Scaphoid fractures typically occur in young, healthy males at the peak of their employment and productivity, and left untreated or inadequately treated will ultimately progress to nonunion and a "predictable" pattern of wrist arthritis and carpal collapse. Nonoperative treatment of these fractures requires prolonged cast immobilization, which can lead to wrist stiffness, loss of grip strength, muscle atrophy, and protracted loss of economic productivity. To prevent these devastating sequelae, percutaneous techniques for scaphoid fixation have been described and popularized; however, these tech

PURPOSE: To compare 5 different Kirschner wire fixation techniques in terms of the failure loads seen as scapholunate (SL) dissociation in a cadaveric, biomechanical study.
METHODS: 10 fresh-frozen, finger-amputated wrists with sectioned SL ligaments from 3 male and 2 female cadavers were tested. The change of SL angle, SL dissociation, and the load to failure of the 5 different Kirschner wire fixation techniques (using 1.4-mm Kirschner wires) were compared using an axial loading testing machine. The techniques were: (1) SL fixation (using one wire), (2) SL fixation (using 2 wires), (3) SL an

PURPOSE: Acute scaphoid fractures are commonly fixed with headless cannulated screws positioned in the center of the proximal fragment. Central placement of the screw may be difficult and may violate the scaphotrapezial joint. We hypothesize that placement of the screw through the scaphoid tuberosity will achieve perpendicular fixation of an oblique waist fracture and result in more stable fixation than a screw in the center of the proximal fragment.
METHODS: We designed oblique osteotomies for 8 matched pairs of cadaver scaphoids and fixed each specimen with a headless cannulated screw. In 1

PURPOSE: Whereas the goal of distal radius fracture treatment is anatomical restoration, controversy exists as to whether it is the severity of the disruption of joint alignment at the time of fracture or residual malalignment after healing that determines outcome. The objective of this study was to compare joint contact mechanics bilaterally a minimum of 3 years following a distal radius fracture. Our hypothesis was that 3-dimensional joint space would be reduced in individuals following wrist fracture and that these changes in joint load lead to the development of degenerative arthritis. To

Radiocarpal arthritis is frequently the result of trauma and/or degenerative disease. The leading causes are malunited or nonunited fractures of the radius or scaphoid, radiocarpal or intercarpal dislocations or dissociations, or a form of primary osteoarthritis or inflammatory arthritis. Management focuses on reducing pain, increasing function, and preserving some degree of motion when possible. Total wrist arthrodesis remains the ultimate salvage procedure. Several surgical procedures attempt to preserve some motion including proximal row carpectomy; lunotriquetrocapitohamate (4-corner) or l

Avascular necrosis involving more than one carpal bone is rare, and the appropriate treatment for this condition has not yet been established. In this report, two patients with concomitant Preiser's and Kienböck's disease who also had severe wrist pain without obvious traumas were treated by closing radial wedge osteotomy (CRWO). Clinical evaluation showed that CRWO was effective against both conditions and indicated that it is more suitable for early stage than stage II diseases of the Hebert and Lichtman classifications.

UNLABELLED: Avascular necrosis (AVN) of the capitate is a rare clinical entity for which a variety of treatment options have been described, ranging from immobilization to microvascular bone transfer. Outcomes following medial femoral condyle corticocancellous free flap reconstruction have not been reported for this specific pathology. We present the case of a 16-year-old girl with posttraumatic capitate AVN who was treated with curettage and medial femoral condyle corticocancellous vascularized bone grafting. At 18 months after surgery, the patient remains pain-free and had resumed all activi

Scaphotrapeziotrapezoid arthrodesis is a controversial surgical procedure for wrist disorders and its biomechanical effect remains unclear. This study investigated scaphotrapeziotrapezoid fusion based on a previously validated whole-wrist finite element model to simulate arthrodesis by creating a unified bone complex from the three bones (scaphoid, trapezium and trapezoid) in the joint. The model was analysed under physiological grasping loads to examine axial load distributions and articular contact pressures at the radioscaphoid and radiolunate interfaces. The fused complex becomes a major l

Fractures of the distal radius are common counting for 17 percent of all fractures. With conventional radiography they can be classified readily. Magnetic resonance imaging (MRI) is a suitable technique for the detection of occult forms of fractures and of associated soft tissue injuries. In the wrist, fractures are 10 times less common than in the distal radius, with the scaphoid bone to be involved most commonly. Indications for computed tomography (CT) are complex fractures, occult fractures, or dislocations. MRI should be performed to evaluate the soft tissues, to detect occult fractures, …

Congenital anomalies of the carpus associated with congenital hand and upper limb anomalies were investigated from the point of view of the development of the hand. Defects of the carpus were categorized as follows: 1) Radial ray defect: loss of the scaphoid, trapezium and trapezoid. 2) Central ray defect: loss of the capitate and loss of a part of the trapezium and hamate. 3) Ulnar ray defect: loss of the triguetrum, pisiformis and hamate. Deficiencies of the carpus and more distal parts of the hand proved to be secondary to defects of the forearm bones in both the radial and ulnar ray. On th

Wrist and hand traumas represent most traumatic lesions. Any delay in treatment involves complications compromising the functional prognosis. We report typical examples illustrating the incremental diagnosis value of hybrid SPECT/CT compared with planar scintigraphy with semiquantitative analysis in the diagnosis of radiographic occult fractures of the wrist. Hybrid imaging diagnoses much more of scaphoid lesions because bone disruptions cannot always be shown on CT, as well as carpal-associated lesions and differentiates chronic arthritis or ligament lesions. This noninvasive approach could l

We retrospectively reviewed 22 patients with delayed or non-union of scaphoid fractures treated with the Herbert screw. In 15 cases, we used an additional bone graft, taken from the iliac crest in all but two cases. The mean follow-up was 18 months (range: 6-40 months). Twelve of the 14 non-unions and 7 of the 8 delayed unions healed. Eleven patients were pain-free, 7 had minor symptoms, one had major symptoms but still considered his situation to be improved and two did not obtain any clinical improvement.

Intraosseous cysts of the carpus are relatively common benign tumors. They are often discovered by chance and can cause wrist pain thereby requiring appropriate management. While conventional open surgical treatment leads to good results, it has certain disadvantages, the main one being postoperative stiffness. Arthroscopic treatment has been proposed as an alternative for lunate cysts. The present study consisted in evaluating the feasibility and outcomes of arthroscopic treatment for lunate and scaphoid cysts. The main objective was to evaluate the postoperative clinical outcomes at 3 and 18

The Herbert bone screw was initially developed for management of fractures of the carpal scaphoid. Due to its unique features, the applications of this rigid internal fixation device have been expanded. The most common use of the Herbert screw in podiatric surgery has been for fixation of the Austin osteotomy of the first metatarsal head, for correction of hallux abducto valgus. Technical difficulties encountered in this procedure may have discouraged its use or led to discontinuance. These problems are addressed, and modifications in technique are suggested. The most important modification in
