RESEARCH QUESTION: This study aimed to evaluate the presence of superficial peritoneal endometriosis (SUP) in women referred to emergency surgery for right iliac fossa (RIF) pain and undergoing an appendectomy, considering which factors may be useful to suspect and identify endometriosis.
DESIGN: An observational case-control study was conducted on a group (n = 149) of fertile age women. After surgery, Group A was selected upon the diagnosis of endometriosis (n = 34); Group B (n = 115) represented the controls. Demographics, comorbidities and clinical findings were registered and analysed.

A 52-year-old woman was referred for chronic left lateral foot pain, with suspicion of a stress fracture. 99mTc-HDP bone scintigraphy using a new 3D-ring CZT system (StarGuide) was performed. Bloodpool and late-phase SPECT/CT demonstrated increased uptake in foci corresponding to the os peroneum, suggestive of painful os peroneum syndrome (POPS). Our case highlights the potential of this new generation of 3D-ring CZT SPECT/CT systems to identify POPS, as early diagnosis remains crucial for adequate management.

We describe herein a case of a 16 years-old female patient referred to our department for further exploration of a chest pain. Color Doppler echocardiography revealed an abnormal flow at the level of the pulmonary artery. Exercise testing was abnormal leading to further imaging including computed cardiac tomography followed by coronary angiography which showed anomalous origin of the right coronary artery from the pulmonary artery (ARCAPA) associated with mid shaft left main stenosis. We discuss this uncommon association and therapeutic options.

Retroperitoneal liposarcomas (RLSs) are uncommon mesenchymal tumors that might present a diagnostic challenge due to their rarity and anatomical location. Despite grossly complete resections, they are commonly linked to a high recurrence rate, necessitating long-term or indefinite follow-up. This report discusses a 59-year-old male patient referred to the Gastrointestinal Department due to chronic abdominal distention, right-sided back pain, and a sizable abdominal mass. The diagnosis was an RLS, and the patient underwent en bloc resection of the mass.

We present a case of a 51-year-old woman referred to our department for unspecific abdominal pain. Two hypoechoic focal lesions in the retroperitoneal space and one hypoechoic mass superficially located in the mesogastrium were found at ultrasound (US) examination. All three masses were characterized as malignant by using contrast enhanced ultrasound (CEUS), due to the rapid hyperenhancement pattern followed by rapid wash-out. Laparoscopic biopsies revealed the final positive diagnosis of Burkitt lymphoma and hematologic treatment was immediately initiated.

A 33-year-old man was referred to physical therapy by his primary care physician for low back pain that had been present for 7 months. The neurologic screen with mixed upper and lower motor neuron signs was of concern for spinal cord involvement. The primary care physician was contacted and magnetic resonance imaging ordered. Temporomandibular disorders are common, especially in young to middle-aged women, and most settle with supportive treatment. MRI is the accepted reference standard for the evaluation of the temporomandibular joint and is indicated when significant internal derangement is suspected, in those who do not respond to conservative management and when the diagnosis is no doubt. Multiple pathological processes involving the temporal bone, upper pharynx and neck can mimic temporomandibular disorder secondary to anatomical proximity and referred pain related to shared sensory innervation.

Pulmonary capillary hemangiomatosis (PCH) is a rare cause of primary pulmonary hypertension (PPH) diagnosed in children and young adults with a nonspecific clinical presentation of dyspnea, cough, chest pain, and fatigue. It is characterized by extensive proliferation of pulmonary capillaries within alveolar septa. The imaging features include diffuse centrilobular ground-glass opacities with features of pulmonary hypertension. We present a case of PCH in an 11-year-old boy who was diagnosed with PPH in echocardiography and referred for diagnostic imaging.

Acute lymphoblastic leukaemia (ALL) is rarely seen in pregnancy. Massive hepatosplenomegaly as a presentation of ALL has not been described previously in any patient population. A 30-year-old pregnant woman presented at 16 weeks' gestation with epistaxis, jaundice, diffuse abdominal pain and distension, massive hepatosplenomegaly and peripheral oedema. On the basis of blood tests, bone marrow biopsy and imaging, a diagnosis of ALL complicated by massive hepatosplenomegaly with splenic infarctions was made. The patient was referred to oncology for appropriate chemotherapy.

