Pontine warning syndrome: case series and review of literature.

Crescendo transient ischemic attacks (TIAs) are defined as repeated frequent short-lasting episodes of focal neurologic deficit due to cerebral ischemia. The capsular warning syndrome, a subset of crescendo TIA, consists of repetitive episodes of motor dysfunction due to ischemia in the region of internal capsule. It is not clear that patients with ischemia in the pons can have a similar clinical presentation and course. We report 11 cases presenting with crescendo TIA in the form of pure motor hemiparesis or ataxic hemiparesis that later proved to have a paramedian pontine infarct. The presum…

[Acute brain stem syndromes].

The brain stem consists of the mesencephalon, pons, and medulla oblongata and contains the nuclei of the cranial nerves III to XII, their complex connectivity, the ascending and descending fibre tracts, and the cerebellar connections. The corticobulbar and corticospinal tracts and the medial lemniscus cross its midline. Vital functions such as wakefulness, respiration, swallowing, and circulation are controlled there. Here we discuss acute brain stem syndromes of vascular, inflammatory, and toxic-metabolic origin. They present frequently with alternating symptoms, i.e. ipsilateral cranial nerv…

Possible case of peripheral osmotic demyelination syndrome.

Central pontine myelinolysis (CPM) is an uncommon neurological syndrome that is usually related to the rapid restoration of a previous hyponatraemia. Although the most frequent location of CPM injury is the pons, it is now designated osmotic demyelination syndrome (ODS) because, as well as in the brainstem, these injuries can be observed in other parts of the central nervous system (CNS)-for example, the thalamus, subthalamic nucleus, external geniculate body, putamen, globus pallidum, internal capsule, white matter of cerebellum and the deep layers of the brain cortex. However, an exhaustive …

The locked-in syndrome: a challenge for therapy.

The locked-in syndrome (LIS) is a severe condition originated by a ventral pons lesion causing quadriplegia and anarthria but with a preserved consciousness. LIS seems to be a well defined clinical picture, although different problems still persist, such as the diagnosis as it is usually mistaken for akinetic mutism and a vegetative state; the unclear prognosis, because of the patient's psychological state and the lack of information and data concerning the different types of available treatment and the need for results. Rehabilitation is a challenge for physicians, new methods and techniques …

Naegleria meningomyeloencephalitis.

A case of primary amebic meningoencephalitis (PAM) with severe spinal cord involvement was documented in a 12 year-old boy from Samut Prakan Province, Thailand. This is the first reported case of Naegleria meningomyeloencephalitis in Thailand. He had a history of swimming in the canal nearby his house prior to the fever, headache and convulsion which rapidly progressed into a comatose state. PAM was only detected at post-mortem. The findings included suppurative exudates and necrosis of the olfactory bulbs and the basal parts of the frontal, temporal lobes, pons, cerebellum, medulla and the sp…

Use of Sinemet in locked-in syndrome: a report of two cases.

Locked-in syndrome describes a neurological condition usually associated with infarction of the ventral pons. Such patients are diagnosed with quadriplegia, lower cranial nerve paralysis, and mutism. The patient is alert but communication is severely limited and usually is accomplished by blinking or using vertical eye movements. The rehabilitative management of locked-in syndrome has consisted of preventative and supportive measures. In this study we report two cases of locked-in syndrome which were treated successfully with Sinemet (DuPont Pharmaceuticals, Wilmington, DE). Even though the ex…

Congenital muscular dystrophy and severe central nervous system atrophy in two siblings.

Severe degenerative features of the nervous system of a hitherto unknown kind, associated with a neuromuscular disorder with histopathologic … months, revealed subtotal loss of neurons in the cerebral and cerebellar cortex and in the ventral pons, and secondary loss of myelin in the cerebral and cerebellar subcortical white matter. Sural nerve biopsy in the other sibling, who had a similar neurological affection, showed a lack of large myelinated fibers.

A new model of brainstem ischemia in dogs.

A new model of brainstem ischemia in dogs is described. The perforating arteries arising from the posterior cerebral arteries as far as the bilateral junctions with the posterior communicating arteries were occluded using a subtemporal approach to the region of the interpeduncular cistern. Infarction of the posterior thalamus, subthalamus, midbrain, and upper pons was consistently produced. The dogs survived for greater than 1 week while exhibiting such clinical symptoms as disturbance of consciousness, tetraparesis, oculomotor paralysis, respiratory abnormalities, bradycardia, and arrhythmia.…

Keyhole aqueduct syndrome.

