Echocardiographic Applications of M-Mode Ultrasonography in Anesthesiology and Critical Care.

Proficiency in echocardiography and lung ultrasound has become essential for anesthesiologists and critical care physicians. Nonetheless, comprehensive echocardiography measurements often are time-consuming and technically challenging, and conventional 2-dimensional images do not permit evaluation of specific conditions (eg, systolic anterior motion of the mitral valve, pneumothorax), which have important clinical implications in the perioperative setting. M-mode (motion-based) ultrasonographic imaging, however, provides the most reliable temporal resolution in ultrasonography. Hence, M-mode c

TAVR in Rheumatic Aortic Stenosis With Mechanical Mitral Prosthesis: Case Series on Short Annulus-Prosthesis Distance.

Patients with rheumatic aortic stenosis and prior mechanical mitral valve replacement present unique challenges for transcatheter aortic valve replacement, particularly when the aortomitral distance is short. This case series examines 4 such patients, focusing on procedural complexities, imaging strategies, and outcomes. Preprocedural multidetector computed tomography and intraprocedural transesophageal echocardiography were critical in assessing anatomical constraints and optimizing valve deployment. Key considerations included preventing mitral prosthesis interference, mitigating left ventri

Metastatic Breast Carcinoma to the Coronaries: A Rare Cause of Acute Myocardial Infarction.

A 63-year-old female with a history of invasive ductal breast carcinoma presented to the emergency department with symptoms characteristic of myocardial infarction. Electrocardiography showed sinus tachycardia and ST elevations in leads II, III, and aVF, consistent with inferior wall myocardial infarction. A computed tomography (CT) scan of the chest, abdomen, and pelvis with intravenous contrast demonstrated widespread intrathoracic metastatic disease. Cardiac magnetic resonance imaging (MRI) with contrast revealed obstruction of the left ventricular (LV) outflow tract by an LV mass. Cardiac

Valsalva maneuver in echocardiography.

The Valsalva maneuver is an easily performed maneuver with an interesting hemodynamic effect which can be used to aid accurate echocardiographic diagnosis. However, correct adequate performance is often missed. Here, we aim to describe the performance of an adequate Valsalva maneuver and the correct interpretation of its effect. The Valsalva hemodynamic effect consists of four basic phases which can be used in echocardiography to yield an accurate diagnosis. Valsalva is used to decrease preload and provoke left ventricular outflow tract (LVOT) gradient in dynamic LVOT obstruction. In addition,

Diversity of mitral valve abnormalities in obstructive hypertrophic cardiomyopathy.

Hypertrophic cardiomyopathy (HCM) is a complex and common genetic disease. The left ventricular outflow tract obstruction is an important determinant of symptoms and outcomes. Its pathophysiology is determined by the complex interaction of the mitral valve, papillary muscles, chordae tendinea, and interventricular septum. The knowledge and importance of the mitral valve apparatus in the physiology of the HCM are expanding with the new imaging modalities, in particular, cardiac magnetic resonance. Several abnormalities of the mitral apparatus have been described in patients with HCM including a

[Atrioventricular asynchrony and pacing].

Atrioventricular dissociation and atrioventricular block are the most common types of atrioventricular asynchrony. Less frequently, atrioventricular asynchrony may occur in the pacemaker syndrome or during sequential pacing with programmed asynchronous atrioventricular delay. A rare type of atrioventricular asynchrony that has relevant hemodynamic effects is observed in first-degree atrioventricular block when associated with mitral stenosis or other conditions characterized by an obstruction of the left atrial outflow. Atrioventricular asynchrony can be defined as systolic when both the atriu

Perioperative management of an elderly patient of hypertrophic obstructive cardiomyopathy for knee arthroplasty and the role of peripheral nerve blocks.

This case report exemplifies how the anaesthetic technique of general anesthesia with continuous bilateral femoral nerve block for bilateral knee arthroplasty was well chosen for the management of perioperative complications in an elderly patient with hypertrophic obstructive cardiomyopathy (HOCM). A 69-year-old female patient of HOCM was scheduled for bilateral total knee replacement. Echocardiography revealed severe left ventricular outflow tract obstruction with peak systolic gradient of 56 mmHg. The surgery was conducted under general anaesthesia with invasive monitoring and bilateral cont

Three-dimensional haemodynamics in patients with obstructive and non-obstructive hypertrophic cardiomyopathy assessed by cardiac magnetic resonance.

