Escherichia coli extended spectrum beta-lactamase meningitis in 3 months infant with subdural abscess and communicating hydrocephalus: A case report.

INTRODUCTION AND IMPORTANCE: Escherichia coli ESBL as the etiology of meningitis is rarely found and frequently missed in infants. Escherichia coli found in the environment is an indication of fecal contamination. CASE PRESENTATION: A 3-month-old infant presented with focal seizures without fever, accompanied by positive meningeal sign and bulging large fontanelle. Laboratory examination found an increase in inflammation marker. A head CT scan showed hydrocephalus and subdural cysts. CLINICAL DISCUSSION: The patient underwent a burr hole drainage. Subdural abscesses with yellowish pus and hy

Acquired Acrodermatitis Enteropathica in an Infant.

Acrodermatitis enteropathica (AE) is an acquired or inborn (congenital) disorder of zinc metabolism that leads to zinc deficiency. The congenital form typically presents in infants during the first few months of life when they are weaned from breast milk, presenting even earlier in those who are formula fed. Acquired deficiency may be seen at any age. The characteristic clinical features of AE include erythematous, dry, scaly papules and plaques that may evolve into crusted, erosive, pustular lesions. These lesions typically are distributed in an acral and periorificial pattern and are associa

Nipple trauma in infants? Bednar aphthae.

INTRODUCTION: Bednar aphthae are infected wounds caused by trauma, localized to the hard palate in infants. They do not require specific treatment because they regress spontaneously in a few days. They often remain undiagnosed; other times, because of the pain they caused, they may worsen the nursing. CASE REPORT: We describe the clinical case of a healthy infant of 2 months, fed with formula, who has 2 aphthous lesions in the oral cavity associated with irritability and inconsolable crying during feeding. We excluded the influence of infectious factors or underlying diseases. The hypothesis

Effect of milk-containing diets on the severity and duration of childhood diarrhea.

Individual components of milk from humans and other mammalian species may influence the severity, duration, and nutritional outcome of childhood diarrhea in different ways. Nonetheless, empirical data from clinical trials can be used to assess children's responses to specific milk-containing diets. Factors modifying the response to milk include its source, amount, frequency of feeding, type of processing, and accompanying foods, as well as the type and severity of enteric infection and specific characteristics of the host. Whereas breast-fed infants have less severe diarrhea when breast milk i

Nonphysiological anaemia of prematurity.

Clinical and haematological findings at the nadir of the refractory, early anaemia of prematurity were compared in a study of 95 preterm infants. 53% of 30 babies less than 32 weeks' gestational age at birth had abnormal clinical features resulting from anaemia at its nadir, with a combination of tachycardia, tachypnoea, dyspnoea and feeding difficulties, diminished activity, and pallor. The expression 'available oxygen', derived from the Hb concentration and Hb-O2 affinity, correlated more closely with clinical features of anaemia that did the Hb concentration alone. A formula is presented th

Patterns of brain electrical activity in infants of depressed mothers who breastfeed and bottle feed: the mediating role of infant temperament.

Successful breastfeeding involves a dyadic interaction between a mother and her infant. The present study was designed to examine the association between breastfeeding and temperament in infants of depressed mothers. Seventy-eight mothers, 31 who were depressed, and their infants participated. Depressed mothers who had stable breastfeeding patterns were less likely to have infants with highly reactive temperaments. Multivariate analyses of variances (MANOVAs) showed that infants of depressed mothers who breastfed did not show the frontal asymmetry patterns, i.e., left frontal hypoactivity, pre

Urinary concentrations of non-essential and essential elements during weaning in infants from the NELA cohort in Spain.

Infancy is a period of continuous growth and development, where inadequate intake of essential elements and exposure to non-essential elements may have lifelong health consequences. Thus, the aim of this study is to evaluate the urine concentration of essential and non-essential elements as a proxy for internal exposure during the weaning period, from 3 to 18 months of age. The Nutrition in Early Life and Asthma (NELA) birth cohort generated the data for this study. Sociodemographic, dietary, and urine concentration of essential (Co, Cu, Mo, I, Fe, Mn, Se, Zn, and Ni) and non-essential element

How Does Pasteurised Donor Human Milk Support Women's Breastfeeding Journeys? A Mixed-Method Study in Collaboration With Te Whare Poipoi - Palmerston North Hospital Neonatal Unit, New Zealand.

BACKGROUND: Pasteurised donor human milk (PDHM) is increasingly available to support vulnerable infants when maternal milk is temporarily unavailable. While evidence suggests benefits for very preterm infants, the impact of PDHM on breastfeeding outcomes for late preterm and early term infants remains unclear. RESEARCH AIM: This study investigated the impact of PDHM on breastfeeding journeys among mothers and infants admitted to the neonatal unit (Te Whare Poipoi) at Palmerston North Hospital, New Zealand. METHODS: We employed a mixed-methods design combining retrospective quantitative data

Determinants of Neonatal Readmission in Healthy Term Infants: Results from a Nested Case-Control Study.

