Validation of a Georgian language headache questionnaire in a population-based sample.

In a pilot phase of a survey of the prevalence of primary headache disorders in the Republic of Georgia, we validated a Georgian language questionnaire for migraine (MIG), tension-type headache (TTH), MIG+TTH and trigeminal autonomic cephalalgias (TAC). A population-based sample of 186 people with headache completed the questionnaire and were blindly examined by one of two headache experts. The questionnaire diagnoses were: MIG 49, TTH 76, MIG+TTH 45 and TAC 16. The physicians' diagnoses were: MIG 59, TTH 77, MIG+TTH 34, TAC 2 and "symptomatic headache" in 14 subjects. Sensitivity and specific…

Speech and language skills of parents of children with speech sound disorders.

PURPOSE: This study compared parents with histories of speech sound disorders (SSD) to parents without known histories on measures of speech sound production, phonological processing, language, reading, and spelling. Familial aggregation for speech and language disorders was also examined. METHOD: The participants were 147 parents of children with SSD (58 fathers and 89 mothers) who were directly tested and interviewed for family history of disorders. RESULTS: Thirty-six parents (18 mothers and 18 fathers) reported enrollment in speech therapy as children for SSD. Parents with a history of s…

Apraxia of speech: how reliable are speech and language therapists' diagnoses?

OBJECTIVE: To discover how reliably speech and language therapists could diagnose apraxia of speech using their clinical judgement, by measuring whether they were consistent (intra-rater reliability), and whether their diagnoses agreed (inter-rater reliability). DESIGN: Video clips of people with communication difficulties following stroke were rated by four speech and language therapists who were given no definition of apraxia of speech, no training, and no opportunity for conferring. SETTINGS: Videos were made of people following stroke in their homes. Ratings of the videos were carried ou…

[Hospitalization and premature surgeries; language and psychism: a report of 2 cases].

BACKGROUND: mutual influences between body, psychism and language. AIM: The purpose of this work was to analyze the possible connections between hospital internations and surgeries, to which children were submitted at their earliest life years, as well as subsequent language and psychic adversities. METHOD: It is a qualitative/clinical research, of a descriptive/interpretative nature, carried out through the longitudinal analysis of two clinical case studies with small children of approximately three years of age, whose families had co-occurrent complaints: surgeries and premature hospital i…

Parent concerns and professional responses: the case of specific language impairment.

BACKGROUND: GPs and health visitors are usually the first to be approached by parents concerned about their child's speech and language development in the early years. The role health professionals play in early detection of speech and language difficulties is therefore crucial to ensure timely referral for speech and language therapy. AIM: To examine parental accounts of health visitor and GP involvement in the assessment and diagnosis of their children's speech and language impairment. DESIGN OF STUDY: Qualitative retrospective interviews. SETTING: Two Local Education Authorities in Merse…

A longitudinal study of language development in two children with Williams syndrome.

Williams syndrome (WS) is often cited as the prime example within developmental disorders of the dissociation of language from other cognitive skills, particularly from visuo-motor skills. This claim has been responsible for the challenges posed by this population to cognitive theories and to models of language acquisition. Two Hebrew-speaking children with WS were followed longitudinally for 18 months, from the first occurrences of two word combinations. Y was 3.9-5.3 and BT was 4.2-5.8 during data collection. Both children had an MLU of 1.8-2.8. The children's linguistic profiles were compar…

The Wada Test: contributions to standardization of the stimulus for language and memory assessment.

The Wada Test (WT) is part of the presurgical evaluation for refractory epilepsy. The WT is not standardized and the protocols differ in important ways, including stimulus type of material presented for memory testing, timing of presentations and methods of assessment. The aim of this study was to contribute to establish parameters for a WT to Brazilian population investigating the performance of 100 normal subjects, without medication. Two parallel models were used based on Montreal Procedure adapted from Gail Risse's (MEG-MN,EUA) protocol. The proportions of correct responses of normal subje

Children with specific language impairment: parental accounts of the early years.

Parent views of specific language impairment have received little attention in the literature, even though speech and language development is the single greatest area of concern for parents. This article examines parental views of events up to, and including, language unit admission for their children. A series of qualitative interviews was conducted with parents. Parents felt that, apart from speech and language difficulties, the children were developing normally. They placed a low value on speech and language therapy, and saw language unit admission as the only intervention that 'worked'. In…

Language and phonological skills in children at high risk of reading difficulties.

BACKGROUND: Dyslexia is now generally acknowledged to involve difficulties in phonological processing. However, the links between reading difficulties and speech difficulties remain unclear. METHOD: In the present study, 17 children with speech difficulties between the ages of four and six were compared to children with a family history of dyslexia and normally developing controls on phonological processing, phonological learning, phonological awareness and literacy tasks. RESULTS: The two groups of children at risk of reading difficulties showed very similar patterns of impairment, with ave

Functional MR imaging in assessment of language dominance in epileptic patients.