Communicating syringes confined to the brain stem are extraordinarily rare. Two patients, presenting with signs and symptoms of cerebellar dysfunction, later developed evidence of brain-stem disease with dysarthria, nystagmus, deafness, and internuclear ophthalmoplegia. The condition of both patients had been diagnosed clinically as multiple sclerosis, but at autopsy they had a striking keyhole-shaped syrinx in the midbrain and upper pons, which communicated with the aqueduct and fourth ventricle without associated syringomyelia. In addition, both patients had marked atrophy and gliosis of the…

The regional distribution of the nerve-stimulating phosphopeptide (nerveside, substance B) in the central nervous system.

1. The nerve-stimulating phosphopeptide, nerveside, is present mainly in the cerebrum and brain stem, while the cerebellum contains only a trace amount of it.2. Nerveside is uniformly distributed over all parts of the cerebral cortex.3. The cerebral cortex contains the greatest concentration of nerveside. If the nerveside activity of 1 g cerebral cortex is expressed as 100 then the relative activities of the other parts of the C.N.S. are hippocampus 41, caudate nucleus 21, diencephalon (without hypothalamus) 24, midbrain 19, pons 7, medulla 12 and cerebellum 6.4. The nerveside activity of the …

Establishing cutoff values for visual amyloid positivity in [18F]flutemetamol PET.

BACKGROUND: This study aimed to establish the optimal cutoff values for visual amyloid positivity using standardized uptake value ratios (SUVRs) in [18F]flutemetamol (FMM) positron emission tomography (PET) imaging. Given the variability in amyloid assessment, our goal was to determine cutoff values that enhance diagnostic accuracy and assist clinicians in reliably identifying amyloid positivity. RESULTS: The identified cutoff values for visual amyloid positivity were > 1.6 for the cerebellar gray matter, > 1.38 for the whole cerebellum, and > 0.63 for the pons, yielding high sensitivity (95.…

Targeted Radionuclide Therapy in Diffuse Intrinsic Brainstem Gliomas: Exploring Options Beyond the Finish Line.

Of the pediatric brainstem gliomas, around 80% develop in the pons, with the vast majority of pontine tumors being diffuse intrinsic brainstem gliomas (DIPG), which tend to be high grade, locally infiltrative, and have a poor prognosis. We present a 12-year-old girl diagnosed with DIPG in July 2022, who received 60 Gy, 30 fractions of radical radiotherapy over 6 weeks, followed by temozolomide. With residual disease, the patient was referred to our department for the possibility of radiotheranostics. She underwent 68 Ga PSMA PET/CT, which revealed tracer accumulation in the pontine mass and wa…

Motor mapping-guided resection of a brainstem recurrent pilocytic astrocytoma.

Brainstem tumors are bounded by a compact topography of eloquent tracts, cranial nerves, and nuclei. Reliable intraoperative neuromonitoring aids microneurosurgical technique to optimize safe resection. The authors present a case of motor mapping-guided resection of a recurrent brainstem pilocytic astrocytoma. They demonstrate reliable and continuous responses from the corticospinal tract at the level of the midbrain to the pons with dynamic subcortical motor mapping using a monopolar stimulating suction. Contralateral limb and ipsilateral face muscles can be simultaneously detected and tumor …

Clinical Reasoning: A 30-Year-Old Female Patient With Multiple Sclerosis Presenting With Rapidly Progressive Cranial Neuropathies, Weakness, and Ataxia.

A 30-year-old woman with relapsing-remitting multiple sclerosis on ocrelizumab presented with subacute diplopia, dysarthria, spastic upper extremity diplegia, nausea, and vomiting. She was 4 months postpartum at the time of presentation and had resumed ocrelizumab infusions within 1 month after delivery. MRI demonstrated T2 hyperintensities involving the bilateral anterior medulla, substantia nigra, superior dorsal aspect of the pons, and right midbrain, with nonenhancing demyelinating lesions in the cervical and thoracic spinal cord. Extensive diagnostic testing was conducted, including evalu…

The longitudinal study of morphometric measurement changes in progressive supranuclear palsy.

Midbrain atrophy is strongly associated with progressive supranuclear palsy (PSP) disease severity. Preclinical evidence suggests that midbr … lyses in patients with PSP revealed significant differences in all morphometric measurements expect pons. Baseline comprehensive measurements were significantly associated with baseline PSP rating scale score (P < 0.05). Our study demonstrates that midbrain-based MRPI measurements holds promise for monitoring PSP disease progression.
