Clinical and economic implications of urinary tract infections.

Urinary tract infections represent one of the most frequent reasons for hospitalization. As a result of their prevalence from community-based origins as well as those which develop in hospital setting, this constellation of infections represents a tremendous burden to the global healthcare system. Areas covered: Over the last several decades the management of these infections has become more complicated due to the underlying comorbid conditions of the patients as well as escalating antimicrobial resistance to many of the most frequently used oral and parenteral agents. One such example is the

Outbreak of enterovirus 71 infection in Victoria, Australia, with a high incidence of neurologic involvement.

An outbreak of infections caused by enterovirus 71 occurred in southeastern Australia during the winter of 1986. Infection was confirmed by virus isolation or serology in 114 patients, 65 of whom were admitted to hospital. Fifty-one percent of inpatients were infants younger than 12 months old and 85% were younger than 5 years old. Many cases of hand, foot and mouth disease occurred in the community during the epidemic, but 51% (33 of 65) of patients admitted to hospital had central nervous system involvement, often associated with severe symptoms. Six patients had encephalitis and one had a p

Hospitalization Through Families' Eyes: Comparing Inpatient Care Quality for Children With Sickle Cell Disease and Cystic Fibrosis in Canada.

BACKGROUND: Sickle cell disease (SCD) is a chronic, inherited hemoglobinopathy that requires frequent hospitalization for disease-related complications. Canadian data on inpatient care is limited. This study compared caregiver-reported hospital experiences of children with SCD to those with cystic fibrosis (CF), a chronic, autosomal recessive condition with cumulative morbidities and frequent hospitalizations, within a provincial Canadian healthcare system. PROCEDURE: Caregivers of pediatric patients with SCD or CF, hospitalized at tertiary children's hospitals in Alberta, completed the Alber

From hospital to home: A heightened window of vulnerability post-critical illness.

BACKGROUND: Healthcare innovations have not kept pace with the burden of critical illness survivorship. The majority of patients treated in an intensive care unit (ICU) will survive but suffer new or worsening physical, cognitive or mental health sequelae, known as post-intensive care syndrome (PICS). For these survivors, the transition from hospital-based acute care to community-based care is often complex, with high rates of emergency department visits and unplanned hospital readmission. The purpose of this analysis is to describe ICU survivor and family caregiver experiences navigating the

Do health assets have a protective effect for hospitalized frail older adults?

BACKGROUND: Although increasing frailty is predictive of increased mortality and length of stay for hospitalized older adults, this approach ignores health assets that individuals can utilize to recover following hospital admission. AIM: To examine whether health assets mitigate the effect of frailty on outcomes for older adults admitted to hospital. DESIGN: Patients of 1418 aged ≥ 70 years admitted to 11 hospitals in Australia were evaluated at admission using the interRAI assessment system for Acute Care, which surveys a large number of domains, including cognition, communication, mood and

Loss of a limb: A consequence of traditional bone setting.

INTRODUCTION AND IMPORTANCE: Traditional bone setting (TBS) is commonly practiced in many communities worldwide, especially in developing countries in Asia, South America, and Africa, where healthcare resources are less developed. It plays an important role in healthcare delivery systems and remains a widely used alternative to modern medical care for limb fractures. This practice involves using wooden straps and casts to immobilize and stretch the injured limb, often leading to complications such as non-union, malunion, infections, compartment syndrome, and even limb amputations. Care coordination gaps due to lack of interoperability in the United States: a qualitative study and literature review.

BACKGROUND: Health information technology (HIT) could improve care coordination by providing clinicians remote access to information, improving legibility, and allowing asynchronous communication, among other mechanisms. We sought to determine, from a clinician perspective, how care is coordinated and to what extent HIT is involved when transitioning patients between emergency departments, acute care hospitals, skilled nursing facilities, and home health agencies in settings across the United States. METHODS: We performed a qualitative study with clinicians and information technology professi

Clinic attendances during the first 12 months of life for Aboriginal children in five remote communities of northern Australia.

BACKGROUND: The vast majority (>75%) of Aboriginal people in the Northern Territory (NT) live in remote or very remote locations. Children in these communities have high attendance rates at local Primary Health Care (PHC) centres but there is a paucity of studies documenting the reason and frequency of attendance. Such data can be used to help guide public health policy and practice. METHODS AND FINDINGS: Clinic presentations during the first year of life were reviewed for 320 children born from 1 January 2001-31 December 2006. Data collected included reason for infectious presentation, antib
