The three cutaneous reaction patterns of erythema multiforme, Stevens-Johnson syndrome, and toxic epidermal necrolysis share many clinical features. Each is multifactorial in etiology; together they account for most severe cutaneous drug eruptions. In some respects, they can represent a progression in the severity of drug eruptions. Controversy continues to cloud the clinical delineation, etiology, and treatment.

BACKGROUND: Rowell syndrome (RS) is an uncommon condition characterized by erythema multiforme (EM)-like lesions and lupus erythematosus. It is more common in females, and EM may be the first manifestation of the disease with positive autoantibodies, such as antinuclear antibody (ANA), SSA, SSB and rheumatoid factor. Systemic fungal diseases are primary pulmonary diseases caused by the dimorphic fungal pathogens, Blastomyces dermatitides, Coccidioides immitis. Many systemic diseases have cutaneous manifestations. In some diseases skin involvement is the predominant factor (Behçet's syndrome, urticaria pigmentosa, discoid lupus erythematosus and pseudoxanthoma elasticum); in others the skin manifestations, when present, are an important part of the condition (sarcoidosis, systemic lupus erythematosus, hypersensitivity angiitis, porphyria). Reactive infectious mucocutaneous eruption is a new classification as of 2021. It describes a parainfectious eruption that may be difficult to distinguish from Stevens-Johnson syndrome, toxic epidermal necrolysis, and erythema multiforme in its early stages. We present a likely fifth recurrent case in a young adult male, who was treated empirically with his condition improving three days post-admission.

A patient is described who initially presented with an acrovesicular eczema which subsequently developed into erythema multiforme with histopathological features of bullous pemphigoid. Although the various laboratory studies pointed to the diagnosis of paraneoplastic pemphigus (PNP), the underlying neoplasm was not detected until 6 months later, when the biopsies of an oral lesion showed the presence of an underlying non-Hodgkin lymphoma.

A survey of Australian dermatologists was conducted into skin reactions observed to diltiazem. These have included: erythema multiforme, subcorneal pustular dermatosis, photosensitive erythroderma, macular exanthem, allergic vasculitis and urticarial vasculitis. The range of clinical and histopathological features are described. A review of the literature is presented. Photosensitive erythroderma and urticarial vasculitis have not, to our knowledge, been reported in the literature previously.

Vesiculobullous lesions of the upper aerodigestive tract are among the most frequently occurring disease processes of the oral soft tissues. Pemphigus vulgaris, mucous membrane pemphigoid, erythema multiforme, and erosive lichen planus are compared and contrasted with each other and with vesiculobullous eruptions of viral origin. Clinical photographs and photomicrographs of these disease processes are presented. Treatment modalities are briefly discussed.

Four patients with unusual features of acute febrile neutrophilic dermatosis (Sweet's syndrome) are reported. This syndrome is more common than is apparent from the small number of case reports. It has well-defined clinical and histological criteria, but may be considered within the erythema multiforme spectrum of reaction patterns. The clinical presentation and course can vary, as illustrated by these 4 case reports.

Imiquimod 5% cream is an approved treatment for actinic keratoses, superficial basal cell carcinomas, and anogenital warts. Severe systemic side effects associated with imiquimod 5% cream are rare, although a few cases of erythema multiforme and Stevens-Johnson syndrome have been described. Toxic epidermal necrolysis (TEN) is a serious cutaneous reaction and is most commonly drug induced. It is associated with significant morbidity and mortality. We describe a patient who developed fatal TEN after re-exposure to lansoprazole. Three years previously he presented with erythema multiforme due to the same drug. To our knowledge this is the first published report of TEN occurring with lansoprazole.

An ostensibly normal 43-year-old man had intermittent fever, erythema multiforme, and leukopenia that evolved into fulminant hepatic and bone marrow necrosis. The cause of his illness was compatible with cytomegalovirus (CMV) infection since there was a fourfold increase in immunofluorescent antibody titer. This report suggests that CMV can produce a rapidly progressive and overwhelming infection in an apparently healthy adult.
