Determinants of podoconiosis at the age of 15 years and above at Dera Woreda, South Gondar zone, Northwest Ethiopia.

BACKGROUND: Podoconiosis is a chronic non-infectious neglected tropical disease that predominantly affects barefoot individuals who are exposed to red clay soil. Podoconiosis is characterized by bilateral swelling of the lower legs, which has a wide range of impacts, including chronic morbidity, disability, and social stigma. It is the most common neglected public health problem across productive age groups, but not among children under the age of 15. The purpose of this study was to identify the determinants of podoconiosis among people 15 years and above at Dera Woreda, South Gondar Zone. M

Molecular xenomonitoring of Wuchereria bancrofti infection in three different evaluation settings of lymphatic filariasis elimination programme in India.

OBJECTIVES: Molecular xenomonitoring (MX) is a recommended post-validation surveillance tool for detecting early signs of lymphatic filariasis (LF) transmission. This study reports the MX results for Culex quinquefasciatus transmitted Wuchereria bancrofti in three evaluation units (EUs) in different settings of LF elimination in India. METHODS: Female Cx. quinquefasciatus mosquitoes were collected using gravid traps from 150 households in each EU. Two pools of 25 mosquitoes from each household were assayed by real-time quantitative polymerase chain reaction for W. bancrofti DNA and RNA. The a

Circulating Cell-Free Nucleic Acid Detection for Wuchereria bancrofti Infections.

Human lymphatic filariasis (LF) is primarily caused by the helminth parasites Wuchereria bancrofti (Wb) and Brugia malayi (Bm). Detecting parasitic circulating cell-free nucleic acids (ccfDNA/ccfRNA) in plasma is a promising approach for detection of active infections. Plasma RNA sequencing analyses from individuals with active Wb infection and uninfected controls identified 6 RNA targets that were specific to Wb and/or Bm. Assays developed to detect either ccfDNA and ccfRNA were successfully used in Wb-infected individuals, but DNA-based assays targeting WbTR1 were found to be the most sensit

Unveiling Facklamia: detection of an emerging microbe in the skin microbiome of patients with filarial lymphedema.

Facklamia is an emerging pathogen in human beings and only a few clinical cases were reported in the literature. We detected the presence of this unusual microbe among the skin flora of three patients with filarial lymphedema in a 16S rRNA-based metagenomic study on the skin microbiome. To our knowledge, this is the first report of detection of this microbe in patients affected with filarial lymphedema. Further investigations are required to elucidate the role of Facklamia in secondary skin and soft tissue infection of filarial lymphedema patients.

Profiling the broad antibody diversity of lymphatic filariasis immune antibody repertoire by deep sequencing.

Lymphatic filariasis is caused by infections of thread-like filarial worms, namely Wuchereria bancrofti, Brugia Malayi and Brugia timori. However, in-depth analysis of the antibody repertoire against Lymphatic filariasis is lacking. Using high-throughput sequencing of antibody repertoires, immunome analysis of IgG (LG) and IgM (LM) repertoires were studied. Despite significant differences between LG and LM in V(D)J gene usage, IGHV4-34, IGHV6-1, IGHD3-10 and IGHJ4 were preferred in both repertoires. The CDR3 in the LG repertoire showed a longer length than LM. Higher SHM level were observed in

Effect of Adding a Six-Week Course of Doxycycline to Intensive Hygiene-Based Care for Improving Lymphedema in a Rural Setting of Mali: A Double-Blind, Randomized Controlled 24-Month Trial.

Lymphedema (LE) is one the most disfiguring chronic manifestations of lymphatic filariasis. Its management relies primarily on limb hygiene and local care. A previous study in Ghana demonstrating a beneficial effect of doxycycline on LE led to the current multicenter trial on the efficacy of doxycycline in filarial LE. A randomized placebo-controlled trial was initiated in two rural health districts in Mali. Patients with LE stages 1-3 were randomized to receive either doxycycline (200 mg/day) or placebo over a 6-week monitored treatment period and were then followed every 6 months for 2 years

The extent of protective footwear use among school-age rural children at high risk for podoconiosis and socio-economic correlates: A household cross-sectional survey in Southern Ethiopia.

BACKGROUND: Podoconiosis is preventable if genetically susceptible people wear shoes starting from an early age and do so consistently. However, lack of routine use of footwear is one of the major risk factors for podoconiosis and several other foot-related Neglected Tropical Diseases (NTDs). This study is aimed at describing the extent of footwear use among school-age rural children susceptible to the disease and investigating associated socioeconomic factors. METHODS: Cross sectional surveys were conducted in 330 randomly selected households in Wolaita zone, southern Ethiopia. A household h

Ivermectin inhibits extracellular vesicle secretion from parasitic nematodes.

