Posterior fracture-dislocation of the shoulder. A superior subacromial approach for open reduction.

We describe 11 cases of posterior fracture-dislocation of the shoulder that required open reduction and fixation. Difficulties with access through anterior approaches led us to use the superior subacromial approach. This is an extension of the approach often used to expose the rotator cuff; the joint is opened by splitting the supraspinatus tendon 5 mm behind the cuff interval. The glenoid, proximal humerus and any fracture fragments can be seen from above, allowing reduction of the dislocation and osteosynthesis to be performed with minimal risk of damage to the humeral head and its blood sup

The standardized exploration of the radial nerve during humeral shaft fixation reduces the incidence of iatrogenic palsy.

BACKGROUND: The purpose of this study is to determine if a standardized protocol for radial nerve handling during humeral shaft repair reduces the incidence of iatrogenic nerve palsy post operatively. METHODS: Seventy-three patients were identified who underwent acute or reconstructive humeral shaft repair with radial nerve exploration as part of the primary procedure for either humeral shaft fracture or nonunion. All patients exhibited intact radial nerve function pre-operatively. A retrospective chart review and analysis identified patients who developed a secondary radial nerve palsy post-

Proximal humerus fractures: anatomy, diagnosis and management.

Proximal humeral fractures are common with a bimodal distribution and sex discrepancy, affecting younger men and older women. The presentation of a proximal humeral fracture can vary greatly because of this bimodal distribution and the associated differences in mechanism of injury. Initial management should involve assessment of life- and limb-threatening injuries as outlined by the British Orthopaedic Association Standards for Trauma, with particular attention paid to axillary nerve function and vascular status. Initial imaging should involve orthogonal X-rays in three planes to determine fra…

Biomechanics of Upper Extremity Double Plating.

Double plating (DP) is an established treatment modality in traumatology. For certain indications, such as open reduction and internal fixation of bicondylar tibial plateau fractures and distal humeral fractures, DP is standard of care in the clinical routine. However, the principles of DP can be transferred to other anatomic regions and indications. In the past years, DP has been brought more and more into focus. For revision cases, such as the treatment of fracture non-unions, DP seems generally useful. In anatomical locations with high torsional stress and bending forces, but also in region…

The relationship between obesity and fractures.

There has been very limited analysis of the relationship between obesity and fractures in the orthopaedic literature. It has been established for some years that underweight individuals are at greater risk of proximal femoral fractures but recently there has been interest in the susceptibility of obese post-menopausal females to fracture. We have undertaken an analysis of 4886 adult patients who presented with a fracture and had their BMI assessed. Analysis has confirmed the relationship between underweight individuals and proximal femoral fractures but there is also a negative association bet

Coronal-plane transcondylar fracture of the humerus in a child.

A coronal plane fracture of the distal humerus in a child is an uncommon fracture, with the most familiar example being a capitellar fracture. An unusual coronal fracture of the entire distal humeral articular surface (capitellum and trochlea) occurred in a three-year-old boy. This fracture pattern has been described in adults but is believed to be previously unreported in children. This Salter-Harris Type IV fracture was initially misdiagnosed as a lateral humeral condyle fracture. Three years after treatment with open reduction and pin fixation, the distal humerus shows no evidence of growth

Scapulo-humeral Paget's sarcoma: Scottish Bone Tumour Registry experience.

This study aimed to analyse the clinical, radiological and histological features of 16 scapulo-humeral Paget's sarcoma cases accrued from Scottish Bone Tumour Registry between January 1950 and December 2000. The mean age was 61.5 (range, 44-77) years with 12 men and 4 women. There were three scapular cases and 13 humeral (1 whole length, 5 upper humeral, 5 lower humeral and 2 mid-humeral). These patients presented with progressively increasing pain in the shoulder, arm or elbow (n = 5), a painful mass associated with a pathological fracture (n = 4), a pathological fracture with progressively w

Management of acute paediatric fractures treated surgically in the UK: a cross-sectional study.

INTRODUCTION: The epidemiology of acute paediatric orthopaedic trauma managed surgically across the NHS is poorly described. Compliance against national standards for the management of supracondylar humeral fractures is also unknown at a national level. METHODS: Collaborators in 129 NHS hospitals prospectively collected data on surgically managed acute paediatric orthopaedic trauma cases. Data were collected over a seven-day period and included demographics, injury characteristics, operative details and timing of surgery. A national audit was also undertaken to evaluate compliance with the Br…

Arm wrestling fractures--a humerus twist.

Fractures of the humeral shaft in various throwing sports is a well recognized clinical entity. The specific etiology remains uncertain. Competitors engaged in Indian and/or wrist wrestling, a sport receiving increased interest, also are known to sustain humeral fractures. It is thought that these particular fractures are the result of bending moment, axial compression, and torsional forces applied to the humerus. The position of the arm during the match appears to be critical in determining the fracture pattern and location. Closed treatment was successful in all reported cases.

Diagnostic accuracy of the Mirels score at the humerus in metastatic bone disease: a narrative review.

