Allergic Contact Dermatitis, Allergic Airborne Dermatitis, and Occupational Asthma Caused by (meth)acrylates in Artificial Nails.

Allergic contact dermatitis (ACD) caused by (meth)acrylates is traditionally an occupational disease among dentists, printers, and fiberglass workers. With the use of artificial nails, cases have been reported both in nail technicians and in users. ACD caused by (meth)acrylates used in artificial nails is a relevant problem for both nail artists and consumers. We present the case of a 34-year-old woman who was working in a nail art salon for two years prior to the appearance of severe hand dermatitis, especially on her fingertips together, with frequent appearance of face dermatitis. The patie…

Glutamine suppresses dinitrophenol fluorobenzene-induced allergic contact dermatitis and itching: inhibition of contact dermatitis by glutamine.

BACKGROUND: Cytoplasmic phospholipase A(2) (cPLA(2)) is importantly implicated in a variety of inflammatory diseases by liberating arachidonic acid from phospholipids. The increased cPLA(2) activities as well as increased levels of cPLA(2) metabolites are associated with pathogenesis of many inflammatory skin disorders including atopic dermatitis. The non-essential amino acid l-glutamine (Gln) has been reported to have an anti-inflammatory activity. Regarding the molecular mechanism of Gln, we have recently shown that Gln effectively inhibits cPLA(2) phosphorylation and activity. OBJECTIVE: T…

Fiddler's neck: Chin rest-associated irritant contact dermatitis and allergic contact dermatitis in a violin player.

Fiddler's neck refers to an irritant contact dermatitis on the submandibular neck of violin and viola players and an allergic contact dermatitis to nickel from the bracket attaching the violin to the chin rest on the violinist's supraclavicular neck. A 26-year-old woman developed submandibular and supraclavicular left neck lesions corresponding to the locations of the chin rest and bracket that was attached to her violin that held it against her neck when she played. Substitution of a composite chin rest, which did not contain nickel, and the short-term application of a low potency topical cor…

Ulcerative contact dermatitis caused by sodium silicate. Coexistence of primary irritant contact dermatitis and contact urticaria.

A 57-year-old man had had recurrent ulcerative lesions on his left hand for two years. The ulcers were associated with chronic eczematous changes resulting from primary irritant contact dermatitis to sodium silicate, as indicated by positive patch tests. The patient also had another type of cutaneous reaction to sodium silicate, ie, contact urticaria. An immediate wheal and flare reaction was found 15 minutes after application of sodium silicate to a scratch test site. This response was not seen in healthy control subjects. To our knowledge, the coexistence of primary irritant contact dermatit…

Merkel Cell Carcinoma Masquerading as Dermatitis.

Merkel cell carcinoma (MCC) is a rare, highly aggressive neuroendocrine skin carcinoma that typically presents as a rapidly enlarging, firm, painless nodule on sun-exposed areas in elderly patients. This malignancy is characterized by high rates of local recurrence, regional spread, and distant metastasis. We report a case of a woman in her 80s who presented with an erythematous patch on the left anterior thigh, a non-sun-exposed site, initially diagnosed as inflammatory dermatitis and treated with topical corticosteroids. After two months of failed therapy and interval growth, a punch biopsy …

New Landscape for Atopic Dermatitis Therapies.

Targeted therapies have revolutionized the treatment of atopic dermatitis (AD) by offering patients highly effective disease control with complete or near-complete resolution. The American Academy of Dermatology updated their 2014 AD treatment guidelines in 2023. Since 2014, the treatment of AD has evolved immensely with the advent of biologics and small molecule inhibitors. While the foundation of AD treatment has stayed relatively consistent (ie, gentle skin care, emollients, and topicals), new therapies have supplanted previous systemic agents given their superior efficacy and safety profil…

T-cell mechanisms in atopic dermatitis.

This review provides an overview of recent advances in understanding T-cell inflammation in atopic dermatitis (AD), a common chronic inflammatory skin disease. After recognizing their cognate antigen in the acute phase of the disease, the homing of T cells from the circulation into the skin via cutaneous lymphocyte antigen (CLA) is the prerequisite to skin inflammation and subsequent systemic and local, tissue resident, memory (TRM) formation. Initial observations suggest that antigen presentation occurs in structures such as induced skin-associated lymphoid tissue (iSALT), in addition to lymp…

The primary mechanisms underlying atopic dermatitis.

INTRODUCTION: Atopic dermatitis (AD) is a complex skin disease frequently linked with other atopic symptoms such allergic rhinitis and asthma. The disease's history consists of persistent relapses with extreme pruritus, which lowers quality of life. AD has become a global health concern as its incidence has increased over the last few decades. It ranks as the third most common dermatologic disorder. AIM: There are several open questions about the mechanisms underlying atopic dermatitis (AD), This review aims to emphasize the recent advances in scientific research regarding the pathophysiologi…

Neurogenic Inflammation in Allergic Contact Dermatitis.

Allergic contact dermatitis (ACD) is a skin condition characterized by inflammation resulting from hypersensitivity upon contact with certain allergens. Although ACD is characterized by an immune-mediated pathomechanism, the involvement of the nervous system in this condition has increasingly been considered, particularly in the amplification and persistence of inflammation. This paper aims to present a comprehensive overview of the mechanisms involved in neurogenic inflammation in ACD, focusing on the role of sensory neurons, the release of neuropeptides, their interaction with immune cells, …

Radiation Recall Dermatitis Following Capivasertib Administration.

Radiation recall dermatitis is a known but rare adverse effect that is characterized by the development of dermatitis in the region of prior irradiated tissue triggered by exposure to a systemic agent. Capivasertib is a small-molecule inhibitor targeting the phosphatidylinositol 3-kinase/protein kinase B pathway recently approved in locally advanced and metastatic breast cancer; however, the safety of its use in the setting of palliative radiation is currently unclear. Here, we report a case of radiation recall dermatitis in a patient with metastatic breast cancer on capivasertib with history …

Periorificial dermatitis: Pathophysiology, diagnosis, and management.

Periorificial dermatitis is a chronic papulopustular facial dermatitis. Despite being frequently encountered in clinical practice and associated with significant effects on quality of life, its etiology remains incompletely understood, and no therapies are specifically approved by the US Food and Drug Administration for its treatment. In this review, the current mechanistic understandings of periorificial dermatitis pathogenesis are discussed, including the role of inciting factors, skin barrier dysfunction, inflammation, and the microbiome. In addition, we review the diagnostic features of pe…

Patch Testing in Allergic Contact Dermatitis.

BACKGROUND: Allergic contact dermatitis is one of the most common forms of skin diseases that require medical intervention. Appropriate detection of allergens by patch test and accurate avoidance of them is the key to management. The objective of this study is to determine the types and frequency of allergens responsible for Allergic contact dermatitis in a tertiary hospital of Nepal. METHODS: Altogether 120 patients with Allergic contact dermatitis were enrolled in the study. Patch test was performed to find out the most common inciting allergen, utilizing the Indian Standard Series of aller…
