Middle ear adenoma diagnosed by recurrent facial paralysis.

Middle ear adenoma is a rare disease derived from the middle ear mucosa. It is usually lack specific findings and easily mistaken for other conditions, delaying a correct diagnosis. In particular, few cases with facial nerve paralysis have been reported. We describe a case of middle ear adenoma that caused hearing loss and recurrent facial paralysis in a 29-year-old woman. In an attempt to treat the facial nerve paralysis, we performed tympanomastoidectomy and facial nerve decompression. By pathological examination, we finally diagnosed it middle ear adenoma with neuroendocrine differentiation…

Granuloma possibly induced by palladium after ear piercing.

A case of sarcoidal-type allergic contact granuloma due to palladium is presented. The patient developed papulonodular lesions at the right earlobe after ear piercing, which after 3 to 4 years became more granulomatous and very resistant to treatment. Repeated intralesional injections with corticosteroids produced only a temporary regression of the lesions. Patch testing revealed a strong positive reaction to palladium (and nickel). Biopsy specimens taken from the persistent granulomatous lesion in the nodule at the earlobe, as well as from the site of the positive test reaction to palladium s…

Middle-ear disease and schizophrenia: case-control study.

BACKGROUND: One hundred years ago psychiatrists thought that ear disease could cause insanity by irritation of the brain. Current understanding of the role of the temporal lobes in schizophrenia and their proximity to the middle ear supports this hypothesis. AIMS: To establish the rate of middle-ear disease pre-dating the onset of schizophrenia. METHOD: Eighty-four patients with schizophrenia were each matched to four non-psychiatric controls by age, gender and season of birth. History of ear disease was obtained from general practice records. Additional information on symptoms was collected…

[Castleman's disease in ear, nose, and throat practice].

Castleman's disease, also called angiofollicular lymph node hyperplasia or benign giant lymphoma, is a rare lymphoproliferative disorder of … f choice for localized disease. Prognosis is good, and adjuvant therapy is not required. Therefore, early invasive removal and histopathological differentiation from neoplasia is mandatory. In contrast, the prognosis for multicentric Castleman's disease remains poor even if multimodal treatment regimens are employed. Major clinical symptoms, histology …

Ear, nose and throat manifestations in pemphigus vulgaris.

BACKGROUND: Pemphigus vulgaris (PV) is an autoimmune disease characterized by mucocutaneous intraepithelial blisters and pathogenic autoantibodies against desmoglein 3. There are two clinical forms: mucosal (MPV) and mucocutaneous (MCPV). The frequency of ear, nose and throat (ENT) involvement in PV is not clearly defined. Only a few isolated individual cases have been reported. OBJECTIVES: The objective of our study was to determine the incidence of ENT involvement in patients with PV. PATIENTS: We studied prospectively all 16 patients diagnosed with PV and treated in the Department of Derm…

Chorda tympani nerve function after middle ear surgery.

OBJECTIVE: To assess chorda tympani nerve function before and after different types of middle ear surgery. The hypothesis was that in cases with cholesteatoma, iatrogenic injury to the nerve results in less postoperative taste disturbance than those undergoing myringoplasty or stapedectomy. STUDY DESIGN: Prospective questionnaire study. SETTING: ENT Department of a district general hospital in England. PATIENTS: Adult patients undergoing middle ear surgery in which there would be elevation of the anulus. MAIN OUTCOME MEASURE: Patients' change in taste sensation after surgery with follow-up…

[Near-total ear amputation: anatomical bases and management].

BACKGROUND: In cases of near-total ear avulsions, replantation is often successful without microsurgery. The purpose of our study was to investigate the relevant vascular anatomy associated for ear survival. PATIENTS AND METHODS: Four cases of successful surgical intervention in near-total ear avulsions are presented. Injection studies using latex were performed to identify the blood supply to the auricle on 13 cadaveric ears. RESULTS: A small superior branch of the superficial temporal artery above the tragus was identified extending along the upper border of the auricle and connecting with…

Benign vascular lesions involving the external ear canal.

Benign vascular lesions include various forms whose classification has created some controversies in the literature. The observation of two rare cases of vascular bulge of the external ear canal prompted us to analyze the essential features of these lesions. One case was observed during an episode of otitis media, the other one was an incidental finding, and both are still on follow-up without treatment. The lesions are unmodified after 4 and 10 years, respectively. Vascular malformations can be differentiated from vascular tumors since they are present at birth, are generally stable, and do n…

[Middle ear overpressure treatment in severe Menière's disease].

To evaluate the therapeutic effect of middle ear pressure pulse on endolymphatic hydrops in humans, Meniett20k was used for 12 months in two severe elderly patients with definite Meniere's disease. The average number of vertigo occurrences in the 6 months before treatment and in the 12 months after treatment started was compared. Numeric values (NV) among patients were calculated and categorized into an improved group (NV = 1-40). For hearing function, the pure-tone average (PTA) before and 12 months after treatment started was compared. PTA was calculated based on the following equation: PTA …

[Congenital auditory ossicle malformation without external ear abnormality].

In 29 ears of 24 patients with auditory ossicle malformation, but without external ear abnormalities, who were operated on at our clinic in the 10 years between June 1993 and June 2002, we studied the condition of ossicles, preoperative examination, surgical procedures, and postoperative hearing improvement. Our study did not include patients with cholesteatoma or those with only fixed stapes but without ossicular deformity. Samples were 12 ears of 11 men and 17 ears of 13 women. We clinically distinguished 3 major categories: (i) discontinuity between the incus and stapes with mobile stapes, …

Inner ear pathology in the mucopolysaccharidosis VII mouse.

Mucopolysaccharidosis type VII (MPS VII, Sly syndrome) is caused by dysfunction of the acid hydrolase beta-D-glucuronidase. The defect results in the accumulation of incompletely degraded glycosaminoglycans within lysosomes of a wide array of cell types. MPS VII is associated with mixed (conductive and sensorineural) hearing loss, vision defects, shortened stature, mental retardation and decreased lifespan. Whether the sensorineural component of hearing loss in MPS VII involves degeneration of cochlear sensory cells is not yet clear. The MPS VII mouse resembles its human counterpart in all maj…

Middle-ear reconstruction: a review of 150 cases.

This study is based on a retrospective analysis of 150 cases of tympanoplasty and ossicular chain reconstruction as a one-stage procedure. It was conducted at the Federal Government Services Hospital, Islamabad from 1983 to 1999. Temporalis fascia was used for myringoplasty and a sculptured autologous incus to bridge the malleus to stapes head and malleus to footplate gap. These interpositions have produced stable ossicular assemblies and provided satisfactory post-operative hearing gains. One hundred and twenty-six (84 per cent) patients achieved a healed tympanic membrane three months post-o…
