[Larsen syndrome--symptomatics and therapy (author's transl)].

We report of three children suffering from Larsen-syndrome with the typical deformity of the face and multiple, mostly bilateral joint-luxations with severe affection of the hip-joints and knee-joints. Two patients had short terminal phalanxes with the typically spatulate terminal phalanxes of the thumb, radiographically visible. One child showed bilaterally dysplastic metacarpophalangeal bones with luxated metaphalanxes. The diagnosis could furtheron be verified by the characteristically twin-formed bone-nucleus of the calcaneus within all patients. Two children early showed a severe, progred

[Comparative study of transosseous suture and suture anchor technique in medial patellofemoral ligament double bundle reconstruction].

OBJECTIVE: To investigate the effectiveness of transosseous suture in medial patellofemoral ligament (MPFL) double bundle reconstruction. METHODS: The clinical data of 75 patients with recurrent patella dislocation who met the selection criteria between January 2014 and December 2017 were retrospectively analyzed. All of them were treated with MPFL double bundle reconstruction, and divided into study group (39 cases, using new transosseous suture technique) and control group (36 cases, using traditional suture anchor fixation) depending on the intraoperative fixation technique. There was no s

No difference in accuracy between pinless and conventional computer-assisted surgery in total knee arthroplasty.

PURPOSE: Many studies have demonstrated higher precision and better radiological results in Total knee arthroplasty (TKA) with computer-assisted surgery (CAS). On the other hand, studies revealed a lengthening of operation time up to 20 min for this technique and demonstrated rare additional complications as fractures and neurovascular injuries caused by the array pins and any intraoperative array dislocation leads to abortion of CAS. To combine the advantages and eliminate the disadvantages of standard CAS, we evaluated the accuracy of a so-called pinless CT-free version of knee navigation (p

The self-aligning knee prosthesis: clinical and radiological outcome and survival analysis of a cruciate retaining meniscal bearing knee at 10-year follow-up.

PURPOSE: Previously, the clinical and radiological results of the self-aligning cruciate retaining total knee prosthesis at 5-years follow-up were described. The 10-year follow-up is now reported to determine the clinical and radiological results and the survival of the prosthesis. METHODS: A 10-year follow-up was performed in this prospectively followed cohort of 246 (232 patients) self-aligning, cruciate retaining total knee arthroplasties performed between 1995 and 1998. Because patients had died and others suffered from comorbidities making them unable to attend a follow-up consultation,

Comparison of 2 different techniques for anatomic reconstruction of the medial patellofemoral ligament: a prospective randomized study.

BACKGROUND: The comparative clinical outcome of anatomic reconstruction of the medial patellofemoral ligament (MPFL) between the Y-graft technique and C-graft technique for chronic patellar instability is unknown. HYPOTHESIS: Anatomic reconstruction of the MPFL with a Y-graft technique (a Y-shaped graft: femoral fixation first with separate tensioning of the 2 bundles) would yield better clinical results than a C-graft procedure (a C-shaped graft: patellar fixation first with simultaneous tensioning of both bundles) for chronic patellar instability. STUDY DESIGN: Randomized controlled trial;

Editorial Commentary: The Best Index to Determine Whether to Medialize the Tibial Tubercle In Patients With Patellar Instability May Be Tibial Tubercle to Trochlear Groove Distance/Trochlear Width, But Check the Intraoperative Patellar Tracking as Well.

An excessively lateral tibial tubercle is a well-accepted risk factor for recurrent patellar dislocation. Generally, it is measured on magnetic resonance imaging as the TT-TG distance (tibial tubercle-trochlear groove distance), and recent meta-analyses have suggested that a value as low as 12.5 mm can be used as the threshold for deciding when to medialize the tibial tubercle. However, a problem with using the TT-TG distance is that it is influenced by the size of the knee. An individualized index helps overcomes this problem, and dividing the TT-TG distance by the trochlear width may be the

Tibial Tubercle Osteotomy With Anteriorization and Distalization for Treatment of Patellar Instability With Patella Alta.

Patellofemoral instability is a common cause of knee pain that can lead to long-standing pain, chondral injury, recurrent dislocations, and degenerative changes if not treated appropriately. Tibial tubercle osteotomy is indicated when there is anatomy predisposing to patellar maltracking and instability, namely abnormal patellar height or tibial tubercle location. In this Technical Note, we describe a technique for tibial tubercle anteriorization and distalization as part of the overall treatment algorithm for patellar instability with associated patella alta. This method of tibial tubercle os

Patellar Tendon-Trochlear Groove Angle Measured on a Single Computed Tomography Slice of the Distal Femoral Trochlear Groove Is a Reliable Measurement for the Evaluation of Patellar Instability.

PURPOSE: To verify the reliability of patellar tendon-trochlear groove angle (PTTG-A) measured by computed tomography (CT) and the clinical significance in evaluation of patellar instability. METHODS: A retrospective study of hospitalized patients with knee pain or injury and had knee CT from January 2017 to June 2021 was performed. PTTG-A and tibial tuberosity-trochlear groove (TT-TG) distance were measured on CT. Spearman correlation analysis was analyzed between the 2 measurements. The intraclass correlation coefficient was determined for inter- and intraobserver reproducibility. The capac

Inability to obtain formal informed consent in the face of a standard surgical indication.