Lymphatic filariasis (LF) is a disease caused by parasitic filarial nematodes that is endemic in 49 countries of the world and affects or threatens over 890 million people. Strategies to control LF rely heavily on mass administration of anthelmintic drugs including ivermectin (IVM), a macrocyclic lactone drug considered an Essential Medicine by the WHO. However, despite its widespread use the therapeutic mode of action of IVM against filarial nematodes is not clear. We have previously reported that filarial nematodes secrete extracellular vesicles (EVs) and that their cargo has immunomodulator

Risk of Recrudescence of Lymphatic Filariasis after Post-MDA Surveillance in Brugia malayi Endemic Belitung District, Indonesia.

Belitung district in Bangka-Belitung Province, Indonesia with a population of 0.27 million is endemic for Brugia malayi and 5 rounds of mass drug administration (MDA) were completed by 2010. Based on the results of 3 transmission assessment surveys (TAS), the district is declared as achieving elimination of lymphatic filariasis (LF) in 2017. The findings of an independent survey conducted by the National Institute of Health Research and Development (NIHRD) in the same year showed microfilaria (Mf) prevalence of 1.3% in this district. In 2019, NIHRD conducted microfilaria survey in 2 villages i

Using intervention mapping to design and implement quality improvement strategies towards elimination of lymphatic filariasis in Northern Ghana.

INTRODUCTION: The Global Strategy to Eliminate Lymphatic Filiariasis (GFELF) through Mass Drug Administration (MDA) has been implemented in Ghana since the year 2000 and transmission has been interrupted in 76 of 98 endemic districts. To improve the MDA in the remaining districts with microfilaria (MF) prevalence above the 1% threshold for the interruption of transmission, there is a need to identify and implement appropriate quality improvement (QI) strategies. This paper describes the use of intervention mapping to select QI strategies to improve an existing evidence-based MDA program in Nor

Changes in Cytokine, Filarial Antigen, and DNA Levels Associated With Adverse Events Following Treatment of Lymphatic Filariasis.

BACKGROUND: Mild to moderate adverse events (AEs) are common after treatment of lymphatic filariasis (LF) and pose a major challenge for the global LF elimination program. We studied changes in cytokine levels and filarial worm components in plasma of subjects with and without AEs following treatment of LF. METHODS: Participants (n = 24) were hospitalized and monitored for AEs following treatment. Cytokines (27), filarial DNA, circulating filarial antigen (CFA), and immune complexes were measured in plasma samples collected before and after treatment. RESULTS: Levels for 16 cytokines increas

Identification and characterization of Loa loa antigens responsible for cross-reactivity with rapid diagnostic tests for lymphatic filariasis.

The Global Program to Eliminate Lymphatic Filariasis (LF) relies on rapid diagnostic tests (RDTs) to determine where annual mass drug administration for LF is required and when it can be stopped. These tests detect a Wuchereria bancrofti glycoprotein in the blood of infected persons via a carbohydrate moiety recognized by the monoclonal antibodies AD12 and DH6.5. Loiasis cross-reactivity with LF RDTs has recently been recognized as a serious obstacle to LF elimination in loiasis-endemic areas. To better understand the nature of this cross-reactivity, we used the DH6.5 antibody to immunoaffinit

Lymphatic filariasis transmission on Mafia Islands, Tanzania: Evidence from xenomonitoring in mosquito vectors.

INTRODUCTION: Lymphatic filariasis (LF) is a chronic nematode infection transmitted by mosquitoes and in sub-Saharan Africa it is caused by Wuchereria bancrofti. The disease was targeted for global elimination by 2020 using repeated community-wide mass drug administration (MDA) distributed in endemic areas. However, recently, there has been a growing recognition of the potential role of including vector control as a supplement to MDA to achieve elimination goal. This study was carried out to determine mosquito abundance and transmission of bancroftian filariasis on Mafia Islands in Tanzania as

Assessing correlations between geological hazards and health outcomes: Addressing complexity in medical geology.

BACKGROUND: The field of medical geology addresses the relationships between exposure to specific geological characteristics and the development of a range of health problems: for example, long-term exposure to arsenic in drinking water can result in the development of skin conditions and cancers. While these relationships are well characterised for some examples, in others there is a lack of understanding of the specific geological component(s) triggering disease onset, necessitating further research. OBJECTIVES: This paper aims to highlight several important complexities in geological expos

Risk Factors for Chronic Intertrigo of the Lymphedema Leg in Southern India: A Case-Control Study.

Rich clinical experiences indicate that toe web intertrigo is a major predisposing condition for cellulitis/acute dermatolymphango adenitis (ADLA) and the number of lesions is the strongest predictor of frequency of ADLA in lymphedema (LE) patients. However, there is scanty information on the risk factors for the establishment of chronic toe web intertrigo in LE patients. We performed a case-control study recruiting 52 lower limb LE with intertrigo and 52 lower limb LE without intertrigo in community settings and assessed general and local potential risk factors for chronic intertrigo.