PURPOSE: To critically evaluate the diagnostic performance of the Mirels score for pathological fracture risk prediction at the humerus, and to determine whether site-specific factors warrant a revised approach to fracture risk stratification at this location. METHODS: Narrative review. PubMed was searched without date restriction for studies addressing Mirels score validation at the humerus, alternative fracture risk assessment tools, and clinical outcomes of completed versus impending pathological humeral fracture. Additional studies were identified by manual reference screening. RESULTS:

Pediatric distal femur fixation by proximal humeral plate.

Distal femoral metaphyseal fractures are common injuries in children. Multiple treatment options have been described for this type of injury. For older children with distal metaphyseal fracture, there is still no optimal method of fixation. We propose that the commonly used proximal humeral plate can provide good method of fixation for this fracture in adolescents. Two children (12 and 14 years old) with distal metaphyseal femoral fracture were treated with proximal humeral plate. We describe the surgical technique and postoperative management. The two children healed with good alignment and f

Fracture-dislocation of the shoulder in a 32-month-old child.

A case of fracture-separation and dislocation of the proximal humeral epiphysis, a rare injury, is reported. The patient, a 32-month-old girl suspected of being an abused child, was reported to have fallen from her crib 2 days earlier. She had a painful, swollen, slightly ecchymotic left shoulder, which she held in the neutral position and refused to move. Radiographs showed a healing, nondisplaced radial fracture and fracture-separation of the proximal humeral epiphysis, which was dislocated into an anterior subglenoid location. Surgery under general anesthesia showed the humeral head to lie

Modern perspectives of open reduction and plate fixation of proximal humerus fractures.

Recent innovations have greatly increased the range of proximal humeral fractures that are amenable to open reduction and plate fixation. The optimum technique for some of the more complex fracture patterns is not yet fully refined. This article aims to describe the recent advances in the treatment of complex proximal humeral fractures by open reduction and locking plate fixation, focusing particularly on the indications for surgery, the operative techniques, and the expected outcomes after treatment.

[Classification of fractures of the proximal end of the humerus following prognostic and therapeutic aspects].

The classification of the proximal humeral fractures according to the four-segment concept of Neer has proven to be very useful. It not only considers the fracture itself but also lesions of the soft tissue. Adapting Neer's concept we propose a modified classification of the proximal humeral fractures which we believe is clearer and more practicable. This classification is illustrated by means of drawings and typical roentgenograms. The description of the different fracture-types includes the mean characteristics such as fragment-displacement, status of the rotator cuff and blood circulation t…

Distal Humeral Epiphyseal Separation in a Newborn.

Distal humeral epiphyseal separations are rare and treatment strategies are not well defined. The case of a full-term male newborn with a distal humeral epiphyseal separation as the result of a birth trauma was reviewed. A literature review of this topic was undertaken to better understand its occurrence, diagnosis, and treatment options. The patient sustained a distal humeral epiphyseal separation during a vaginal delivery. Deformity and decreased movement in the elbow were observed. Radiographs and subsequent ultrasound were used to make the diagnosis of distal humeral epiphyseal separation.

A Case Report of Humeral Nail Breakage after 11 Years Secondary to Shaft Nonunion: Treatment with Autogenous Iliac Crest Bone Graft and Compression Plate.

INTRODUCTION: The rate of nonunion of operatively treated fractures of humeral shaft is low. Increased incidence of nonunion is associated with different conditions such as open fractures, bone loss, or fracture gapping. Nonunions after prior intramedullary nailing can be difficult to address, even more with hardware failure. We present the case of a humeral nail breakage 11 years after implantation, secondary to nonunion. CASE REPORT: A 33-year-old man referred to our hospital with oligotrophic nonunion of the middle humeral diaphysis and nail breakage after 11 years from the first fracture.

Late appearance of arterial injury caused by fracture of the neck of the humerus.

A 65-year-old man sustained a fracture of the humeral neck. Four days later he suddenly developed symptoms of occlusion of the axillary artery. The fracture was reduced and fixated and the torn intima occluding this artery was fixated. Clots were removed with a Fogarty balloon catheter. Late appearance of arterial injury is rare and we found no reports of this complication after fracture of the humeral neck.

Excellent shoulder function after consolidation has been found in 90%, excellent elbow joint function in 86%. Among four cases of postoperative radial nerve palsy, three fully recovered within three months and one patient needed a secondary exploration. In three cases additional comminution at the fracture site occurred. Interlocking bolt breakage has been seen in three, slight nail migrati

A novel method of image-based navigation in fracture surgery.

The treatment of three- and four-part fractures of the humeral head is still controversially discussed. Some advocate primary arthroplasty while the results of primary fixation seem to be superior if no necrosis of the humeral head develops. Today navigation is used in orthopaedic surgery mainly for interventions on the spine, the pelvis and arthroplasty. In trauma surgery it is still rarely used and some technical problems need to be overcome. We report on a case of a three part fracture of the humeral head with mini-open reduction and fixation with image-based guided headless compression scr
