Applying a hexapod circular frame to gradually correct persistent knee flexion due to congenital aplasia of the quadriceps-a case report and review of the literature.

Congenital aplasia or hypoplasia of the quadriceps muscle are rare conditions that significantly impair the extension of the knee joint, resulting in a sustained flexion deformity. These conditions alter overall gait quality, impede daily activities and require effective, timely intervention. Surgical techniques for correction have been described, including soft-tissue release, guided growth, tendon transfer and external fixators. Among the external fixators available, hexapod circular frames are probably the most effective because they enable the precise, progressive correction of complex mul…

Toxic shock syndrome as a complication of orthopaedic surgery.

Toxic shock syndrome (TSS) was initially described by Todd et al. in 1978. TSS as a complication of orthopaedic surgery was reported in 1984. There have been previously a total of nine cases of TSS reported in orthopaedic patients. These patients presented at an average of 13 days postoperatively compared to 2 days for general surgical patients. Patients with external fixators, however, presented an average of 25 days postoperatively. Menstrual TSS and nonmenstrual TSS present similarly; however, the fatality rate is reported as 10 and 50%, respectively. There was a 27% case fatality rate in o…

Use of antibiotic-cement coated plate in the treatment of Gustilo-Anderson type III long bone fractures in low- and middle-income countries vs external fixation: A multicentre randomized control trial study protocol.

BACKGROUND: One of the great challenges in the management of open fractures is postoperative infection with a higher incidence in Gustilo-Anderson type III fractures. Definitive management of such fractures in developing countries is usually with external fixators with its attendant complications such as deep fracture-related infection, non-union, and consequent increased re-operation rates. Recently, there has been a novel method of using antibiotic-cement coated implants such as intramedullary nails and locking plates in the treatment of infected non-unions with reported excellent outcomes. …

Surgical options for regaining motion include external fixators and open surgical release. Although they can lead to improved extension at the proximal interphalangeal joint, external fixators carry a risk of reduced finger flexion and pin site infection. TCM treatment of allergy induced by stainless steel implants for tibiofibular fracture: A case report.

BACKGROUND: Metal allergy is frequently seen. Orthopedic metal implants, such as external fixators or other stainless implants, contain chromium, nickel, and molybdenum, which can cause type IV hypersensitivity. CASE SUMMARY: A patient diagnosed with open comminuted tibiofibular fracture was treated with external fixation surgery, and she showed contact dermatitis and eczema-like symptoms 2 weeks postoperatively. She was then diagnosed as allergic to several metals by patch test and subsequently treated with traditional Chinese medicine (TCM), both orally and externally for 1 month. TCM treat…

Comparison of the results of treatment of humeral shaft fractures by different methods.

The aim of this study was to compare the functional results of non-operative and different operative techniques of managing humeral shaft fractures. The average follow-up was one year after the trauma in 61 patients with united fracture, where the initial method of treatment was the definitive one. The patients were divided into four groups: 22 were treated with hanging cast, 20 with intramedullary nails, 10 with dynamic-compressive plates and screws and 9 with external fixators. We estimated the presence of bone union by radiographic and clinical evaluation and compared the results with Const…

Biological activation of prolonged delayed fracture healing following abaloparatide under stable circular external fixation: successful limb salvage of a Gustilo type III-C pilon fracture in severe immune thrombocytopenia-a case report.

BACKGROUND: Delayed fracture healing may persist despite adequate mechanical stability, reflecting biological insufficiency rather than mechanical failure. Although abaloparatide has osteoanabolic effects, its role in prolonged delayed fracture healing remains unclear. CASE PRESENTATION: A 59-year-old Japanese man with severe immune thrombocytopenia (platelet count approximately 5000/microL) sustained a Gustilo type III-C open pilon fracture after a crush injury. Emergency management included aggressive debridement, posterior tibial artery and vein repair, and circular external fixation. Beca

Evaluation of criteria for temporary external fixation in risk-adapted damage control orthopedic surgery of femur shaft fractures in multiple trauma patients: "evidence-based medicine" versus "reality" in the trauma registry of the German Trauma Society.

BACKGROUND: Femur-shaft fracture treatment (FSFT) follows controversial management concepts after multiple trauma: primary-definitive osteosynthesis, secondary-definitive osteosynthesis after temporary external fixation (EF) in all patients, or individualized primary- or secondary-definitive osteosynthesis ("risk-adapted damage control orthopedics"). This study compares the concepts by analyzing literature evidence and a prospective multicenter database. METHODS: A systematic literature analysis was performed. The German Trauma Society trauma registry was used to assess variables predictive o

Open reduction capsular arthroplasty using a modified Codivilla-Hey Groves-Colonna procedure augmented with tensor fascia lata graft, supported by Ilizarov hinged external fixator in a patient with chronic traumatic posterior hip dislocation.

Untreated traumatic hip dislocation is a rare condition in children. Capsular arthroplasty is a previously abandoned surgical technique in dealing with developmental dysplasia of the hips but not described in traumatic dislocations. We present a 2-year follow-up of a 7-year-old boy who sustained chronic traumatic posterior hip dislocation treated as a first case in published literature combining a modified version of the Codivilla-Hey Groves-Colonna procedure, using tensor fascia lata graft and application of a hinged Ilizarov frame. The patient is currently independent on all activities of da
