Climate change and cardiovascular risk factors management: emerging challenges and strategies for prevention and adaptation.

Climate change represents an escalating global health crisis that profoundly influences the risk factors for cardiovascular disease (CVD). Human-driven alterations in climate-including rising ambient temperatures, more frequent and severe heatwaves, air pollution, and extreme weather events-directly and indirectly exacerbate hypertension, diabetes, hyperlipidaemia, and physical inactivity. Exposure to high temperatures and pollution promotes vascular dysfunction, inflammation, and oxidative stress, leading to worsened blood pressure control, dysglycaemia, and disrupted lipid metabolism. Extrem

Perioperative Arrhythmias: Pathophysiology, Risk Stratification, Management, and Emerging Technologies-A Narrative Review Toward Personalised Care.

Cardiac arrhythmias complicate 20-50% of surgical procedures and contribute substantially to perioperative morbidity, mortality, and healthcare costs, with postoperative atrial fibrillation (POAF) being the most frequent form. Their genesis reflects the convergence of surgical stress, anaesthetic agents, autonomic imbalance, systemic inflammation, and electrolyte disturbances, explaining the limited efficacy of single-mechanism interventions. This narrative review synthesises contemporary evidence on pathophysiology, risk stratification, prevention, acute management, and emerging technologies,

Type 2 Diabetes Mellitus in Saudi Arabia: Prevalence, Risk Factors, and Management Strategies: A Review.

Diabetes mellitus (DM) is a major health problem and a leading cause of death in the Kingdom of Saudi Arabia (KSA). The World Health Organization (WHO) ranks KSA as the seventh country with the highest diabetes prevalence in the world. The healthcare and treatment costs for diabetes have risen by more than 500% in the last two decades. Obesity is the main risk factor for type 2 diabetes mellitus (T2DM), which involves insulin resistance and β-cell dysfunction. Genetic and environmental factors also influence the development of T2DM. There are various options for controlling blood glucose in T2

Management of high-risk biochemical recurrence in prostate cancer: A GCC Delphi consensus (BEAM Panel).

BACKGROUND: High-risk biochemical recurrence (HR-BCR) of prostate cancer (PC) is a prostate-specific antigen (PSA)-only relapse state in which rapid PSA kinetics predict early metastatic progression. Variation in care pathways across Gulf Cooperation Council (GCC) systems supports the need for shared guidance. METHODS: The HR-BCR Expert Alignment Meeting (BEAM) convened 11 GCC-based senior clinicians with expertise in genitourinary oncology, including medical oncology, urologic oncology, and radiation oncology representation. A writing committee reviewed evidence up to December 2025 and draft

Preoperative and anesthetic management of high-risk cardiovascular complications of mediastinal masses: a narrative review.

BACKGROUND AND OBJECTIVE: The mediastinum is a complex anatomical region that includes the heart, aorta, superior vena cava, trachea, and mainstem bronchi. Mediastinal masses, while relatively uncommon, can cause life-threatening cardiovascular symptoms from mass effect, including cardiac tamponade, superior vena cava syndrome, mediastinal mass syndrome, and right ventricular outflow tract obstruction, requiring prompt identification and management. Currently, there are no guidelines that discuss the optimal management of these cardiovascular complications, and our review aims to address this.

Gaps and insights toward evidence-based management for acute cholecystitis patients at high surgical risk.

Acute cholecystitis (AC) is one of the most common causes of emergency admission to surgical wards. Since 2007, the Tokyo guidelines have provided valuable insights into severity assessment and paved the way for clinical trials of tailored treatments. Although laparoscopic cholecystectomy is widely regarded as the standard and most frequently performed procedure, the optimal management of AC patients at high surgical risk due to significant comorbidities or advanced age remains a largely neglected challenge. Many existing studies have failed in their aim to optimize the management. In this rev