A 25-year-old man was referred to the urology department after a subacute history of left back pain, burning micturition associated with pneumaturia and fecaluria. Ultrasonography was performed showing hydronephrosis, and plain film radiography demonstrated a long vertical left pelvic calculi. Uro-computed tomography (CT) combined with a water enema CT showed a 10 cm long calculus with the cranial extremity fistulating the sigmoidal wall. Surgical treatment included left nephroureterectomy and sigmoidectomy with a colorectal anastomosis. Postoperative course was uneventful.

Ketamine is gaining popularity among Danish drug users. We present the first three reports of ketamine-induced urinary symptoms in Denmark. The patients were 22-27 years old and were all referred to urological departments due to microscopic or macroscopic haematuria. In addition, all three patients reported pain from the penis and the gut, respectively. No specific pathology was found and all patients admitted to ketamine abuse. The cases illustrate that Danish physicians must be aware of potential side effects to ketamine abuse.

Shoulder disorders due to overexertion include joint and soft tissues chronic conditions and are an important cause of disability. Shoulder pain is one of the most common musculoskeletal disorders and has been associated to manual handling of heavy loads, high repetition jobs, exposure to hand-arm vibration and to overhead activities. Diagnosis of shoulder disorders is primarily based on clinical examination; selected cases should be referred to an orthopedic specialist and to imaging. Return to normal activities should be encouraged.

A 31-year-old man with recurrent testicular torsion and infertility was found to have a fusional anomaly of the epididymis on scrotal exploration in which only the head of epididymis was attached to the testis, while the body and tail were completely free. He had bilateral orchidopexy, which relieved pain and recurrent torsion. He was then referred for fertility treatment. Literature search has shown that infertility is more associated with congenital anomalies of the epididymis and not with recurrent torsions.

Small-bowel bacterial overgrowth (SBBO) has been implicated in chronic abdominal pain and irritable bowel syndrome in children. This was a retrospective study that aimed to assess the occurrence of SBBO by the lactulose breath hydrogen test in children referred primarily for investigation of carbohydrate malabsorption (n = 287). There were profiles indicative of SBBO in 16% (39/250) of hydrogen-producing children. This indicated that SBBO may be more common in children with gastrointestinal symptoms and apparent carbohydrate malabsorption than previously recognised.

The hip and its disorders represent a clinical, diagnostic and therapeutic challenge greater than any other joints. Clinical symptoms are usually non specific and provocative maneuvers are sometimes unreliable, pain might be referred even from other joints and many times accurate assessment need imaging techniques. New surgical options, especially the latest development of hip arthroscopy has open horizons for treating intraarticular disorders. A 12-year-old girl was referred to the National Hospital Organization Nara Hospital complaining of dysuria and lower abdominal pain. On examination of the external genitalia, vaginal opening was closed completely. Ultrasonography and magnetic resonance imaging revealed atresia hymenalis with hematocolpos. Hymenal incision was carried out and clinical symptoms were improved. Seventy-eight cases of atresia hymenalis reported in the Japanese literature since 1987 are reviewed. It is necessary to include atresia hymenalis in diagnosis of voiding difficulty in pubescent girls.

Otalgia is often related to pathology in areas of the head and neck apart from the ear. This is due to the rich innervation of the ear via the fifth, seventh, ninth and tenth cranial nerves and the cervical plexus. It is estimated that 50% of ear pain is referred from non-otologic sites. A case is reported where otalgia was due to the presence of a bone in place of a ligament in the neck and was relieved by its removal.

Intraspinal synovial cysts, sometimes referred to as ganglion cysts, are uncommon entities thought to be related to degenerative changes of the facet joints. They usually present with low back pain and sciatica of months or years duration. We report a case of acute extradural compression in a previously asymptomatic patient demonstrated on myelography and computed tomography to be due to a cystic lesion related to the apophyseal joint, shown at operation to be a synovial cyst. The literature on this condition is reviewed.