OBJECTIVE: The aim of this study was to estimate the incidence and identify the factors associated with neonatal readmission among healthy term infants. STUDY DESIGN: A nested case-control study with matching was conducted at a large level III perinatal hospital with approximately 8,700 deliveries each year. Each case infant (n = 130) was matched to two control infants (n = 260) on the case infant's date of birth (±7 days) and the mother's maternal age (<20 years, 20-29, 30-39, and >39 years). All infants were selected from a cohort of eligible term, healthy, in-state infants admitted to the

Weight loss in exclusively breastfed infants delivered by cesarean birth.

BACKGROUND: Rates of exclusive breastfeeding during the postpartum hospital stay are a key measure of quality maternity care. Often, however, concern for excessive in-hospital weight loss leads to formula supplementation of breastfed infants. The American Academy of Pediatrics defines 7% weight loss as acceptable for breastfed newborns regardless of mode of delivery. Typical weight loss in exclusively breastfed infants delivered by cesarean birth has not been studied nor have possible correlates of greater weight loss in this population. OBJECTIVES: To determine average weight loss in a cohor

Incidence and clinical outcome of cytomegalovirus transmission via breast milk in preterm infants </=31 weeks.

AIM: To evaluate incidence, timing and clinical relevance of acquired human cytomegalovirus (HCMV) infection in preterm infants. METHODS: The prospective longitudinal study included preterm infants </=31 weeks. Congenital HCMV infection was excluded by negative HCMV culture from urine or by HCMV-PCR-negative umbilical cord blood. Infants from HCMV-IgG-positive mothers received thawed frozen breast milk until 33 weeks. Urine samples were obtained weekly for HCMV culture. Data were collected regarding clinical course and milk-intake. RESULTS: Twenty-nine mothers (29/48, 60%) of 35 infants were

Apoptotic epithelial cells in biopsy specimens from infants with streaked rectal bleeding.

BACKGROUND: Histologic studies of rectosigmoidal mucosal biopsies of infants with isolated blood-streaked stool have shown many eosinophils and revealed aggregates of small dark granules (nuclear dust). However, no description of the nuclear dust has been made for this condition and the nature of the nuclear dust has not been thoroughly investigated. We determined the characteristics of these particles in biopsies from infants with streaked rectal bleeding. METHODS: Nineteen infants who were younger than 6 months old and had isolated rectal bleeding were studied, as were six age-matched contr

Enteral nutrition in infants with congenital heart disease and growth failure.

To determine an effective nutritional regimen for management of growth failure in infants with congenital heart disease and congestive heart failure, the authors studied 19 infants with cardiac anomalies who were not candidates for early corrective surgery. Patients were randomly assigned to one of three feeding groups: group 1 (n = 7) received continuous, 24-hour nasogastric alimentation; group 2 (n = 5) received overnight, 12-hour nasogastric infusions plus daytime oral feedings as tolerated; and group 3 (n = 7) received oral feedings alone. For all patients, commercial infant formula (cow's…

Short- and longer-term growth and development of fat mass in preterm infants.

Preterm infants typically experience faster growth rates than term-born infants, often doubling their weight in six to eight weeks. However, many face challenges leading to growth faltering and suboptimal neurodevelopment. To achieve optimal growth, these infants often require fortified breastmilk or high-nutrient formula. While meeting nutrition and growth targets are essential, concerns arise about rapid postnatal growth during their catch-up phase, particularly regarding increased body fat at term-corrected age, possibly increasing their risk for obesity and chronic health conditions later.

[Enteral feeding volume advancement by using a standardized nutritional regimen in preterm infants ≤1,750 g birth weight: a controlled randomized trial].

BACKGROUND: Rapid enteral feeding volume advancement in preterm infants can reduce the use of intravenous fluids. This practice may decrease the hazards of intravenous infusion solutions and potentially the morbidity rate. Several cohort trials demand the standardised nutritional regimen to reduce the complications and the time to reach full enteral feeds. AIM: to determine whether using a standardized nutritional regimen the rapid enteral feeding advancement in preterm infants is practicable without increasing the incidence of feeding complications. PATIENTS AND METHODS: A prospective, rand

Polypropylene micro- and nanoplastics affect the digestion of cow's milk proteins in infant model of gastric digestion.

Infants may ingest large quantities of micro- and nanoplastics (MNP) due to the heating and sterilization of the feeding bottles, which promotes the generation and migration of MNP into the milk or milk formula. The effect of MNP on the digestion of proteins, which are crucial for the growth and development of infants remain unknown. The current study investigated the in vitro digestion of cow's milk proteins (1 mg/mL) with or without polypropylene (PP) microplastics (MP, pristine or oxidized) (20 mg/mL and 63-180 μm) in simulated gastric fluids (SGF) using an infant (pH 5.0; pepsin activity;

Severe Cow's Milk Allergy Presenting With Life-Threatening Metabolic Acidosis in an Extremely Preterm Infant: A Case Report.

Cow's milk allergy (CMA) is a common food allergy in infancy, but its diagnosis in preterm infants remains challenging due to nonspecific symptoms that often mimic complications of prematurity. Early recognition is crucial to prevent significant morbidity in this vulnerable population. We present a case of a male infant born at 27 weeks and four days of gestation with a birth weight of 960 grams. His clinical course was complicated by persistent feeding intolerance, respiratory symptoms, severe metabolic acidosis, and profound growth failure despite multiple interventions for suspected sepsis
