We present a 45 year-old man with an eight-year history of discoloration of the nail plate on his left hallux. He had been treated with two courses of oral terbinafine and topical 8% ciclopirox for presumed onychomycosis. On exam, his left great toenail contained a wide yellow-white longitudinal band involving a majority of the nail plate. METHODS: A post-hoc analysis of 112 patients, aged 29-70 years, randomized to receive efinaconazole topical solution, 10% or vehicle from two identical multicenter, double-blind, vehicle-controlled 48-week studies evaluating safety and efficacy. The diagnosis of onychodystrophy has increased in dermatology consultation. This could be due to its esthetic effect, pain or disability, physician awareness for detecting the disease in the context of dermatologic or systemic diseases, or a greater incidence. One of the first differential diagnoses that should be considered when an onychodystrophic nail is observed is onychomycosis, which is the cause in 50% of cases.

The efficacy of terbinafine (250 mg/day) in the treatment of toenail onychomycosis was evaluated in a large open-label, multicenter trial of 12, 18, and 24 weeks of therapy. A new antimycotic drug lamisil (Sandoz, Switzerland) was given as pills to 35 patients with pedal mycosis (23 of them suffered from onychomycosis). Pedal mycosis was cured for 2 weeks, onychomycosis for 12 weeks. In 2 cases the drug was discontinued because of gastrointestinal toxicity. By tolerance and efficacy, lamisil is superior to griseofulvin and nizoral.
