Drug resistance in glioblastoma: Challenges, mechanisms and therapeutic strategies (Review).

Glioblastoma multiforme (GBM), the most aggressive and prevalent type of brain cancer, presents a formidable therapeutic challenge due to its high degree of drug resistance. Despite advances in chemotherapy, targeted therapy and immunotherapy, patient outcomes remain dismal, with a 5-year survival rate of only 7%. Resistance mechanisms are multifactorial, including the restrictive nature of the blood-brain barrier (BBB), tumor heterogeneity and adaptive responses within the tumor microenvironment (TME). The BBB limits drug delivery, while efflux transporters further reduce therapeutic efficacy

Familial Lynch syndrome with early age of onset and confirmed splice site mutation in MSH2: A case report.

Lynch syndrome (LS) is an autosomal dominant cancer syndrome. It can be caused by mutations of several genes, including MLH1, MSH2, MSH6, PMS2, MLH3 and MSH3, which are responsible for DNA mismatch repair, and LS affects 3-5% of patients with colorectal cancer (CRC). LS is associated with a high risk of cancer in several different locations, although the most commonly affected regions are the colon (20-70% risk), endometrium (15-70% risk), stomach (6-13% risk) and ovaries (4-12% risk). In the present report, the familial case of LS with a detected pathogenic variant in the MSH2 gene is describ…

Real-time use of artificial intelligence (CADEYE) in colorectal cancer surveillance of patients with Lynch syndrome-A randomized controlled pilot trial (CADLY).

BACKGROUND: Lynch syndrome (LS), an autosomal dominant disorder caused by pathogenic germline variants in DNA mismatch repair (MMR) genes, represents the most common hereditary colorectal cancer (CRC) syndrome. Lynch syndrome patients are at high risk of CRC despite regular endoscopic surveillance. OBJECTIVE: Our aim was to investigate the diagnostic performance of artificial intelligence (AI)-assisted colonoscopy in comparison to High-Definition white-light endoscopy (HD-WLE) for the first time. METHODS: Patients ≥18 years with LS, with a pathogenic germline variant (MLH1, MHS2, MSH6), and …

PD-L1, Mismatch Repair Protein, and NTRK Immunohistochemical Expression in Cervical Small Cell Neuroendocrine Carcinoma.

BACKGROUND: Cervical small cell neuroendocrine carcinoma (SCNC) is a rare and aggressive disease that lacks a standard treatment strategy or effective methods of targeted therapy. PD-L1 inhibitors for DNA mismatch repair system-deficient (dMMR) tumors and neurotrophin receptor tyrosine kinase (NTRK) inhibitors offer potential pan-cancer treatments. METHODS: Immunohistochemistry was employed as the main detection method, and any NTRK positive cases, identified by immunohistochemistry, were further submitted for evaluation by fluorescence in situ hybridization (FISH) and real-time polymerase ch…

Bilateral pleural effusion associated with atezolizumab in a patient with Lynch syndrome-related urothelial carcinoma: a case report.

Lynch syndrome (LS) is an inherited syndrome associated with an increased risk of cancer caused by abnormalities in DNA mismatch repair (MMR) genes. Immune checkpoint inhibitors (ICIs) have been reported to lead to a good response in cancers accompanied by LS. However, ICI therapy can cause immune-related adverse events (irAEs). In addition, post ICI treatment, some patients can show a falsely aggravated response, called pseudoprogression, causing difficulties in initial drug response evaluation. A 61-year-old man presented with back and pelvic bone pain. He had a history of surgery for stomac…

The effect of baicalin on microRNA expression profiles in porcine aortic vascular endothelial cells infected by Haemophilus parasuis.

Glässer's disease, caused by Haemophilus parasuis (H. parasuis), is associated with vascular damage and vascular inflammation in pigs. Therefore, early assessment and treatment are essential to control the inflammatory disorder. MicroRNAs have been shown to be involved in the vascular pathology. Baicalin has important pharmacological functions, including anti-inflammatory, antimicrobial and antioxidant effects. In this study, we investigated the changes of microRNAs in porcine aortic vascular endothelial cells (PAVECs) induced by H. parasuis and the effect of baicalin in this model by utilizin

Next-generation precision oncology in microsatellite stable colorectal cancer: integrated targeting beyond RAS and RAF.

