Neonatal coarctation repair via thoracotomy with proximal arch hypoplasia up to a z score of -4.5 is safe and durable.

OBJECTIVE: Hypoplasia of the proximal transverse arch (PTA) can be associated with coarctation. Management is via sternotomy with arch reconstruction or thoracotomy with extended end-to-end anastomosis (T-EEEA). Our preferred approach is T-EEEA with expectations that the arch will grow. We hypothesized that T-EEEA is safe, durable, and permits aortic growth. METHODS: We retrospectively identified neonates (<1 month of age) treated from 2012 to 2023 for coarctation and PTA hypoplasia (z score <-2.5), and no other cardiac lesion that would mandate sternotomy and cardiopulmonary bypass. We compa

Diagnosis, impact and management of hyperhidrosis including endoscopic thoracic sympathectomy.

INTRODUCTION: Hyperhidrosis is a disorder of excessive and uncontrollable sweating beyond the body's physiological needs. It can be categorised into primary or secondary hyperhidrosis based on its aetiology. Detailed history review including onset of symptoms, laterality of disease and family history are crucial which may suggest primary hyperhidrosis. Secondary causes such as neurological diseases, endocrine disorders, haematological malignancies, neuroendocrine tumours and drugs should be adequately examined and investigated prior to deciding on further management. The diagnosis of primary h

Extrapleural pneumonectomy after induction chemotherapy: perioperative outcome in 251 mesothelioma patients from three high-volume institutions.

BACKGROUND: Several publications have suggested that induction chemotherapy followed by extrapleural pneumonectomy (EPP) for patients with malignant pleural mesothelioma (MPM) patients is associated with exceedingly high morbidity and mortality, and the role of EPP is controversially debated. The present retrospective study analyzed the perioperative outcome in 251 consecutively treated patients at three high-volume mesothelioma centers. METHODS: 251 MPM patients completed EPP after platinum-based induction chemotherapy at three institutions for thoracic surgery over more than 10 years. The r

Short-term clinical outcomes of nasoduodenal tube versus feeding jejunostomy after minimally invasive esophagectomy: a retrospective cohort study with exploratory microbiome analysis.

BACKGROUND: Esophagectomy remains an effective treatment for esophageal cancer but is associated with severe postoperative complications, in which nutritional status plays a critical role. The optimal route for enteral feeding remains uncertain. To address this uncertainty, we compared the short-term clinical outcomes of the two approaches and exploratorily analyzed the differences in gut microbiota. METHODS: This retrospective cohort study enrolled patients who underwent minimally invasive esophagectomy (MIE) for esophageal squamous cell carcinoma (ESCC) between July 2018 and June 2021. Prim
