The morphologic diagnosis of histiocytic lesions of the gastrointestinal tract can be challenging, and several disorders have to be considered in their differential diagnosis. We present one of the most widespread examples of xanthomatosis of the gastrointestinal tract published so far and give a short review on histiocytic disorders of the gastrointestinal tract in general. Two months after being started on Depakote (divalproex sodium: Abbott Laboratories Inc., Abbott Park, Illinois), a 57-year-old female noticed the development of a transverse yellow band on all 20 proximal nails that eventually led to complete nail plate discoloration. A two-year-old male child presented with bizarre hypopigmented skin lesions, severe mental retardation and generalized tonic-clonic seizures. Examination showed hypopigmented patterned whorls and irregular patches over the trunk and linear streaks over the flexor aspects of upper and lower limbs. A 24 year old woman presented with generalised livedo reticularis and progressive intellectual decline. A CT scan of her head showed multifocal low density areas, consistent with cerebral infarction. BACKGROUND: Facial melanoses (FM) represent a heterogeneous group of pigmentary disorders characterized by altered facial pigmentation, including melasma, lichen planus pigmentosus (LPP), Riehl's melanosis, erythema dyschromicum perstans, and others. These conditions constitute a major proportion of dermatology outpatient visits and, despite being largely asymptomatic, exert a substantial negative impact on quality of life (QoL) due to cosmetic disfigurement. The etiology of many forms of FM remains unclear, and studies correlating clinical, dermatoscopic, and QoL parameters are limited.
