Recent developments in renal denervation.

Renal denervation is a novel catheter based approach that has been shown to effectively lower blood pressure in patients with resistant hypertension. Small studies have shown additional benefits in patients being treated for hypertension, such as improvements in insulin sensitivity, sleep apnoea and arrhythmias. More recently, it has been used for the first time for the treatment of sympathetic overactivation in patients with chronic systolic heart failure. Currently renal denervation therapy uses electrode-tipped catheters which deliver radio frequency energy to sympathetic afferent and effer

Renal sympathetic nerve activity during asphyxia in fetal sheep.

The sympathetic nervous system (SNS) is an important mediator of fetal adaptation to life-threatening in utero challenges, such as asphyxia. Although the SNS is active well before term, SNS responses mature significantly over the last third of gestation, and its functional contribution to adaptation to asphyxia over this critical period of life remains unclear. Therefore, we examined the hypotheses that increased renal sympathetic nerve activity (RSNA) is the primary mediator of decreased renal vascular conductance (RVC) during complete umbilical cord occlusion in preterm fetal sheep (101 ± 1

Modulation of ocular hydrodynamics and iris function by bremazocine, a kappa opioid receptor agonist.

This study was designed to determine the activity of bremazocine (BRE), a relatively selective kappa opioid receptor agonist, on intraocular pressure (IOP), aqueous humor formation and pupil diameter (PD) in conscious, normal, dark-adapted New Zealand white (NZW) rabbits. IOP was measured in normal and unilaterally sympathectomized rabbits using a calibrated pneumatonometer and the aqueous flow rate was determined by the use of a Fluorotron Master. A masked-design study was conducted in which the rabbits' eyes were treated with BRE topically and unilaterally; the fellow eyes received vehicle.

[Sympathetic reflex dystrophy and phantom pain. Diagnosis, therapy and prognosis].

The incidence of phantom limb pain has been significantly underestimated for many years. However, studies published during the recent decade indicate that the real incidence of phantom limb pain may be between 60% and 90%. Reflex sympathetic dystrophy (RSD) occurs with an incidence of about 15.000 new cases every year in Germany. Both diseases show early centralisation and chronification. Hence, only early diagnosis and onset of correct therapy in time provide significant pain reduction. When therapy is started too late, prognosis in regard to sufficient pain reduction is poor. Phantom limb pa

Participation of Na+ channels in the response of carotid body chemoreceptor cells to hypoxia.

The role played by Na+ channels of carotid body (CB) chemoreceptor cells was investigated by studying the effects of tetrodotoxin (TTX) on the release of 3H-labeled catecholamines ([3H]CA) by adult rabbit CBs previously incubated with the precursor [3H]tyrosine. TTX inhibited partially the release of [3H]CA elicited by mild hypoxia (10 or 7% O2) or by depolarizing incubation medium containing 20 or 30 mM KCl, but the response to more intense hypoxia (5 or 2% O2) or to higher KCl concentration (40 or 50 mM) was not significantly affected. The release of [3H]CA elicited by acidic stimuli, either

Endoscopic transthoracic electrocautery of the sympathetic chain for palmar and axillary hyperhidrosis.

Endoscopic transthoracic electrocautery of the sympathetic chain has been the preferred treatment for palmar or axillary hyperhidrosis in this unit since 1980. A retrospective study was carried out of the first 112 patients with case material derived from a postal questionnaire, chart review and outpatient assessment. Eighty-five patients undergoing bilateral transthoracic electrocautery who replied to the questionnaire (76 per cent response rate) form the basis of this study. There were 65 females and 20 males with a mean age of 24.3 years (range 15-40 years). The hands alone were affected in

Comparison of the monoamine and catabolite content in the cat and rabbit carotid bodies.

The monoamine and catabolite contents of a large number of rabbit (n = 95) and cat (n = 32) carotid bodies (CBs) have been measured by high performance liquid chromatography with electrochemical detection (HPLC-ED). The dopamine (DA) content as well as that of its precursors tyrosine (TYR), dihydroxyphenylalanine (DOPA) and catabolites dihydroxyphenylacetic acid (DOPAC), homovanilic acid (HVA) were approximately equal in both species. The noradrenaline (NA) content was 10 times larger in the cat than in the rabbit CBs. Twenty-nine out of the 32 cat CBs contained more NA than DA while the rever

Relation of supersensitivity to cerebral vasospasm induced by oxyhaemoglobin.

There have been reports that superior cervical ganglionectomy (SCG) is effective as a treatment for cerebral vasospasm following subarachnoid haemorrhage (SAH). On the other hand, denervation supersensitivity is supposed to play an important role in vasospasm. The two accounts contradict each other. To investigate this problem, various examinations were performed using the exposed basilar arteries of cats. We have accepted that oxyhaemoglobin (Oxy-Hb) is an important spasmogenic substance. Therefore, the change in vascular reactivity to Oxy-Hb was investigated after various kinds of denervatio

Differential effects of sympathetic activity on AV junctional automaticity and AV conduction.

Rapid ventricular response during episodes of supraventricular tachycardia are often followed, on abrupt cessation of the tachycardia, by prolonged pauses terminated by a sluggish and sometimes erratic escape of a supraventricular pacemaker. Such chronotropic-dromotropic paradoxes are readily reproduced in the animal laboratory following elimination of the sinus node and bilateral decentralization of the stellate ganglia and vagi. This study examined whether left stellate stimulation (0.5, 1, 2, 4, 8 and 16 Hz) or lack thereof differentially affected AV junctional automaticity and AV conductio

Blood flow in the ovary and oviduct of rats after sympathetic denervation.

The effect of sympathetic denervation on blood flow in the ovary and oviduct was studied in rats undergoing oestrous cycles or at Day 14 of pregnancy. The ovary and oviduct on one side were denervated by briefly freezing the ovarian vascular pedicle and the ovarian suspensory ligament. Blood flow was measured using 15 microns 57Co-labelled microspheres while the rats were under barbiturate anaesthesia. In cyclic rats denervation raised blood flow to the oviduct by 90% the next day (P less than 0.01) and 39% at 4-10 days (0.05 less than P less than 0.1). Blood flow to the ovary was not affected

[Effect of sympathetic denervation on the outflow of intraocular fluid and the concentration of pyrocatechins in the fluids and tissues of the eye].

After removal of the supper cervical sympathetic ganglion in cats followed by increase of the intraocular pressure from 20 to 32 mm Hg, the outflow of intraocular fluid was sharply enhanced due to lowering tone of tissues of the eye drainage apparatus in the desympathised eye. Noradrenaline was found to reduce sharply by the 4th-5th day in the desympathised eye's tissues due to degenerative processes whereas accumulation of noradrenaline by the 6th-8th day occurred on account of humoral factors.

Vasospastic disorders.

Acute digital ischemia not due to thromboembolism may be the result of either a primary vasospastic disorder (Raynaud's disease, acrocyanosis, livedo reticularis) or vasospasm associated with a systemic, regional, or traumatic disorder (Raynaud's syndrome, cold injury). Raynaud's disease versus syndrome is distinguishable up to 95% of the time from clinical criteria.

Neurosurgical control of chronic pain.

The neurosurgeon can play a significant role in the management of chronic pain. This potential is all to often unrealized or distorted by a lack of knowledge about patients who are suffering from chronic pain. Patient selection is the most important determinant of successful surgical therapy. Patients with chronic pain due to a neoplasm should be offered prompt, definitive neurosurgical intervention. Neurosurgical therapy is less likely to be successful if the pain is secondary to benign disease; tic douloureux is an exception to this rule. If chronic pain behavior is not primarily due to noxi