The value of functional MR Imaging (fMRI) in assessing language lateralization in epileptic patients candidate for surgical treatment is increasingly recognized. However few data are available for left-handed patients. Moreover determining factors for atypical dominance in patients investigated with contemporary imaging have not been reported. We studied 20 patients (14 males, 6 females; 9 right handed, 11 left handed) aged from 9 to 48 years, investigated for intractable partial epilepsy. Epileptic focus location was temporal in 14 cases, extratemporal in 6, and lateralized in the left hemisp…

Attitudes of speech and language therapists towards stammering: 1985 and 2000.

BACKGROUND: Past research has indicated that speech and language therapists hold some negative attitudes towards people who stammer, their parents and the treatment of stammering. However, studies on attitudes towards stammering have predominantly focussed on therapists in the USA. Recent trends towards earlier intervention suggest that more therapists in the UK will be involved in working with stammering. AIMS: This study aimed to gather current attitudes of therapists in the UK and to compare them with attitudes measured 15 years previously. It also considered the impact of postgraduate tra…

The impact of developmental visuospatial learning difficulties on British Sign Language.

There has been substantial research interest in recent years in the relationship between the development of language and cognition, especially where dissociations can be seen between them. Williams syndrome, a rare congenital disorder characterized by a fractionation of higher cortical functions, with relatively preserved language but marked difficulties with visuospatial constructive cognition, has been extensively studied. The case of Heather, who is remarkably similar to the characteristic phenotype of Williams syndrome in physical appearance and cognitive abilities, but who is also congeni…

The role of language areas in motor control dysfunction in Parkinson's disease.

We evaluated the differences in motor control organization between parkinsonian patients with (seven cases) and without (ten cases) gait disorder. We used positron emission tomography (O15-H2O-PET) to measure regional cerebral blood flow as a correlate for local neuronal activation. This has been assessed during repetitive joystick movements of the right hand, externally triggered using a metronome as an auditory cue. In patients with Parkinson's disease (PD) without gait disorder, the contralateral supplementary motor cortex and the bilateral cerebellum were activated, while in PD patients wi

Language hemispheric dominance in patients with congenital lesions of eloquent brain.

OBJECTIVE: In 90% of normal subjects, the left hemisphere is dominant for language function. We investigated whether congenital lesions of the left perisylvian regions altered cortical language representation in right-handed individuals. METHODS: Using functional magnetic resonance imaging, we studied language hemispheric dominance in five right-handed adult patients with congenitally acquired arteriovenous malformations (AVMs) originating from left hemispheric cortical language regions. The AVMs had not caused neurological symptoms during early development, but patients presented as adults w…

Issues to consider in the evaluation of speech and language therapy for preschool children.

This paper reviews some of the methodological issues involved in the design of a randomized controlled trial currently underway in Bristol. The trial compares the progress of preschool children randomly allocated to a 'watching-waiting' control group with the experimental group who have immediate access to therapy. This paper reviews a number of relevant studies, which have either followed up preschool children with early language delays or have investigated the effects of therapy with this age group. The basic design of the trial is outlined along with a discussion of the sample, the measurem…

Screening for primary speech and language delay: a systematic review of the literature.

Screening young children for developmental conditions such as speech and language delay is considered to be a part of the Child Health Surveillance programme in the UK. It is currently practised in many different ways throughout the country and like screening for other conditions conventionally identified in infancy, has been the subject of some concern for those responsible for providing such services. This systematic review (Law et al. 1998) was hypothesis driven and aimed to: i) establish whether, given the available evidence, there was sufficient evidence to warrant the introduction of uni…

Comprehension of language in congenitally deaf children with and without cochlear implants.

The consequences of profound early deafness on oral language in children are drastic. The modern cochlear implant (CI) has been shown to enhance speech production skills in prelingually deaf children. Many factors may contribute to a poor or an excellent outcome, making it difficult to compare groups of children wearing or not wearing cochlear implants. The present study compared receptive language levels in matched pairs of children from CI group and non-CI groups. The pre-op receptive language development curve suggest a possible growth over time with the maturation and the speech therapy. C…

Efficacy of speech and language therapy for particular speech sounds in children.

At the present time there is a demand for research into the efficacy of speech and language therapy. This study aims to assess the efficacy of therapy provided in South Tees for children who do not use word-initial velar plosives. The study is in progress at the time of writing and it is expected that therapy will be completed by July 1998. Subjects are split randomly into an 'immediate treatment' and a 'deferred treatment' group. All subjects are reassessed after treatment of each group. It is of course possible that analysis of the results will not show a significant difference between pre- …

Voice, speech and language characteristics of children with Prader-Willi syndrome.

Eleven individuals with Prader-Willi syndrome (PWS), aged between 4 and 25 years, were compared with II non-PWS children of the same sex, age, body mass index and IQ level. Voice, speech and language skills were generally impaired in subjects with PWS. Oral motor function, pitch level and resonance were specifically disordered and clearly differentiated the two groups from each other. Certain biological perinatal factors separated subjects with PWS from other obese children and adolescents, but did not differentiate within the group with PWS and could not account for the speech/language proble…
