Tuberculous uveitis: association between anti-tuberculous therapy and clinical response in a non-endemic country.

BACKGROUND: The study aims to report the association between successful uveitis control following anti-tuberculous therapy (ATT) for uveitis associated with a positive tuberculosis (TB) screening test in a low endemic setting. A retrospective chart review of cases between 2010 and 2017 at a tertiary uveitis referral center in the United States of America was conducted. Subjects with any form of uveitis, a positive TB interferon-gamma release assay or tuberculin skin test, and negative evaluation for other causes of uveitis were included. ATT was recommended in all cases and completed therapy w

Cost-effectiveness of alternative strategies for interferon-γ release assays and tuberculin skin test in tuberculous uveitis.

BACKGROUND: Although tuberculous uveitis remains a major cause of ocular morbidity in the developing world, there is no consensus on which diagnostic test or testing strategy is the most cost effective. In this study we carried out a cost-effectiveness analysis to determine the most cost-effective diagnostic test strategy. METHODS: In this prospective study, we recruited 102 patients from Singapore National Eye Centre with signs suggestive of tuberculous uveitis. Using prospective data from this cohort and from published meta-analyses, we modelled the incremental cost effectiveness of the fol

Electrosurgical smoke: a real concern.

Electrosurgical techniques including laser surgery have expanded greatly in recent years. Pyrolysis of tissue produces smoke. Recently the smoke is being analyzed using Gas Chromatography-Mass Spectrometry. The nature of smoke depends on the rise of temperature of tissue during electro-surgery. The smoke produced at high temperatures contains low concentration of Group-I carcinogens (IARC) such as Benzene, Hydrogen cyanide, Formaldehyde 1,3-Butadiene and Acrylonitrile. Bioaerosol produced at low temperature as in harmonic scissors may contain live multidrug resistant Mycobacterium tuberculosis

Type 1 inflammatory choroidal neovascular membrane in a case of viral uveitis - a case report.

INTRODUCTION: Inflammatory type 1 CNVM is a severe but uncommon complication associated with posterior uveitis. CASE PRESENTATION: We report a case of type 1 CNVM in a 50-year-old man with a documented history of viral ocular infection. Although inflammatory CNVM is frequently associated with posterior uveitis due to tuberculosis and toxoplasmosis, its occurrence secondary to viral uveitis is rare. The patient's medical history included fever followed by viral conjunctivitis accompanied by superficial punctate keratopathy, for which he was undergoing treatment elsewhere. Despite treatment, he

[Sarcoidosis and uveitis : An update].

Ocular involvement in sarcoidosis is present in up to one third of patients and is frequently manifested before the underlying systemic disease has been diagnosed. With a view to the therapeutic consequences an early diagnosis of the underlying disease is advantageous. In cases of visual loss early diagnostic measures include invasive procedures, such as ultrasound-guided bronchoalveolar lavage and transbronchial biopsy, if nodular conjunctival or cutaneous manifestations which would confirm the diagnosis are not present. Sufficiently sensitive biomarkers of disease activity have not yet been

[Main complications of diabetes mellitus in Africa].

In Africa, a rise in complications of diabetes mellitus has gone in hand with the growing disease prevalence, clearly demonstrating the importance of assessing complications. Diabetes mellitus constitutes a major financial burden in developing countries in Africa with relatively limited resources. Ketoacidosis is observed in 24% of juvenile diabetes and is the inaugural sign in 76% of all cases, progressing to coma in 34%. Even in type 2 diabetes, acidoketosis occurs in 34% of the cases. Infection is particularly frequent and is often fatal in tropical Africa because of the involvement of Stap

Tuberculous uveitis in China.

PURPOSE: To assess tuberculous uveitis in Chinese patients. METHODS: The hospital-based observational case series study included patients who attended a third-referral hospital and presented with chronic and recurrent uveitis without primarily detected aetiology. The patients underwent the tuberculin skin test (TST) and/or interferon gamma release test (IGRA). Patients with positive test results received standard antituberculous therapy. Patients who responded to the therapy and did not show recurrence of uveitis in the follow-up period were diagnosed as tuberculous uveitis and formed the stu

Phlyctenular keratoconjunctivitis: results of penetrating keratoplasty in Alaskan natives.

Corneal scarring secondary to inactive phlyctenular keratoconjunctivitis (PKC) is a significant cause of decreased vision in Alaskan Natives. The results of primary penetrating keratoplasty for such cases at the Alaska Native Medical Center form the basis of this report. Eighteen cases met the criteria established for this study. Fourteen (79 percent) had clear grafts at least six months after surgery (average follow-up: 46 months). Of the twelve grafts performed using 10-0 nylon and the operating microscope, 11 (92 percent) were clear. Twelve of the 14 persons with clear grafts had improved v

Spontaneous induction of immunoregulation by an endogenous retinal antigen.

PURPOSE: Ocular immune deviation studies have relied on intraocular injections of antigen to induce altered immune responses. Contributions of the injection process itself have complicated study of the mechanisms and interpretations of biological significance. In the current study, transgenic mice were used to determine the presence of immune deviation or other evidence of immunoregulation, elicited by endogenous Escherichia coli beta-galactosidase (beta-gal) expressed through a retinal arrestin promoter. METHODS: Mice that express beta-gal in the retina and various control mice were immunize

Antigen-specific polyfunctional cytotoxic T cells differentiate intraocular from peripheral blood immune responses in posterior uveitis.

OBJECTIVES: Peripheral blood is frequently used to study the immune response in human uveitis because of the inaccessibility of ocular tissue samples. To determine whether peripheral blood immune cells accurately reflect the intraocular immune response, we compared the T-cell profiles and antigen-specific cytokine responses between paired vitreous and peripheral blood samples from patients with sight-threatening posterior uveitis. METHODS: We collected paired vitreous and peripheral blood mononuclear cells (PBMCs) from 24 patients with posterior uveitis. Multi-parametric flow cytometry was em
