BACKGROUND: In children who require prolonged and multiple venous catheterizations, the superior vena cava and iliofemoral veins may become occluded, making central venous access a difficult challenge. We report an innovative technique of catheter insertion percutaneously from the neck into the right atrium traversing a thrombosed superior vena cava using video-assisted thoracoscopic surgery.
METHODS: Two children with irreversible intestinal failure had 4 central venous accesses insertions using the above-mentioned technique. Both had occluded major central veins after multiple catheterizati

RATIONALE: Early lung cancer diagnosis is crucial for prognosis, but elderly patients with comorbidities poorly tolerate invasive procedures. Conventional percutaneous biopsy for major vessel-adjacent high-risk nodules has a mere 60% to 70% success rate and high complications, while artificial pneumothorax boosts it to 85% to 90%. This case verifies the technique's safety in this population.
PATIENT CONCERNS: A 72-year-old female had a 7-mm left lung ground-glass nodule (2021, no intervention). Follow-up computed tomography (CT) (November 2024) showed the nodule enlarged to 10 × 10 mm with a

A 42-year old woman presented with rheumatoid arthritis, dyspnoea and a congenital foregut cyst referred by a rheumatologist for thoracic surgery. The cyst was removed by video-assisted thoracoscopic surgery. The patient developed acute haemoptysis in the immediate postoperative period, which necessitated pulmonary resection due to infracted right lower lobe. This case report highlights the complication related to an unusual anatomy of a congenital foregut cyst.

Birt-Hogg-Dubé syndrome (BHDS) is an autosomal dominant inherited disorder caused by mutation in a tumour suppressor gene, FLCN, leading to skin tumours (fibrofolliculomas), renal tumours and pulmonary cysts. Lung involvement is predominantly observed in 70% of the cases of BHDS, manifesting in the form of recurrent primary spontaneous pneumothorax. This video tutorial showcases the surgical management of recurrent right primary spontaneous pneumothorax in a young adult with a history of multiple episodes of bilateral pneumothorax managed by surgical intervention previously. The study undersco

Mitral valve therapy is rapidly evolving. In this context, there is a clear evolution towards the diffusion of minimally invasive techniques for surgical mitral valve replacement or repair, namely in the context of primary mitral regurgitation. The robotic-assisted mitral surgery strategy allows the greatest reduction in surgical trauma to the patients, as well as improved ergonomics and video-assistance for performance of the mitral procedure. We currently observe a rapid diffusion of robotic-assisted mitral valve surgery across Europe, which rightfully forms part of the treatment modalities

Thymectomy is a treatment option for new-onset or treatment-resistant myasthenia gravis in patients with muscle-type acetylcholine receptor autoantibodies, and it is the standard treatment of early-stage thymomas. However, patients with severe forms, particularly those with titin and ryanodine receptor antibodies, may require long-term immunosuppressive therapy instead. The optimal approach to thymectomy remains debated, with minimally invasive techniques like video-assisted and robot-assisted thoracoscopic surgery offering better perioperative outcomes. Single-port robot-assisted thymectomy h

The catamenial pneumothorax (CP) is defined as recurrent pneumothorax occurring from the day before menstruations until 72 hours after their beginning, but remains a diagnostic and therapeutic problem. We herein report the cases of two young women who presented several episodes of pneumothorax. The first patient (28 years old) underwent 18 recurrent pneumothorax before the diagnosis of CP. The video-assisted mini-thoracotomy found many diaphragmatic perforations, which were sutured after resection and biopsy. The latter patient underwent four pneumothorax before diagnosis of CP. Safety and feasibility of the self-made thoracic needled suspending device with a snare in the uniportal video-assisted thoracic lobectomy and segmentectomy for the treatment of non-small cell lung cancer were explored. In total, 80 pulmonary lung major resections (including lobectomy and segmental resections) with systematic mediastinal lymphadenectomy were retrospectively analyzed. Patients were randomly divided into an observation group and a control group. In the observation group, the device was used to hang affected lungs, left and right vagus nerve at the level of tracheal bifurcation,

BACKGROUND: Robot-assisted thoracoscopic surgery (RATS) is more precise and flexible than video-assisted thoracoscopic surgery (VATS) for early-stage non-small cell lung cancer (NSCLC) treatment. This study compared the early postoperative functional recovery of patients who underwent triportal RATS with that of patients who underwent uniportal video-assisted thoracic surgery (UVATS) for segmentectomy.
METHODS: This observational, prospective study included 172 patients with clinical stage I or II peripheral NSCLC who underwent RATS or UVATS segmentectomy. Propensity score matching (PSM) was

BACKGROUND: Carinal resection and reconstruction are complex surgical procedures often necessitated by tumors or other pathologies involving the tracheobronchial junction. Traditional approaches to these surgeries are highly invasive. The advent of uniportal video-assisted thoracoscopic surgery (VATS) along with the integration of extracorporeal membrane oxygenation (ECMO) offer potential advantages in reducing surgical trauma and improving outcomes.
CASE PRESENTATION: A 42-year-old female patient was admitted to the hospital with the chief complaint of "chest tightness for 20 days". Enhanced

There are few data on current trends in pain management for thoracic surgery in Australia and New Zealand. Several new regional analgesia techniques have been introduced for these operations in the past few years. Our survey aimed to assess current practice and perceptions towards various modalities of pain management for thoracic surgery among anaesthetists in Australia and New Zealand. A 22-question electronic survey was developed and distributed in 2020 with the assistance of the Australian and New Zealand College of Anaesthetists Cardiac Thoracic Vascular and Perfusion Special Interest Gro

A 40-years-old Japanese female suffering from misswallowing and hoarseness, who had undergone hepatectomy due to hepatocellular carcinoma(HCC) 4 years before, revealed solitary subaortic mass in the chest computed tomography scan and fluorodeoxyglucose-positron emission tomography. The mass was surgically resected by video assisted thoracoscopic surgery and pathological examination revealed lymph node metastasis of HCC. Symptoms of recurrent nerve palsy disappeared after surgery.

BACKGROUND: Major vascular complications during video-assisted thoracoscopic surgery (VATS) are uncommon, and complete transection of a non-target pulmonary vein by an endoscopic stapler is exceedingly rare. When venous drainage of a preserved lobe is abruptly interrupted, rapid pulmonary congestion and life-threatening complications may ensue. Optimal management is particularly challenging when direct reconstruction is not technically feasible and additional lung resection is poorly tolerated.
CASE PRESENTATION: A 76-year-old man with left lower lobe lung cancer and moderate-to-severe obstru

BACKGROUND: Robotic-assisted thoracic surgery (RATS) is a minimally invasive procedure; however, some patients experience persistent postoperative pain. This study aimed to investigate factors related to postoperative pain following RATS.
METHODS: The data of 145 patients with lung cancer, who underwent RATS with a four-port (one in the sixth intercostal space [ICS] and three in the eighth ICS) lobectomy or segmentectomy between May 2019 and December 2022, were retrospectively analyzed. Factors associated with analgesic use for at least 2 months following postoperative pain (PTP group) were a

Surgery for mediastinal and chest wall tumors requires various approaches, including open and thoracoscopic, depending on the size and localization of the tumor. While robotic surgery for anterior mediastinal tumors has become a standardized approach, the approaches for tumors of the superior, middle, and posterior mediastinum, in particular, have not been generalized. Our institution introduced robotic surgery in 2017 and has performed 785 robot-assisted surgeries until November 2022. In this report, we describe our clinical experience with robotic surgery for mediastinal tumors, which requir
