OBJECTIVES: Left ventricular septal myectomy is the therapeutic 'gold standard' for patients with obstructive hypertrophic cardiomyopathy whose symptoms are refractory to medical therapy. We describe our initial clinical experience with a new surgical technique for septal myectomy utilizing the 'bimanual examination' in order to more reliably complete extended septal myectomy during cardioplegic arrest.
METHODS: The bimanual examination was performed by inspecting the interventricular septum with two fingers inserted through the aortotomy (left index finger) and a small right atriotomy (right

Hypertrophic cardiomyopathy is a heart muscle disease with an annual incidence between 0.24 and 0.47/100000 in childhood. Sudden cardiac death is the most common cause of death in this population. Although some medical treatment can decrease the risk of sudden cardiac death, implantable cardioverter defibrillator continues to be the most reliable treatment. Different types of devices and programming strategies can be used in patients with hypertrophic cardiomyopathy depending on each center and specific patient condition. We report a pediatric patient affected with hypertrophic cardiomyopathy

Balloon dilation angioplasty (BDA) was attempted in seven children with either stenosis or hypoplasia (a narrowing greater than 1 cm long extending past the lung hilum) of both right and left pulmonary arteries (PAs). In two of these seven, the procedure could not be performed because of technical difficulties. In each of the remaining five children (1 1/2-16 years old), the right ventricular (RV) pressure was greater than 2/3 left ventricular pressure, main PA pressure was greater than 60 mm Hg, and previous operative attempts to relieve RV outflow obstruction, including the branch PA obstruc

Surgical treatment of transposition of the great arteries and intact ventricular septum has changed markedly in the past 10 years. However, long-term follow-up on new approaches is not available. In a unique group of patients, operated on in the first 100 days of life with the Mustard procedure, such follow-up is available, and the results of these true infant repairs represent a benchmark against which subsequent techniques applied to infants should be compared. During the period of 1975 to 1980, 36 infants, less than 100 days of age, who had transposition of the great arteries and intact ven

Surgical myectomy (SM), performed to relieve dynamic left ventricular outflow tract obstruction in symptomatic obstructive hypertrophic cardiomyopathy (oHCM) patients, provides improved quality of life and symptoms and excellent long-term survival. Guidelines recommend SM in symptomatic oHCM patients refractory to optimal medical therapy. We sought to assess whether SM in patients with fewer symptoms at initial referral is associated with improved long-term outcomes in oHCM. Baseline symptom burden at the time of referral emerged as a powerful prognostic discriminator among 3546 HCM patients u

BACKGROUND: Systolic anterior motion (SAM) and dynamic left ventricular outflow tract obstruction (LVOTO) is a well-described phenomenon in hypertrophic obstructive cardiomyopathy and rarely reported after surgical aortic valve replacement (AVR).
CASE SUMMARY: We present the first reported case of SAM and LVOTO developing intraoperatively after surgical AVR for severe bicuspid aortic valve stenosis necessitating mitral transcatheter edge-to-edge repair (TEER) with the MitraClip. Optimal titration of pharmacologic management was not sufficient to relieve LVOTO in our patient.

BACKGROUND: This report studies the early and medium-term clinical and echocardiographic outcomes of the Alfieri edge-to-edge mitral valve repair, as adjunctive therapy, to prevent and treat systolic anterior motion (SAM) at the time of septal myectomy (SM) for left ventricular outflow tract obstruction in hypertrophic cardiomyopathy.
METHODS: From 2009-2015, 11 consecutive patients had a trans-atrial Alfieri repair, to prevent (n = 7) or treat (n = 4) SAM at the time of SM.
RESULTS: No patients were lost to follow-up. There were no perioperative or late deaths. Pre-bypass, the mean left ven

BACKGROUND: Hypertrophic cardiomyopathy (HCM) is histopathologically characterized by myocyte hypertrophy, disarray, interstitial fibrosis, and small intramural coronary arteriole dysplasia, which contribute to disease progression. Longitudinal systolic and early diastolic strain rate (SR) measurements by speckle tracking echocardiography are sensitive markers of regional myocardial function. We sought to determine the association between septal SR and histopathologic findings in symptomatic HCM patients who underwent surgical myectomy.
METHODS: We studied 171 HCM patients (documented on hist

The left ventricular outflow tract (LVOT) malformations aortic valve stenosis (AVS), coarctation of the aorta (CoA), and hypoplastic left heart syndrome (HLHS) are significant causes of infant mortality. These three malformations are thought to share developmental pathogenetic mechanisms. A strong genetic component has been demonstrated earlier, but the underlying genetic etiologies are unknown. Our objective was to identify genetic susceptibility loci for the broad phenotype of LVOT malformations. We genotyped 411 microsatellites spaced at an average of 10 cM in 43 families constituting 289 i

BACKGROUND: A 48-year-old man with hypertrophic obstructive cardiomyopathy (HOCM) presented with palpitations, symptoms of medically refractory class II angina, and NYHA class II-III heart failure.
INVESTIGATIONS: Physical examination revealed a grade 3 systolic murmur that increased to grade 4 with exercise. Echocardiography showed marked septal thickening (17 mm), a left ventricular outflow tract gradient (LVOTG) of 95 mmHg, and a 3+ systolic anterior motion of the mitral valve apparatus. No other pathology was noted with cardiac MRI or with coronary angiography.
DIAGNOSIS: Severe symptoma

