Complications occurring with jejunostomy feeding tubes are common and sometimes life-threatening. We describe a case of peristalsis-induced intraluminal antegrade migration of a jejunostomy tube's distal end with concomitant retrograde movement of the small bowel over the tube, which was first discovered at autopsy. This ultimately resulted in the jejunostomy feedings entering the distal ileum, therefore bypassing most of the small intestine. Two puppies, each with fractures of the seventh lumbar (L7) vertebra, had their vertebral canals aligned surgically with the aid of a Senn retractor. The retractor was used to provide leverage during vertebral manipulations. Surgical fixation was achieved using Steinmann pins and polymethylmethacrylate. Both puppies were improved immediately after surgery and were clinically normal six months after surgery.

OBJECTIVE: We investigated the spontaneous course and the possibility of transhepatic removal of displaced biliary stents.
MATERIALS AND METHODS: Displaced biliary stents were observed in 11 patients (13-75 years old) between October 1988 and August 1996. Stent types included the Palmaz stent (n = 3), Wallstent (n = 3), and the Strecker stent (n = 5). We are going to present a special case of head injury caused by a gunshot. In this case it resulted in a fracture of the skull, but the bullet did not penetrate the skull. It was deflected by the bone, leaving the body at an angle. But bone fragments, acting as secondary bullets, penetrated the brain. In spite of a massive cerebral trauma and brain injury, no retrograde amnesia could be diagnosed. The patient recovered to such an extent, that he could return to his former job. Special characteristics of head injuries caused by bullets will be referred to.

This report describes the autopsy findings in a young man who died after having been shot with a shotgun from about 30 m. Although not suspected clinically, a right middle cerebral artery territory infarct was found; its cause was shown to be a shotgun pellet embolus that had lodged just proximal to the trifurcation of the right middle cerebral artery. The case underscores the importance of performing a thorough postmortem examination, including a careful study of cerebral vasculature, in instances of brain infarction.

Vesicovaginal fistulas in three patients and a vesicocutaneous fistula in one patient were treated by percutaneous transrenal occlusion of the ureter with a commercially available silicone occluder (Angiomed, Karlsruhe, Germany) which was secured by means of histoacryl placed on top. Urinary flow was diverted by a permanent nephrostomy tube. We observed no complications (e.g., urinary tract infection, occluder migration, or recurrence of urine discharge) at an average follow-up period of 9.1 months.

Between 1986 and 1989, 12 patients underwent ureteric substitution with a Boari bladder flap at this Institute. The indications were ureteric injury following hysterectomy, difficult forceps delivery, difficult ureterolithotomy, ureteric strictures caused by a Dormia basket and previous ureteric surgery, tuberculosis, retroperitoneal fibrosis and a post-ureteric reimplantation fistula. A 58-year-old white male patient developed corneal complications following cataract surgery and implantation of a silicone posterior chamber intraocular lens. At the time of penetrating keratoplasty, the implant was noted to be partially dislocated into the anterior chamber, surrounded by a dense fibrovascular membrane. Despite a paucity of published clinical trials, use of the Angelchik antireflux prosthesis has increased dramatically since its introduction in 1979. Serious complications of this device have begun to appear. We dealt with two cases of erosion of the prosthesis into the gastrointestinal tract, one of which represents, to our knowledge, the first reported case of a gastroenteric fistula secondary to the device. Various examples of foreign body embolization of cerebral arteries, usually followed by serious consequences, have been reported (Lindberg et al., 1961; Chason et al., 1963; Steele et al., 1972; Wetli et al., 1972). However, a shotgun pellet entering the left atrium of the heart through a gunshot wound of the chest with subsequent embolic occlusion of one of the carotid arteries appears to be unique. Inverting serosa-to serosa staple applications may lead to intraluminal extrusion of staples. The possible clinical pathological consequences of such a migration are examined. Technical maneuvers designed to prevent errors and expedite safer handling of automatic stapling devices are presented.