AIMS: The left ventricular outflow tract (LVOT) peak pressure gradient is an important haemodynamic descriptor in patients with hypertrophic cardiomyopathy (HCM); however, secondary alterations in aortic blood flow have not been well described in these patients. Aortic flow derangement is not easily assessed by traditional imaging methods, but may provide unique characterization of this disease. In this study, we demonstrated how four-dimensional (4D) flow MRI can assess LVOT peak pressure gradients in HCM patients and also evaluated the ascending aorta (AAo) haemodynamic derangement associate

Periconceptional Folic Acid Supplementation and Infant Risk of Congenital Heart Defects in Norway 1999-2009.

BACKGROUND: The birth prevalence of congenital heart defects (CHDs) has decreased in Canada and Europe. Recommended intake of folic acid in pregnancy is a suggestive risk-reducing factor for CHDs. We investigated the association between periconceptional intake of folic acid supplements and infant risk of CHDs. METHODS: Information on maternal intake of folic acid supplements before and during pregnancy in the Medical Birth Registry of Norway 1999-2009 was updated with information on CHD diagnoses from national health registers and the Cardiovascular Diseases in Norway Project. The association

Relationship Between Maximal Left Ventricular Wall Thickness and Sudden Cardiac Death in Childhood Onset Hypertrophic Cardiomyopathy.

BACKGROUND: Maximal left ventricular wall thickness (MLVWT) is a risk factor for sudden cardiac death (SCD) in hypertrophic cardiomyopathy (HCM). In adults, the severity of left ventricular hypertrophy has a nonlinear relationship with SCD, but it is not known whether the same complex relationship is seen in childhood. The aim of this study was to describe the relationship between left ventricular hypertrophy and SCD risk in a large international pediatric HCM cohort. METHODS: The study cohort comprised 1075 children (mean age, 10.2 years [±4.4]) diagnosed with HCM (1-16 years) from the Inter

Regression of left ventricular hypertrophy provides an additive physiological benefit following treatment of aortic stenosis: Insights from serial coronary wave intensity analysis.

AIM: Severe aortic stenosis frequently involves the development of left ventricular hypertrophy (LVH) creating a dichotomous haemodynamic state within the coronary circulation. Whilst the increased force of ventricular contraction enhances its resultant relaxation and thus increases the distal diastolic coronary "suction" force, the presence of LVH has a potentially opposing effect on ventricular-coronary interplay. The aim of this study was to use non-invasive coronary wave intensity analysis (WIA) to separate and measure the sequential effects of outflow tract obstruction relief and then LVH

Myocarditis combined with hypertrophic cardiomyopathy: a case report.

Myocarditis can cause ventricular wall thickening due to myocardial edema. If the condition improves, the ventricular wall thickening should gradually decrease; a persistent thickening of the patient's ventricular wall indicates the coexistence of hypertrophic cardiomyopathy (HCM) and myocarditis. A 30-year-old man was referred to our hospital with continuous chest pain accompanied by profuse sweating. He suffered from fever for two days (the maximum body temperature: 38 °C) and the conditions improved following the use of antipyretics as self-administered medication before admission. Electroc

Aficamten is associated with improvements in cardiac mechanics in obstructive hypertrophic cardiomyopathy: Results from FOREST-HCM.

BACKGROUND: Aficamten is a next-in-class, selective cardiac myosin inhibitor (CMI) that relieves left ventricular outflow tract (LVOT) obstruction in obstructive hypertrophic cardiomyopathy (oHCM) by addressing underlying myocardial hypercontractility. The long-term impact of aficamten on myocardial mechanics as measured by global longitudinal strain (GLS) is unknown. METHODS: FOREST-HCM (NCT04219826) is an ongoing open-label extension trial for eligible patients with HCM who completed a parent study of aficamten. Here we report echocardiographic parameters (LVOT gradient and myocardial GLS)

Sudden cardiac death in hypertrophic cardiomyopathy.

Sudden death is an important facet of the natural history of hypertrophic cardiomyopathy. Issues related to the nature and etiology of sudden death and the prospective identification of those patients at increased risk have been the subject of intense study, and consequently, our concepts and knowledge have continued to evolve. Occurrence of sudden death has been reported to be about 2-3% per year in a hospital-based referral population and, although described in most age groups, is most common in older children and young adults. The typical profile is that of a young asymptomatic patient with

Effect of the preservation of annulo-papillary continuity on left ventricular ejection fraction after mitral valve replacement. A clinical randomized study on patients affected by rheumatic valve disease.

This perspective study has been designed to evaluate the modifications induced on left ventricular contractility by the interruption of annulo-papillary continuity during mitral valve replacement in patients with rheumatic valve disease. Patients with associated cardiac diseases were not admitted to the study. Sixty-nine patients entered the trial, each patient was randomly assigned to mitral valve replacement either with preservation of the annulo-papillary continuity (PAPC) or with excision of all the chordae (EC). Patients with mitral stenosis (MS) and combined mitral disease (MS & R) were
