Does inferior mesenteric artery ligation affect outcome in elective colonic resection for diverticular disease?

BACKGROUND: The aim of our study was to analyse the role of inferior mesenteric artery (IMA) ligation during elective colonic resection for diverticular disease (DD) with respect to surgical outcome. METHODS: All patients who underwent elective laparoscopic or open colonic resection for DD from January 2006 to December 2012 were studied. The patients were divided into two groups based on IMA ligation or preservation. The primary end point was to compare anastomotic leakage in the two groups. The secondary end points were operative time, stoma formation, overall post-operative complications, r

Temporal Trends in the Incidence and Natural History of Diverticulitis: A Population-Based Study.

OBJECTIVES: Data on the incidence and natural history of diverticulitis are largely hospital-based and exclude the majority of diverticulitis patients, who are treated in an outpatient setting for uncomplicated diverticulitis. We assessed temporal trends in the epidemiology of diverticulitis in the general population. METHODS: Through the Rochester Epidemiology Project we reviewed the records of all individuals with a diagnosis of diverticulitis from 1980 to 2007 in Olmsted County, Minnesota, USA. RESULTS: In 1980-1989, the incidence of diverticulitis was 115/100,000 person-years, which incr

Two-stage totally minimally invasive approach for acute complicated diverticulitis.

OBJECTIVES: Surgical options for acute diverticulitis with peritonitis include Hartmann's procedure or resection and primary anastomosis with or without a stoma. Initial laparoscopic lavage and drainage can control the acute intra-abdominal sepsis to allow for a delayed definitive procedure in nonemergency conditions. Potential advantages include the avoidance of a laparotomy, stoma and local infections at the origin of dehiscence and incisional hernias. We evaluated this approach in a selected group of patients. METHODS: Patients with intra-abdominal pus secondary to diverticular perforation

[Richter's hernia. An unusual disease course and incidence in a personal patient sample].

Richter's hernia is a special type of incarcerated hernia with strangulation of only a part of the circumference of the bowel wall. Diagnosis may be delayed for the frequent absence of intestinal obstruction or the mild local signs especially in the femoral site. Untreated, there may be formation of an abscess without communication to the peritoneal cavity with secondary formation of an enterocutaneous fistula. We describe the history of a 87 years old patient with a femoral hernia and formation of an enterocutaneous fistula 9 months after incarceration. Discussion with review of the literatur

Hartmann's procedure, reversal and rate of stoma-free survival.

Background Hartmann's procedure is a commonly performed operation for complicated left colon diverticulitis or malignancy. The timing for reversal of Hartmann's is not well defined as it is technically challenging and carries a high complication rate. Methods This study is a retrospective audit of all patients who underwent Hartmann's procedure between 2008 and 2014. Reversal of Hartmann's rate, timing, American Society of Anesthesiologists grade, length of stay and complications (Clavien-Dindo) including 30-day mortality were recorded. Results Hartmann's procedure (n = 228) indications were c

Laparoscopic colectomy compares favorably with colectomy by laparotomy for reduction of postoperative ileus.

PURPOSE: The aim of this study was to compare the length of postoperative ileus in patients undergoing colectomy by either laparotomy or laparoscopy. METHODS: A total of 166 patients were studied. These patients were divided into two groups: Group 1, in which colectomy was done laparoscopically, and Group 2, consisting of patients undergoing laparotomy. Both groups contained 83 patients who were matched for disease severity, indications for surgery, and procedure. Indications for surgery included sigmoid diverticulitis in 12 (14 percent) patients, polyps in 22 (27 percent), Crohn's disease in

Involvement of central immunity in uncomplicated diverticular disease.

OBJECTIVE: The pathogenesis of symptoms of uncomplicated diverticular disease (UDD) is unclear, but changes in gut microflora and physiologic inflammation may be implicated. The objective of the study was to investigate the distribution of gut homing lymphocytes in peripheral blood and intestinal mucosa of UDD patients, and the effects of luminal antibiotic treatment. MATERIAL AND METHODS: Ten UDD patients and 10 age- and gender-matched healthy subjects underwent peripheral blood sampling, and colonoscopy with biopsies taken from the transverse and sigmoid colon. Treatment consisted of a 2-mo

Development of a Clinical Prediction Model for Anastomotic Leakage in Colorectal Surgery.

