Absent stapedial reflex: otosclerosis or middle ear tumor?

We present an unusual case in which a patient diagnosed as having otosclerosis on the basis of clinical and audiologic findings actually had a middle ear facial nerve schwannoma. To the best of our knowledge, this is the first reported case in English literature in which a facial nerve schwannoma presented with conductive deafness of gradual onset and absent stapedial reflex with a normally functioning facial nerve. We also include a review of the literature.

[Reconstructive mastoidoplasty following sanitation surgery on the ear].

The objective of the present study was to enhance the effectiveness of surgical rehabilitation of the patients presenting with "operated ear disease". Sixty four patients underwent sanitation surgery on the ear by means of plastic correction with the use of a muscular-periosteal flap on a feeding pedicle with the simultaneous restoration of the posterior wall of the external auditory canal by the implantation of septal autocartilage. The surgical technique is described. The analysis of long-term results of the treatment of such patients has demonstrated its high efficacy.

Periorbital Ecchymosis and Subconjunctival Hemorrhage following Ear Surgery.

Objective. To evaluate the occurrence of two periorbital complications of surgery for Chronic Suppurative Otitis Media (CSOM) and discuss the potential pathophysiologic mechanisms. Materials and Methods. This is a retrospective review of the CSOM surgeries performed between Oct, 2005, and Jan, 2011, in our hospital. The early postoperative conditions of the patients were scrutinized to identify periorbital ecchymosis and subconjunctival hemorrhage. Results. Eight cases out of 756 patients were noted to have periorbital ecchymosis, and two of the patients also had simultaneous subconjunctival h…

ALH-L1005 attenuates endotoxin induced inner ear damage.

OBJECTIVE: To assess whether this compound (ALH-L1005) is conceivably an effective agent in protecting against cochlear damage induced by LPS. MATERIALS AND METHODS: Tube formation using human umbilical vein endothelial cell (HUVEC) and matrix metalloproteinase (MMP)-9 inhibition assay was performed. 24 guinea pigs were randomly divided into three groups. Intratympanic instillation of LPS (n=8) as negative control, instillation of oxytetracycline 1h after LPS as positive control (n=8), and intratympanic instillation of ALH-L1005 (n=8) 1h after LPS were considered experimental group. Evaluatio…

Large jugular bulb abnormalities involving the middle ear.

OBJECTIVE: Jugular bulb abnormalities (JBA), such as jugular bulb diverticula (JBD) or large jugular bulbs, rarely present in the middle ear. We review a large series of temporal bone histopathologic specimens to determine their prevalence and present a series of cases of JB abnormalities involving the middle ear (JBME) that shed light on the probable mechanism for their development. PATIENTS: 1,579 unique temporal bone specimens and individuals with radiographically-diagnosed JBME. INTERVENTION: Histopathologic and clinical review of temporal bone specimens and patient presentations, radiog…

Temporomandibular joint fistula into the external ear canal.

BACKGROUND: Temporomandibular joint fistula into the external auditory canal is a very rare condition. We report a case of spontaneous fistula which presented with repeated episodes of clear fluid otorrhoea and ear infection. CASE REPORT: A 53-year-old woman complained of occasional episodes of clear fluid otorrhoea from the right ear followed by infection. Otoscopic examination showed a normal tympanic membrane and normal skin in the external auditory meatus. Otomicroscopy showed a small punctum on the anteroinferior part of the meatus. Computed tomography identified a foramen of Huschke. Su…

Inner ear dysfunction in caspase-3 deficient mice.

BACKGROUND: Caspase-3 is one of the most downstream enzymes activated in the apoptotic pathway. In caspase-3 deficient mice, loss of cochlear hair cells and spiral ganglion cells coincide closely with hearing loss. In contrast with the auditory system, details of the vestibular phenotype have not been characterized. Here we report the vestibular phenotype and inner ear anatomy in the caspase-3 deficient (Casp3(-/-)) mouse strain. RESULTS: Average ABR thresholds of Casp3(-/-) mice were significantly elevated (P < 0.05) compared to Casp3(+/-) mice and Casp3(+/+) mice at 3 months of age. In DPOA…

Reconstruction following traumatic partial amputation of the ear.

