[A case of congenital muscular dystrophy with changes in the white matter of the brain].

A case of cerebro-muscular disease is reported in a 5-year-old girl with normal mental development. The changes resembled those in Fukuyama dystrophy, but the clinical picture differs from that dystrophy in normal mental development and absence of other pathological changes in the central nervous system.

An autopsied case of acute hemorrhagic leukoencephalitis.

This is a clinico-pathological study of one case of acute hemorrhagic leukoencephalitis occurring in a patient with lung cancer. The main changes observed by light microscopy were in the white matter. All of these changes were the result of hemorrhage and demyelination around the venous vessels. Glial mesenchymal reactions were minimal. In view of these findings, this case supports the hypothesis that immune mechanisms play a role in the pathogenesis of neuropsychiatric symptoms during malignant illnesses. Here we will discuss the problem of differential diagnoses of acute hemorrhagic leukoenc

Detection of polyoma virus DNA in PML-brain tissue by (in situ) hybridization.

The human papova (JC) virus was extracted from brain of a patient with progressive multifocal leukoencephalopathy. A single band of virus was obtained at a density of 1.345 g/ml CsCl.JC virus DNA was purified and a highly specific cRNA was generated in vitro. In situ hybridization with JC virus cRNA and autoradiography on sections of the same brain revealed silver grains over oligodendrocytes, astrocytes and possible vascular endothelial cells, indicating the presence of JC virus DNA in these different cell classes.

Herpes simplex encephalitis and sarcoidosis.

Fatal herpes simplex encephalitis occurring in a patient with sarcoidosis is described. Ultrastructural study of brain tissue revealed the presence of herpesvirus particles. The association of these 2 diseases has not been recorded before. The encephalitis in this case probably represents an opportunistic viral infection in the face of depressed cellular immunity. Progressive multifocal leukoencephalopathy, a disease of putative viral aetiology, is known to occur in patients with sarcoidosis and it is suggested that this case represents a similar pathogenetic association.

Multifocal varicella-zoster virus leukoencephalitis temporally remote from herpes zoster.

Two patients with cancer, one with Hodgkin's disease and the other with a granulosa cell tumor of the ovary, developed a progressive, eventually fatal infection of the central nervous system exhibiting multifocal symptoms and signs. Pathologically, gross abnormalities of the brain resembled those in progressive multifocal leukoencephalopathy (PML), with discrete and confluent plaque-like lesions concentrated in the white matter, particularly along the gray-white junction. Microscopically, pathological changes differed distinctly from those associated with PML; in addition to confluent foci of

Application of molecular tools for the diagnosis of central nervous system infections.

Many infectious agents can cause central nervous system (CNS) diseases in humans. Since microbial agents infecting CNS are numerous and have different features, conventional laboratory tests may not be sensitive enough to identify and characterise viruses and bacteria in human biological specimens. Thus, the need to define methods for the diagnosis of infectious neurological diseases, such as progressive multifocal leukoencephalopathy (PML), is urgent, in order to improve the outcome of the diseases with rapid and accurate detection of the pathogens.

[Natalizumab in multiple sclerosis: unclear patient benefits].

Two large randomised clinical trials were published in March 2006 that evaluated the effect of natalizumab on the incidence of disease exacerbation in patients with multiple sclerosis (MS). Both trials showed that natalizumab reduces the incidence of MS exacerbations, but it is unclear whether patients ultimately benefited from treatment. It is clear that natalizumab is associated with serious adverse events, such as progressive multifocal leukoencephalopathy. Therefore, the use of natalizumab in patients with MS cannot be considered advisable at this time.

Myelodysplasia associated with acquired immunodeficiency syndrome.

Two cases of acquired immunodeficiency syndrome with myelodysplasia are presented. Case 1 was admitted because of Pneumocystis carinii pneumonia. Mild anemia, thrombocytopenia and hypersegmented neutrophils were observed. After the administration of trimethoprim-sulfame-thoxazole and antiretroviral therapy, pancytopenia progressed. Bone marrow (BM) showed dysplastic hematopoiesis, suggesting human immunodeficiency virus-myelopathy. Case 2 was hospitalized due to progressive multifocal leukoencephalopathy. BM specimen obtained for thrombocytopenia showed myelodysplasia similar to myelodysplasti

AIDS- and non-AIDS-related PML association with distinct p53 polymorphism.

A population-based analysis of progressive multifocal leukoencephalopathy (PML) showed PML frequencies of 5.1% among patients with AIDS and 0.07% among patients with hematologic malignancies, but similar clinical features of PML in both groups. Sequencing of the p53 gene, exon 4, showed heterozygosity (Arg-Pro) at codon 72 in five of six PML patients. These findings indicate that frequencies of non-AIDS- and AIDS-related PML differ markedly but p53 polymorphisms may influence the occurrence of PML in both groups.

[Slow virus infections of the central nervous system].

The major slow viral infections discovered in man manifest themselves in chronic neurological disease. Studies of these diseases have allowed a better understanding of the mechanisms of virus persistence in the central nervous system. Findings are summarized from recent investigations on such slow viral infections as subacute sclerosing panencephalitis, progressive multifocal leukoencephalopathy and the spongiform encephalopathies. From the examples presented it is evident that there is a great diversity of virus-host interactions in slow viral infections of the central nervous system.

[Movement disorders and AIDS].

We describe the clinical characteristics, causes and response to treatment in 6 patients with AIDS who presented with abnormal movement disorders between January 1987 and July 1993 in our hospital, 3 with hemiballismus-hemichorea, 1 with athetosis, 1 myoclonia and 1 with "rubric" tremor. Brain imaging showed lesions in the corpus striatum in all the patients. Suspected diagnoses were cerebral toxoplasmosis in 4, cerebral lymphoma in 1 and progressive multifocal leukoencephalopathy in 1. The toxoplasmosis patients showed improvement (2 cases) or disappearance (2 cases) of movements with antipar

Cytomegalovirus meningoencephalitis after rituximab treatment for primary central nervous system lymphoma.

The efficacy and safety of rituximab against B-cell lymphomas is well established. However, there has been an increased incidence of infectious complications after rituximab treatment, mostly hepatitis B reactivation and progressive multifocal leukoencephalopathy. This is the case of a 67-year-old patient with primary central nervous system lymphoma, who developed cytomegalovirus meningoencephalitis after receiving high-dose chemotherapy and rituximab. As there was no evidence of lymphoma relapse or additional immunosuppression, besides his previous treatment, an association between rituximab

Case report. Simultaneous fungal and viral infection of the central nervous system.

A patient with lymphoma developed central nervous system dysfunction and the diagnosis of cryptococcal meningitis was made. Because of discordant clinical and laboratory responses to appropriate treatment, further investigations were performed and the additional diagnosis of progressive multifocal leukoencephalopathy (PML) was established. Computerized axial tomography (CT scanning) proved helpful in pointing toward the latter disorder. Clinical, neuropathological, and microbiologic features are presented. The diagnostic approach to central nervous system illness in patients with impaired immu

Treating multiple sclerosis with natalizumab.

Natalizumab is the first monoclonal antibody approved for the treatment of relapsing multiple sclerosis. Pivotal trials demonstrated the efficacy of natalizumab on clinical and paraclinical measures of disease activity and disability progression. Although a direct comparison has not been performed yet, natalizumab seems to be more efficacious than the currently available immunomodulant drugs, such as IFN-β and glatiramer acetate. Despite its efficacy, the occurrence of an increased risk of progressive multifocal leukoencephalopathy with the treatment, raises concerns about its widespread use i