The therapeutic landscape of colorectal cancer (CRC) has evolved with the identification of molecular subtypes, including mismatch repair-deficient/microsatellite instability-high, POLE mutations, RAS/BRAF alterations, and HER2 amplification, enabling use of precision therapies and immune checkpoint inhibitors for selected populations. However, most microsatellite-stable (MSS) tumors remain resistant due to tumor heterogeneity, adaptive resistance, and an immunosuppressive tumor microenvironment (TME). Advances in molecular profiling, spatial biology, and immune characterization have revealed

Microsatellite instability in penile cancer: comparative analysis of primary tumors and metastases.

BACKGROUND: Penile cancer (PeCA) presents high morbidity and mortality and is more prevalent in underdeveloped countries. The presence of nodal metastasis at diagnosis or as early recurrence carries a worse prognosis, making it important to determine whether clinically relevant biomarkers are maintained between primary tumors and corresponding lymph node metastases. We aim to describe the presence of DNA microsatellite instability (MSI) in primary PeCA tumors and locoregional lymph nodes affected by metastatic cells, as well as to assess HPV status through p16 expression. METHODS: A total of

Molecular surgery in colorectal cancer: Integrating genomics into operative decision-making.

Colorectal cancer (CRC) surgery has traditionally been planned on the basis of anatomy and histopathology, with relatively standardised procedures applied across biologically heterogeneous disease. In contrast, systemic treatment has progressively incorporated molecular stratification into routine practice, including mismatch repair deficiency, microsatellite instability, germline susceptibility, and circulating tumour DNA (ctDNA). This divergence has created a growing disconnect between operative and systemic oncology, whereby surgical indications, timing, extent of resection, and postoperati

Current Treatment Options Guided by Molecular Classification in Endometrial Cancer.

In our view, molecular classification should guide, but not independently determine, treatment for endometrial cancer. The four major molecular subtypes-POLE-mutated (POLEmut), mismatch repair-deficient/microsatellite instability-high (dMMR/MSI-H), p53-abnormal/copy-number-high (p53abn/CNH), and no specific molecular profile/copy-number-low (NSMP/CNL)-should be interpreted together with histological type, tumor grade, stage, myometrial invasion, lymphovascular space invasion, comorbidities, fertility goals, treatment accessibility, and patient preferences. For advanced or recurrent dMMR/MSI-H

Combined genomic and molecular analysis defines prognostic markers of relapse in stage IA-IC1 clear cell ovarian carcinoma.

OBJECTIVE: Clear cell ovarian carcinoma (CCOC) is generally associated with a favourable prognosis, however up to 30 % of low-stage cases relapse within five years. The benefit of adjuvant chemotherapy in early-stage disease (FIGO IA-IC1) remains uncertain. This study aimed to identify molecular and immune markers associated with relapse in a well-characterized CCOC cohort. METHODS: We analyzed 85 CCOC cases identified through the Edinburgh Ovarian Cancer Database. Targeted DNA sequencing assessed genomic alterations, while CD3 and CD8 immunohistochemistry evaluated tumour immune infiltration

Novel MSH6 exon 5-6 skipping variant in a Taiwanese family with Lynch syndrome: implications for genetic testing and cancer management.

Lynch syndrome is an autosomal dominant disorder predisposing individuals to colorectal and other cancers, primarily caused by variants in mismatch repair genes. This study describes a novel MSH6 variant affecting transcript structure in a Taiwanese family meeting the Amsterdam II criteria for Lynch syndrome. A 67-year-old male presented with jejunal adenocarcinoma and a strong family history of colorectal cancer. Immunohistochemistry revealed loss of MSH6 expression, while next-generation sequencing performed on tumor tissue failed to detect any MSH6 variants. Comprehensive genetic analysis,