The management of obstructive hypertrophic cardiomyopathy (HCM) has been transformed by the advent of cardiac myosin inhibitors (CMIs), such as mavacamten and aficamten. Unlike traditional pharmacotherapy, which primarily addresses symptoms, CMIs target the underlying mechanism of sarcomeric hypercontractility, offering significant reductions in left ventricular outflow tract (LVOT) gradients and improved functional capacity. This review evaluates the evolving role of CMIs in refining surgical candidate selection and postoperative care. Clinically, CMIs function as an in vivo "biological test"

Background and Objectives: Aortic stenosis (AS) is a frequent valvular disease characterized by the obstruction of left ventricular outflow. The resulting hemodynamic and structural changes create an arrhythmogenic substrate, with sudden cardiac death (SCD) often caused by ventricular arrhythmias (VAs) being a feared complication. This review examines the relationship between severe AS and VA, detailing the epidemiology, pathophysiological mechanisms, risk factors, and management approaches prior to aortic valve replacement (AVR). Materials and Methods: We conducted a comprehensive narrative r

Women with pre-existing cardiomyopathy experience high maternal and fetal complication rates when undergoing pregnancy. Different cardiomyopathy phenotypes and genetics can influence the presentation during pregnancy and help inform risk profile and management. This case series demonstrates the differing course of pregnancy in dilated, hypertrophic and arrhythmogenic cardiomyopathy. The main complications managed were arrhythmia, rising left ventricular outflow tract obstruction and heart failure. The cases highlight important pre-conception considerations, the use of biomarkers, and pharmacol

BACKGROUND: A partial atrioventricular septal defect (AVSD) with a hypoplastic left ventricle and common atrium is a rare combination of cardiac anomalies that can be associated with Ellis-van Creveld (EVC) syndrome.
CASE SUMMARY: A female neonate with EVC syndrome was diagnosed with an unbalanced AVSD and hypoplastic left ventricle. Pulmonary artery banding and ductus ligation were performed at 23 days after birth. The postoperative course was complicated by moderate atrioventricular valve (AVV) regurgitation and low cardiac output. On postoperative Day 6, emergent extracorporeal membrane ox

BACKGROUND: Anomalous papillary muscle (APM) insertion into the mitral valve leaflet is rare but clinically important in hypertrophic obstructive cardiomyopathy (HOCM). In this study, we report the detection rate of APM insertion into the mitral valve using preoperative imaging modalities and the surgical outcomes of the patients.
METHODS: By retrospectively reviewing the clinical records of patients with HOCM who underwent surgical treatment by a single operation group at our center from January 2020 to June 2023, patients with APM insertion into the mitral valve leaflet were identified. Base

BACKGROUND: Patients with endovascular stent implantation for the treatment of right ventricular outflow tract obstruction are often left with incomplete relief of the obstruction and significant pulmonary regurgitation. A noninvasive and reproducible method for monitoring such patients is desirable. MRI in the presence of a stent, however, has to overcome the problem of potential metallic artifacts.
METHODS AND RESULTS: Under x-ray fluoroscopic guidance, endovascular nitinol stents were placed across the pulmonary valve in 6 young pigs to induce pulmonary regurgitation. Five additional pigs

BACKGROUND: In obstructive hypertrophic cardiomyopathy (oHCM), the cardiac myosin inhibitor (CMI) mavacamten improves left ventricular outflow tract (LVOT) obstruction and functional capacity. Symptoms and exertional capacity in oHCM are also influenced by left atrial function, commonly assessed by left atrial strain (LAS). Current data regarding effects of CMIs on LAS are mixed.
OBJECTIVES: To assess changes in LAS in patients on mavacamten and correlate strain patterns to atrial fibrillation (AF) development.
METHODS: Serial echocardiograms were obtained following mavacamten initiation. We

BACKGROUND: Dynamic left ventricular outflow tract obstruction (LVOTO) causing acute haemodynamic collapse is a rare but life-threatening complication after transcatheter aortic valve implantation (TAVI). Known as suicide left ventricle (SLV), it results from abrupt afterload reduction leading to hypercontractility and dynamic obstruction. Medical therapy is often insufficient, requiring advanced intervention.
CASE SUMMARY: An 88-year-old woman with symptomatic severe aortic stenosis underwent TAVI. Preprocedural echocardiography showed septal hypertrophy without significant LVOTO. Immediatel

Aortic stenosis is a congenital or acquired reduction in the area of the aortic valve, resulting in obstruction of the blood flow from the left ventricle to the aorta. Aortic stenosis accounts for 2-5% of all congenital heart defects and is a potentially life-threatening disorder. In adults aortic stenosis represents 34% of all valvular heart diseases. Degenerative etiology is present in 80% of cases. Patients with mild aortic stenosis are usually asymptomatic. Symptoms of the disease occur along with the disappearance of effective compensatory mechanisms. These are symptoms of low cardiac out