BACKGROUND: Reconstruction following traumatic amputation of the external ear remains a unique challenge to the plastic surgeon. The authors report a series of ear reconstructions with autologous costal cartilage in patients following traumatic partial amputation of the ear. Technical points regarding the carving of the cartilage framework and methods of skin coverage are discussed. METHODS: Fifty partial ear reconstructions with autologous costal cartilage were performed over a 4-year period. All patients had suffered previous traumatic amputation of part of the external auricle due to bite …

Nonmicrosurgical single-stage auricular replantation of amputated ear.

Traumatic auricular amputation due to human bite is an uncommon event. Nonetheless, it poses a difficult challenge for the reconstructive surgeon. Although microsurgery must be performed in some cases, most microsurgical techniques are too complex and must be performed in specialized centers only. Replantation of a severed ear without microsurgery can be a safe alternative as long as a proper technique is selected. Here, 2 successful ear replants performed without microsurgical technique are described. The ear cartilage was used with the perichondrium after degloving of the skin from the carti…

Indications and candidacy for active middle ear implants.

Currently, there are two active middle ear implants available commercially: the Vibrant Soundbridge system and the Carina system. A third active middle ear implant, the Esteem, is under clinical evaluation. All devices are indicated for patients with moderate-to-severe hearing loss. Because active middle ear implants are directly coupled to middle ear structures, many of the problems that patients with conventional hearing aids report, such as acoustic feedback, occlusion, and irritation of the outer ear canal, are avoided. In addition, AMEI patients perform well in background noise. However, …

Modification of closed tympanoplasty in middle ear cholesteatoma.

BACKGROUND: Classic mastoidectomy and modified mastoidectomy are traditional surgical procedures for middle ear cholesteatoma with goals of eradicating diseases, creating dry ears and preventing severe complications. However, the drawback of these procedures is the lack of hearing improvement. Therefore, our study aimed to investigate the effects and safety of a modification of closed tympanoplasty for middle ear cholesteatoma. METHODS: Eighty-three patients were recruited in this study based on the following two criteria: each patient had middle ear cholesteatoma in one ear; the affected ear…

Ear, nose, and throat effects of high altitude.

High altitude changes human physiology and can result in illnesses such as acute mountain sickness, high-altitude cerebral edema, and high-altitude pulmonary edema. The physiological impacts of high-altitude illnesses occur secondary to extravasation of fluid from the intravascular space into the extravascular space during a rapid ascent. Headache, hearing disturbances, vestibular disturbances, epistaxis, sleep apnea, coughing, respiratory tract infections, and nasal obstruction are main ear, nose, and throat complaints of individuals travelling to high altitude. These complaints can cause del…

[Application of quantitative tissue expansion in ear reconstruction].

OBJECTIVE: To explore the application of tissue expander in ear reconstruction and to deal with the complications. METHODS: 50 ml kidney-shape tissue expanders were implanted subcutaneously in the mastoid area. The drainage tube was removed 3 days after operation. The suture was removed 10 days later. Since 7 days after operation, 5 ml NS was injected into the expander every time, three times a week. The total injection volume was about 60 ml. After that, the expander was maintained for one month. RESULTS: From January 1992 to December 2006, 5,248 patients of microtia were treated with 6,252…

Inner ear protection and regeneration: a 'historical' perspective.

PURPOSE OF REVIEW: In evaluating strategies to preserve or regenerate the cochlea, understanding the process of labyrinthine injury on a cellular and molecular level is crucial. Examination of inner ear injury reveals mechanism-specific types of damage, often at specific areas within the cochlea. Site-specific interventions can then be considered. RECENT FINDINGS: The review will briefly summarize the historical perspective of advancements in hearing science through 2006. Areas of research covered include hair cell protection, hair cell regeneration, spiral ganglion cell regeneration, and str…
