This article presents the core principles and value of a family and community resilience-oriented approach to recovery from traumatic loss when catastrophic events occur. In contrast to individually based, symptom-focused approaches to trauma recovery, this multisystemic practice approach contextualizes the distress in the traumatic experience and taps strengths and resources in relational networks to foster healing and posttraumatic growth. The intertwining of trauma and traumatic losses is discussed. Key family and social processes in risk and resilience in traumatic loss situations are outl

Victims of terror attacks, whether or not physically injured, sometimes suffer long-term posttraumatic symptoms, although the intensity of symptoms differs among individuals. Often, after discharge from the hospital, additional posttraumatic symptoms and emotional distress are evident, together with difficulty in readjusting to a normal life. This paper describes an ongoing multi-faceted program to empower victims and their families and assist them on the journey to recovery. The program is operated by the social work department in one of the main hospitals in Israel, in alliance with a volunt

The Harry Potter books are centered around the psychic development of a traumatised young boy starting from his internalised early experience of being loved by his mother to the sudden loss of both of his parents at the anal stage and the later cumulative traumatisation by being negleced and mistreated by his relatives up to the present times. By identifying with his father and other father replacements Harry Potter finally acquires self-assurance and wins new friends. Harry goes through different stages of initiation into the adult world with all its conflicts and rivalries but also friendshi

CONTEXT: Arts-based knowledge mobilisation (ABKM) is a powerful means of interpreting and communicating health services research, centreing experiential, iterative, and reflective knowledge rooted in co-creation. We developed a research-to-public-to-research feedback loop methodology extending beyond existing ABKM practices and our previous research-to-public methodology, creating a novel bidirectional epistemic process wherein meaning making occurs from research to public and from public to research. This approach produces knowledge that is rich, contextually grounded, and otherwise potential

BACKGROUND: Community engagement in maternal and perinatal death surveillance and response (MPDSR) could support health systems in providing people-centred care and ensure accountability for the prevention of maternal and perinatal deaths. Although community engagement activities in MPDSR have been described, the literature does not adequately explain which community engagement in MPDSR strategies succeed, the contexts in which they work, the outcomes they produce, and for whom.
METHODS: We conducted a realist review, which involved the identification and refinement of programme theories. An

AIMS: Those who experience emotional and behavioural difficulties or mental health problems, are unemployed or bereaved, or have been in trouble with the law can be socially marginalized and can become isolated from their families, community and wider society. This paper explores the role of active hands-on contact with the environment through a general environmental volunteering programme and through a targeted therapeutic volunteer programme, highlighting how these two approaches can potentially aid some marginalized people to reintegrate into society.
METHODS: This study draws on concepts

Prenatal diagnosis of congenital heart disease (CHD) causes parental distress, at least in part due to uncertainties regarding the diagnosis, prognosis, treatment, and family impact. In initial fetal cardiology consultations, over half of dialogue focuses on uncertainties, and clinician communication can influence how parents cope. No prior study has gathered parental perspectives on what would be helpful when communicating about uncertainties in these initial conversations. The objective of this study was to gather parental perspectives on optimal clinician-caregiver communication about uncer

BACKGROUND: People with advanced illness at home, and their families, rely on 'out-of-hours' services provided by community, primary and specialist palliative care services. Home is commonly expressed as the preferred place to be cared for and die, and an increasing proportion of people are dying at home, but what constitutes 'good' care is poorly understood from the combined perspectives of healthcare professionals and patients and family caregivers.
OBJECTIVE: To understand the convergence and divergence of the perspectives of healthcare professionals with those of patients and family careg

BACKGROUND: According to recent legislation, facilitated advance care planning (ACP) for nursing home (NH) residents is covered by German sickness funds. However, the effects of ACP on patient-relevant outcomes have not been studied in Germany yet. This study investigates whether implementing a complex regional ACP intervention improves care consistency with care preferences in NH residents.
METHODS: This is a parallel-group cluster-randomized controlled trial (cRCT) with 48 NHs (≈ 3840 resident beds) between 09/2019 and 02/2023. The intervention group will receive a complex, regional ACP int

BACKGROUND: In sub-Saharan Africa, common mental disorders (CMDs) like depression and anxiety are under-investigated amongst young people living with HIV (YLWH). To address the gap, in Kenya we: a) determined the prevalence of CMDs among YLWH compared to their uninfected peers; b) investigated HIV status as an independent predictor of CMDs in young people; c) investigated CMDs risk and protective indicators with more focus on YLWH.
METHODS: Between November 2018 and September 2019, 819 young people aged 18-24 years (407 HIV-infected) were recruited from two Counties on the Kenyan coast. Local

BACKGROUND: Armed conflict exposes children to prolonged and multifaceted adversity, including violence, displacement, bereavement, family disruption, poverty, insecurity, and disrupted schooling. Although research on psychosocial interventions for conflict-affected children has grown, the evidence remains heterogeneous, and causal conclusions are often difficult to distinguish from feasibility, contextual, or implementation findings.
OBJECTIVE: This systematic interpretive mapping review aimed to synthesize evidence on psychosocial interventions for children affected by armed conflict by map…

There have been a striking diagnostic inflation and a corresponding increase in the use of psychotropic drugs during the past 30 years. DSM-5, scheduled to appear in May 2013, proposes another grand expansion of mental illness. In this article, we will review the causes of diagnostic exuberance and associated medical treatment. We will then suggest a method of stepped care combined with stepped diagnosis, which may reduce overdiagnosis without risking undertreatment of those who really need help. The goal is to control diagnostic inflation, to reduce the harms and costs of unnecessary treatmen

Research on body image and older women has grown in the past decade. However, there is a gap in the literature regarding body image, older women, and countertransference. This article provides 7 case examples of racially and ethnically diverse women over 60, drawn from MSW student and agency staff supervision, and participant feedback from a national conference on aging workshop. Themes related to loss and grief, adult daughter and aging mother issues, incest, anger, disability, personality disorders, phobic reactions, and shame are discussed. OBJECTIVES: Pediatric intensive care unit (PICU) nurses spend the most time with patients and families during a critical illness. However, nurses, families, and other clinicians conceptualize suffering differently, leaving some types of suffering when a child is nearing the end-of-life (EOL) unaddressed. PICU nurses are thus well-positioned but underequipped to address suffering during EOL. We aimed to 1) characterize distinctive features that indicate the presence and/or absence of suffering; and 2) identify nursing care responses that help attend to EOL suffering.
METHODS: Using qualitative

BACKGROUND: Chronic refractory breathlessness is distressing, disabling, and common in advanced disease. Unpaid/family carers provide support for patients, but often lack knowledge and confidence, experiencing anxiety which can lead to crises. The Learning about Breathlessness programme (LaB) sought to address this. Working with carers and clinical experts, the LaB1 Study identified six topics carers of patients with breathlessness due to cancer or chronic obstructive pulmonary disease (COPD) want to learn about breathlessness, and how they want to learn. To accommodate carers' varied learning