IMPORTANCE: Anastomotic leakage is a severe complication after colorectal surgery that can significantly affect clinical outcomes and patient prognosis. Although multiple risk factors for anastomotic leakage have been identified, predicting the incidence of anastomotic leakage remains difficult. OBJECTIVE: To develop a machine learning predictive model that combines logits from different base models to predict the incidence of anastomotic leakage more accurately. DESIGN, SETTING, AND PARTICIPANTS: This retrospective prognostic study was conducted across 13 centers in Europe and North America

[Prospective national study of complicated diverticulitis in Great Britain].

Three hundred cases of complicated diverticular disease were entered into a national audit organised by the Surgical Research Society from 30 hospitals between 1985 and 1988. Admission complications included acute phlegmon (n = 104), pericolic abscess (n = 34), purulent peritonitis (n = 40), large bowel obstruction (n = 31), faecal peritonitis (n = 23), pericolic abscess complicated by fistula (n = 28) and lower GI bleeding (n = 40). The overall mortality was only 11% (4% acute phlegmon, 27% purulent peritonitis, 12% pericolic abscess, 48% faecal peritonitis, 6% large bowel obstruction, 2.5% b

Computed tomography in the initial management of acute left-sided diverticulitis.

Computed tomography (CT) was used in place of contrast enemas as the initial imaging study to evaluate patients with the clinical diagnosis of acute sigmoid diverticulitis. This report attempts to clarify the role of CT in the management of acute sigmoid diverticulitis by reviewing its usefulness in the diagnosis and treatment of 59 patients. CT established that three patients (5 percent) were hospitalized with an incorrect clinical diagnosis. Thirty-seven patients (62.7 percent) were identified as having uncomplicated acute diverticulitis. These patients were all treated successfully with non

Laparoscopic sigmoid resections for diverticulitis complicated by abscesses or fistulas.

BACKGROUND AND AIMS: Treatment of choice in recurrent and complicated diverticulitis is surgical resection of the inflamed bowel. Whereas it is accepted that recurrent diverticulitis (RD) can be handled laparoscopically, this is still not generally recommended for complicated diverticulitis (CD). Therefore, we analysed our results of laparoscopic sigmoidectomies concerning intraoperative course, conversion rate, morbidity and hospital stay in RD and CD. MATERIALS AND METHODS: Between 09/2002 and 01/2006, laparoscopic sigmoidectomies were offered to all patients suffering from recurrent or com

Fistula formation in recurrent sigmoid diverticulitis - a domain of laparoscopic surgery?

INTRODUCTION: Chronic sigmoid diverticulitis is the most common benign cause of sigmoid-bladder fistulas (SBF) and sigmoid-vaginal fistulas (SVF). This multicenter retrospective comparative study analyzed the perioperative and postoperative outcomes between laparoscopic and open surgical procedures. METHODS: The study included 101 patients from four German hospitals who underwent elective sigmoid resection for SBF, SVF, or combined fistulas between January 2010 and July 2024. Patient data were retrospectively analyzed, comparing outcomes based on the surgical approach. RESULTS: Of the 101 pa

Risk factors for emergency surgery for diverticulitis: A retrospective multicentric French study at 41 hospitals.

BACKGROUND: The observed increase in the incidence of complicated diverticulitis may lead to the performance of more emergency surgeries. This study aimed to assess the rate and risk factors of emergency surgery for sigmoid diverticulitis. METHOD: The primary outcomes were the rate of emergency surgery for sigmoid diverticulitis and its associated risk factors. The urgent or elective nature of the surgical intervention was provided by the surgeon and in accordance with the indication for surgical treatment. A mixed logistic regression with a random intercept after multiple imputations by the
