An analysis of 5 methods of treatment for Legg-Calvé-Perthes disease was done on 124 patients with 141 affected hips. Before treatment, all groups were statistically similar concerning initial Mose measurement, age at onset of the disease, gender, and Catterall class. Treatments included the Scottish Rite orthosis (41 hips), nonweight bearing and exercises (41 hips), Petrie cast (29 hips), femoral varus osteotomy (15 hips), or Salter osteotomy (15 hips). Hips treated by the Scottish Rite orthosis had a significantly worse Mose measurement across time interaction (repeated measures analysis of

When amputation becomes necessary in a patient with peripheral vascular disease, it is important to preserve as much tissue as possible to preserve maximum function. This is especially important because the other extremity may have similar involvement in the future. With appropriate care, an amputation at the Chopart (calcaneocuboid-talonavicular) level can give a good functional result.

The current study examined the medical and economic impact of various treatment modalities of unstable cervical spine dislocations. Thirty-five patients with dislocations of the cervical spine were reviewed retrospectively from two large university hospitals. There were 23 unilateral and 12 bilateral facet dislocations. Fourteen patients were treated nonoperatively and 21 patients were treated with early surgery. Nine of the 14 patients treated nonoperatively redisplaced their dislocations while in an orthosis and required late surgery. Analysis of variance shows that it is the amount of injur

A survey was made of the parents of 380 children whose mobility impairments require the use of a wheelchair, walker, or braces. They were asked about equipment, health services, related services, and family support services used during the previous year. There was extensive use of equipment and traditional medical and health services, such as visits to primary care and specialist physicians; there was moderate use of related health services, such as physical or occupational therapy and child counseling; and there was very little use of community-based family support services, such as respite c

The management of upper limb problems in arthrogryposis multiplex congenita (AMC) is described on the basis of the author's own experience gained from the care of a large number of affected children. Emphasis is placed on evaluation of the disability by repeated testing and observation. In many cases, the deformities will be accepted and improvements in function will be gained by orthoses or attention to details of seating, dressing, and toileting. Surgery is indicated in some cases, especially at the elbow, to obtain mobility, flexor power, or both. Arthrodesing the wrist is also useful in so

Metatarsalgia is a symptom, not a diagnosis. A careful study of ninety-eight patients who had complaints of pain in the fore part of the foot revealed twenty-three distinct diagnoses. These diagnoses were grouped as primary metatarsalgia, secondary metatarsalgia, and pain under the fore part of the foot. This division assisted in determining the form of nonoperative or operative treatment. The use of a pressure-sensitive cholesterol-crystal force-plate analysis or intra-articular or digital nerve blocks was helpful both in accurately pinpointing abnormal pressures and in determining whether op

Some 550 disabled patients were examined by a rehabilitation team by organizing a rural camp at Meerut. This rehabilitation team consisted of orthopaedic surgeons, doctors, a prosthetist, an orthotist, a medical social worker and other skilled workers. Out of the 550 disabled patients, 271 were amputees, and 184 cases had poliomyelitis. Males predominated by 4:1. Disability was most common up to the age of 30 years. Of the 271 amputees, 146 were of upper limb while 125 were of lower limb. Only 86 lower limb amputees were suitable for fitting a prosthesis, while 39 others needed surgery of phys

BACKGROUND: Limb trauma often results in long-term disability. Carbon fiber custom dynamic orthoses (CDOs) include a proximal cuff, posterior strut, and footplate, and improve outcomes following limb trauma, but the effects of cuff design are not well understood.
RESEARCH QUESTION: Does CDO cuff design influence limb mechanics, foot loading, or CDO comfort?
METHODS: Fifteen unimpaired individuals completed testing without an orthosis (NoCDO) and in a CDO with four interchangeable cuffs: anterior shell with straps, patellar tendon bearing shell with ratcheting system, patellar tendon bearing

BACKGROUND: Though the Pavlik harness is often used to reduce dislocated hips in infants with developmental dysplasia of the hip (DDH), residual acetabular dysplasia may occur following reduction. It is unclear whether patient or treatment-related factors, including Pavlik wear duration, are associated with residual dysplasia. As a result, there is considerable variability in treatment practices using the harness. The purpose of this study was to determine whether the duration of Pavlik wear predicts radiographic success at 2-year follow-up for newborns with successfully reduced hips.

BACKGROUND: War-related traumas can lead to orthopedic and neurological disorders in victims. However, the scope of such disorders may expand months or even years after the trauma. Orthotic treatment as a rehabilitation process aims to enable people with disabilities to reach and maintain their optimal physical, sensory, intellectual, psychological, and social functional levels. This study aimed to investigate the rate of using orthoses among Iranian veterans with neuromuscular and skeletal disorders of the ankle and foot. Furthermore, the priorities of orthotic treatment in those veterans wer

INTRODUCTION: Carpal tunnel syndrome (CTS) has been described as the most common compression neuropathy. Many modalities exist for conservative treatment. Efficacy of each modality has been described in the literature. However, the effectiveness of combination of these modalities is not well established. The purpose of this study is to assess the short-term clinical outcome of conservative treatment for CTS comparing orthosis alone with combination of orthosis, nerve/tendon gliding exercises, and ultrasound therapy.
METHODS: Forty-one patients who

BACKGROUND AND PURPOSE: Mobility limitations are prevalent among older adults and are related to falls, morbidity, and mortality. BalanceWear Therapy (BWT) improves measures of mobility among people with multiple sclerosis but has not been studied in older adults. Therefore, the purpose of this investigation was to examine the effects of BWT on measures of mobility among older adults with limited mobility.
METHODS: This study was a double-blind, randomized controlled trial of older adults recruited from senior living facilities. Adults aged 86.0 (6.1) years were randomized into a BWT, weighte

Current practice and guidelines recommend the use of neck orthoses for people with amyotrophic lateral sclerosis (ALS) to compensate for neck weakness and to provide surrogate neck control. However, available options are frequently described by patients as restrictive and unsuitable and there was a need for a new device that addressed the needs of people with ALS. This project utilized a co-design process to develop a new neck orthosis that was more flexible yet supportive. Following development of a prototype device, a mixed methods cohort study was undertaken with patients and carers, in ord
