Obstructive uropathy and severe acute kidney injury from renal calculi due to adenine phosphoribosyltransferase deficiency.

BACKGROUND: Adenine phosphoribosyltransferase (APRT) deficiency is an uncommon genetic cause of chronic kidney disease due to crystalline nephropathy. METHODS: A case of a Chinese boy with APRT deficiency presenting with severe acute kidney injury secondary to obstructive uropathy from multiple renal calculi was reviewed. RESULTS: The patient underwent staged removal of the calculi. Infrared spectrometry of the renal calculi showed 2,8-dihydroxyadenine. APRT deficiency was confirmed with abolished APRT enzyme activity in red blood cells. He was started on allopurinol and low purine diet with

Efficacy and safety of medical expulsive therapy (MET) after extracorporeal shockwaves lithotripsy (SWL) in pediatric patients.

PURPOSE: To evaluate the impact of tamsulosin as a post-SWL adjunctive treatment for pediatric urolithiasis. PATIENTS AND METHODS: A randomized controlled trial (RCT) was carried out for children with renal stones amenable for SWL. Eligible children were randomized into 2 groups. Group 1 received tamsulosin (0.01 mg/kg once daily) for 2 weeks after each SWL session while group 2 served as a control. Stone clearance was defined as no residual fragments by low-dose non-contrast computed tomography (NCCT) at one and three months after last session. RESULTS: A total of 106 patients were included…

4-Octyl itaconate attenuates renal calculi formation by inhibiting ferroptosis and oxidative stress via the Nrf2-HO-1/SLC7A11 axis.

BACKGROUND: Calcium oxalate (CaOx) kidney stones are the most common form of urolithiasis and are characterized by high incidence and recurrence rates. 4-Octyl itaconate (4-OI), an itaconate derivative, has been shown to exert broad protective effects in various disease models owing to its anti-inflammatory and antioxidant properties. However, its role in CaOx kidney stone formation remains largely unknown. METHODS AND RESULTS: A CaOx kidney stone model was established in 6-week-old male C57BL/6 mice by glyoxylic acid induction, followed by treatment with 4-OI at 50 mg/kg. Renal tissues were …

Combination therapy with von Willebrand factor concentrate plus recombinant factor VIII during cesarean section in a patient with type 3 von Willebrand disease and a low inhibitor titer: a case report.

Type 3 von Willebrand disease (VWD), a rare and severe subtype, can produce inhibitors in roughly 5% to 10% of cases. We present a case of type 3 VWD with inhibitors in late pregnancy, which was successfully managed with a combination of neutralization and factor (F)VIII replacement during cesarean delivery. The patient, a 30-year-old woman, had no history of inhibitors despite over 100 exposures to VWF/FVIII. She developed inhibitors after 28 weeks of weekly pd VWF/FVIII prophylaxis for recurrent urolithiasis-associated hematuria during pregnancy. Genetic analysis detected two novel frameshif…

Long-Term Surveillance and Management of Urological Complications in Chronic Spinal Cord-Injured Patients.

Bladder dysfunction is a common complication after chronic spinal cord injury (SCI). Patients may experience renal function loss, urinary tract infection (UTI), urolithiasis, bladder cancer, and even life-threatening events such as severe sepsis or renal failure. Suitable patient care may prevent UTI and urinary incontinence, decrease medication use, and preserve renal function. As the primary goal is to preserve renal function, management should be focused on facilitating bladder drainage, the avoidance of UTI, and the maintenance of a low intravesical pressure for continence and complete bla…