A thirty-eight-year-old intoxicated man was admitted to the surgical trauma service following a single motor-vehicle accident. He had a severe closed head injury, bilateral pulmonary contusions, a fracture-dislocation of the right acetabulum, and an open injury of the right knee joint. The acetabular fracture pattern was an associated both-column fracture with the femoral head dislocated into a widely displaced posterior-column fracture line. The treating physicians agreed that it would be in the patient's best interest to take him to the operating room for emergent debridement and irrigation

[Treatment of Permanent Patellar Dislocation in Flexion by Patellar Osteotomy Combined with Modified Trochleoplasty and Tibial Tubercle Medialisation].

Patellar dislocations in flexion, which occurs permanently with every movement, is the most serious manifestation of patellofemoral malalignment. Surgical approaches to correct this problem have been mostly unsuccessful. In a new therapeutic approach, the concave posterior surface of the patella, which slides on the hypoplastic lateral condyle as if guided by splints, is seen as the main pathology. The appropriate surgical strategy is trochleoplasty, combined with closed wedge patellar osteotomy, tuberosity medialisation, procedure for lengthening lateral retinaculum and MPFL plasty with the q

Bilateral Staged Total Hip Replacement and the Natural Progress of an Untreated Case of Developmental Dysplasia (Dislocation) of the Hip: A Clinical Case Report by the Surgeon and the Patient.

The natural history of an untreated case of a Developmental Dysplasia (Dislocation) of the Hip (DDH) associated with multiple congenital abnormalities is reported in a 55-years-old man. The patient's complaints and the varieties of the typical manifestations emerged in other parts of the body throughout the life are reviewed and discussed as comorbidities of a dysplastic condition. Two-stage bilateral total hip replacement (THR) operations were performed at the age of 55. In addition, to relieve the pain, the walking disabilities were overcome, hence gaining normal walking in swing and stances

The contemporary management of anterior knee pain and patellofemoral instability.

In this review the evidence for the management of patients with patellofemoral disorders is presented confined to anterior knee pain and patellar dislocation (excluding patellofemoral arthritis). Patients present along a spectrum of these two problems and are best managed with both problems considered. The key to managing these patients is by improving muscle function, the patient losing weight (if overweight), and judicious use of analgesics if pain is an important feature. Hypermobility syndrome should always be looked for since this is a prognostic indicator for a poor operative outcome. Op

Zone of injury of the medial patellofemoral ligament after acute patellar dislocation in children and adolescents.

BACKGROUND: Patellar dislocation is a common traumatic injury in the pediatric and adolescent population. The primary constraint to lateral subluxation and dislocation of the patella is the medial patellofemoral ligament (MPFL), which serves to resist lateral translation of the patella. Injury to the MPFL may predispose to recurrent dislocation but the anatomic site of injury is poorly characterized in children and adolescents. PURPOSE: The authors addressed 2 questions: (1) What is the zone of injury to the MPFL in a pediatric/adolescent population after primary patellar dislocation? (2) Wha

Occult Incarcerated Osteochondral Fracture of the Medial Patellar Facet Following Acute Lateral Patellar Dislocation in an Adolescent: A Case Report.

Acute lateral patellar dislocation is a common injury in adolescents and is most frequently associated with rupture of the medial patellofemoral ligament (MPFL). Concomitant osteochondral fractures may remain occult or be misinterpreted on initial imaging, resulting in delayed diagnosis and potentially compromising long-term joint function. We present a rare case of an incarcerated osteochondral fracture of the medial patellar facet that was initially misinterpreted as arising from the medial femoral condyle following acute lateral patellar dislocation. A previously healthy 15-year-old boy pre

Medial patellar ligament reconstruction in combination with derotational distal femoral osteotomy for treating recurrent patellar dislocation in the presence of increased femoral anteversion: a systematic review.

BACKGROUND: Medial patellar ligament reconstruction (MPFL-R) in combination with derotational distal femoral osteotomy (DDFO) for treating recurrent patellar dislocation (RPD) in the presence of increased femoral anteversion is one of the most commonly used surgical techniques in the current clinical practice. However, there are limited studies on the clinical outcomes of MPFL-R in combination with DDFO to treat RPD in the presence of increased femoral anteversion. PURPOSE: To study the role of MPFL-R in combination with DDFO in the treatment of RPD in the presence of increased femoral anteve

Medial Patellofemoral Ligament Reconstruction Using Allografts in Skeletally Immature Patients.

BACKGROUND: Patellar instability has the highest incidence in adolescents aged between 14 and 18 years. The unique relationship between the medial patellofemoral ligament (MPFL) and the distal femoral physis in skeletally immature patients warrants precisely positioned MPFL graft insertion. A paucity of data are available evaluating the results of MPFL reconstruction using allograft tendon before skeletal maturity. PURPOSES: (1) To assess the results of MPFL reconstruction using allograft tendon in skeletally immature patients by analyzing redislocation and reoperation rates, radiological out
