hyperhidrosis. We studied 35 patients with idiopathic hyperhidrosis and 35 age- and sex-matched controls. A thermoregulatory sweat test (TST) was performed in all subjects. Sweating was qualitatively (Minor's test at 22°C) and quantitatively (skin conductance) evaluated. Orthostatism, tilt to 65°, cold pressor test, deep breathing, Valsalva maneuver and hyperventilation were performed in patients and controls. A greater fall in blood pressure values was observed …

Endoscopic thoracic sympathectomy for primary palmar hyperidrosis.

BACKGROUND: Primary hyperhidrosis is a disorder that is characterized by excessive sweating in disproportion to that required for thermoregulation. In most cases, this is aggravated by emotional factors and by heat. Hyperhidrosis can be seen in the palms of the hands, armpits, soles of the feet and face. The principal characteristic of this disease is the intense discomfort of patients, which affects their social and professional life. Treatment modalities include topical application of aluminum chloride, iontophoresis, anticholinergics, botulinum toxin injection, liposuction, excision of swea…

Treatment of palmar hyperhidrosis using needlescopic T2 sympathetic block by clipping: analysis of 102 cases.

Endoscopic thoracic sympathicotomy, or sympathectomy by a 2-mm scope, is an effective method for treating palmar hyperhidrosis. However, postoperative compensatory sweating may be troublesome in some patients. We report needlescopic T2 sympathetic block by clipping, which may provide reverse operation for patients encountering compensatory sweating. Between January 1998 and January 2002, a total of 102 patients with palmar hyperhidrosis underwent video-assisted thoracoscopic sympathetic blocking of the T2 ganglion. There were 47 males and 55 females (mean age, 24.1 years; range, 9-50 years). A…

Signs of sympathetic denervation associated with a thoracic melanoma in a horse.

Sympathetic denervation in a 20-year-old, gray, Thoroughbred-Percheron gelding was manifested by cutaneous hyperthermia and sweating over the right side of the body, demarcated by a line from the withers to the elbow and extending cranially. There was cutaneous hyperthermia over the right side of the head, but other signs of Horner's syndrome (sweating, ptosis, miosis, enophthalmos) were not present. The pattern of cutaneous hyperthermia and sweating was consistent with sympathetic denervation localized to the cervicothoracic ganglion, and thoracic radiographs revealed increased density in the…

Development of a Low-Cost Portable Iontophoresis Device.

Hyperhidrosis, particularly of the palms and soles, is a common condition characterized by excessive and uncontrollable sweating that can significantly affect daily activities and quality of life. Among the many various treatments, the iontophoresis has emerged as a safe, simple, and non-pharmacological solution. Iontophoresis is used to treat excessive sweating (hyperhidrosis) by temporarily disabling sweat glands through a small direct current through the skin via water-filled electrodes. However, such devices are not widely available, and the price can range from hundreds to thousands of do…

Radiofrequency Ablation for Refractory Palmar and Plantar Hyperhidrosis: A Case Report.

INTRODUCTION: Palmar and plantar hyperhidrosis (PH) is characterized by sweat production that far exceeds thermoregulatory needs and often has substantial effects on daily functioning, social interaction, and mental health. The condition results from sympathetically mediated overstimulation of the eccrine glands, which are innervated by unmyelinated cholinergic C-type fibers. Radiofrequency ablation (RFA) offers a minimally invasive approach to symptom control in refractory PH through heat-mediated disruption of these sympathetic fibers. Reports describing staged upper- and lower-limb RFA for …

The role of improved molecular genetic testing in the diagnosis of cystic fibrosis - a case series.

Cystic fibrosis (CF) is diagnosed through a combination of clinical symptoms, a sweat test, and genetic analysis. Over the last five years, significant advances in cystic fibrosis treatment have been driven by a better understanding of the disease’s genetic basis. This has enabled the development of more sophisticated genetic tests, allowing for more precise treatment for each individual patient. We present a case series of children with clinical presentations suggestive of cystic fibrosis, ambiguous sweat test results, and previous “negative” genetic analysis results. More precise and advance…

Utility of Immunohistochemistry in Diagnosis of Papillary Eccrine Adenoma/Tubular Apocrine Adenoma (PEA/TAA).

Papillary eccrine adenoma/tubular apocrine adenoma (PEA/TAA) is a rare sweat gland neoplasm. Because PEA/TAA shares morphologic features with other sweat gland tumors, including digital papillary adenocarcinoma, histologic diagnosis may be challenging. We report a case of PEA/TAA presenting as a nodule on the left foot of a 43-year-old man. Histopathologic examination revealed a well-circumscribed dermal proliferation of variably sized glands composed of ductal cells with an outer layer of myoepithelial cells. By immunohistochemistry, the ductal tumor cells showed strong, diffuse expression of…
