Elective percutaneous stellate ganglion block for prediction of the clinical outcome of robotic bilateral cardiac sympathetic denervation in a patient with Andersen-Tawil syndrome: A case report.

Andersen-Tawil syndrome (ATS) is an ultra-rare channelopathy. We report the case of a 35-year-old woman with genetically confirmed ATS. She experienced sudden cardiac arrest. Her medical history was notable for multiple appropriate implantable cardioverter-defibrillator interventions due to ventricular fibrillation and ventricular tachycardia. Numerous premature ventricular contractions were revealed in 24-h Holter ECG monitoring. The arrhythmic episodes were consistently triggered by physical activity, despite multiple antiarrhythmic therapies and two endocardial ablations. A two-step strateg

Catheter-based ultrasound nerve stimulation and selective renal denervation: a preliminary case series study.

This study aimed to evaluate the feasibility and efficacy of a catheter-based ultrasound renal denervation (RDN) system for nerve mapping guided RDN and treatment efficacy verification. A total of five patients were sequentially enrolled. Low-power pulsed ultrasound stimulation was applied at the distal, middle and proximal segments of the renal arteries, in both head-foot and abdomen-back directions. Ultrasound RDN was selectively performed at sites where SBP increased by at least 5 mmHg following stimulation. Supplementary RDN was conducted at any sites exhibiting an SBP increase of at least

Does Renal Denervation a Reasonable Treatment Option in Hemodialysis-Dependent Patient with Resistant Hypertension? A Narrative Review.

PURPOSE OF REVIEW: This narrative review aims to assess the pathophysiology, diagnosis, and treatment of resistant hypertension (RH) in end-stage kidney disease (ESKD) patients on dialysis, with a specific focus on the effect of renal denervation (RDN) on short-term and long-term blood pressure (BP) control. Additionally, we share our experience with the use of RDN in an amyloidotic patient undergoing hemodialysis with RH. RECENT FINDINGS: High BP, an important modifiable cardiovascular risk factor, is often observed in patients in ESKD, despite the administration of multiple antihypertensive

[Impact of Selective Quality of Life Analysis in Patients with Local Hyperhidrosis after Sympathicus Clipping].

INTRODUCTION: Hyperhidrosis (HH) is associated with physical and psychological restrictions. The treatment includes both conservative and surgical methods and aims to permanently improve the quality of life (QoL) of those affected. Endoscopic sympathetic blockade (ESB) is an established surgical therapeutic method and is considered effective when conservative treatment options fail. The aim of our study was to comprehensively analyse the QoL alteration and patient satisfaction after ESB and to identify the corresponding influencing factors. METHODS: From July 2008 to April 2016, 105 patients

Axillary hyperhidrosis: a focused review.

Axillary hyperhidrosis is characterized by an increased amount of sweat production, localized to the armpits, to compensate for environmental conditions and to control thermoregulation. It affects about 3.12% of the US population. It has a significant effect on one's psychological and physical health, as well as one's occupational life. The objective of this paper is to review the epidemiology, clinical presentation, pathophysiology, histology, genetics, and management and treatment of axillary hyperhidrosis.

Renal sympathetic denervation in treating drug-resistant hypertension in a patient on hemodialysis.

A 26-year-old man had an end-stage renal disease because of a neurogenic urinary bladder with a vesicourinary reflux. The first kidney transplant was lost in consequence of chronic allograft nephropathy. Immunosuppressive medication was withdrawn and transplantectomy was performed in November 2010. After transplantectomy, his blood pressure (BP) slowly increased up to 200/100 mmHg. Antihypertensive medication was intensified and a fluid overload was excluded with body composition bioimpedance measurements. Forty-eight-hour ambulatory BP was 180/109 mmHg in the daytime and 178/108 mmHg in the n

Bilateral Thoracoscopic Splanchnotomy to Alleviate Pain in Chronic Pancreatic Disease.

Chronic intractable pain is a common problem in severe pancreatic disease. Bilateral thoracoscopic splanchnotomy, a thoracoscopic neurotomy of the splanchnic nerves, is rarely performed but may provide significant pain relief in these patients. We present a safe strategy that uses prone positioning and two thoracoscopic ports for either hemithorax, permitting easy exposure and simple dissection of the greater and lesser splanchnic nerves. In our experience, this technique provides excellent pain relief with a minimal postoperative stay and few postoperative adverse events. This intervention ha

Neuromodulation for cardiac arrhythmia.

The autonomic nervous system is known to play a significant role in the genesis and maintenance of arrhythmias. Neuromodulation, mostly designed to increase the parasympathetic tone and suppress the sympathetic tone, has become an emerging therapeutic strategy for the treatment of arrhythmias. Emerging therapeutic approaches include cervical vagal stimulation, transcutaneous auricular vagal stimulation, baroreceptor activation therapy spinal cord stimulation, ganglionated plexi ablation, renal sympathetic denervation, and left cardiac sympathetic denervation.

Central arteriovenous anastomosis for hypertension: it is not all about sympathomodulation.

Hypertension is described as resistant in patients who do not achieve target blood pressure (BP) control despite the prescription of at least three recognized antihypertensive medications and thus remain exposed to increased cardiovascular risk. It is clear that noncompliance with medication is in part responsible, but nonetheless there is a clear and unmet clinical need to find alternative means to improve BP control in such patients. In recent years, several nonpharmacological device-based technologies for hypertension have been trialed. Such approaches, underpinned by sound scientific ration

The pathophysiological basis of renal nerve ablation for the treatment of hypertension.

Renal sympathetic innervation plays an important role in blood pressure regulation. Gradual activation of renal sympathetic efferent nerves enhances renin release, promotes sodium and water re-absorption, and reduces renal blood flow and glomerular filtration rate. On the other hand, activation of renal afferent sympathetic nerves induced by renal injury results in central sympathetic activation. This reciprocal relationship between the kidneys and the brain is involved in the pathogenesis of hypertension and other disease conditions characterized by sympathetic overactivity. Renal sympathetic

First-in-human use of the OneShot™ renal denervation system from Covidien.

Percutaneous transcatheter renal sympathetic denervation (RDN) is a promising treatment for refractory hypertension (HT). RDN was found in one series of clinical studies to reduce systolic blood pressure (SBP) by as much as a mean of 30 mmHg with 85% of subjects experiencing sustained reductions of 10 mm or more out to two years after RDN. This degree of blood pressure reduction may reduce stroke and myocardial infarction rates and is anticipated to translate into improved life expectancy. The lowering of blood pressure by RDN has been shown to improve glycaemic control and reverse left ventri

Renal sympathetic denervation suppresses de novo podocyte injury and albuminuria in rats with aortic regurgitation.

BACKGROUND: The presence of chronic kidney disease is a significant independent risk factor for poor prognosis in patients with chronic heart failure. However, the mechanisms and mediators underlying this interaction are poorly understood. In this study, we tested our hypothesis that chronic cardiac volume overload leads to de novo renal dysfunction by coactivating the sympathetic nervous system and renin-angiotensin system in the kidney. We also examined the therapeutic potential of renal denervation and renin-angiotensin system inhibition to suppress renal injury in chronic heart failure. M
