The fear of using tramadol for pain control (tramadolophobia) among Egyptian patients with cancer.

OBJECTIVES: The fear of using tramadol for pain control (tramadolophobia) by Egyptian patients with cancer is a frequent problem in our practice. This study was conducted to explore the prevalence of and the reasons behind tramadolophobia among Egyptian patients with cancer. METHODS: A structured interview including open-ended and closed questions. The study included 178 adult patients with cancer from two cancer centers in Cairo and Sharkia, Egypt. RESULTS: The source of information about tramadol was a non-healthcare-related source in 168 (94 percent) patients, mainly the media (50 percent…

The Circumscribed Fear Measure: Development and initial validation of a trans-stimulus phobia measure.

Extant stimulus-specific fear measures are limited to a small number of stimuli and contain significantly different content. This article describes 2 studies that develop a more flexible fear measure—the Circumscribed Fear Measure (CFM)—and examine its psychometric properties. In Study 1, participants (N = 771) completed an initial item pool while considering their most feared stimulus. Results of factor analyses were used to propose a 25-item, 5-factor measure that would span the domain of specific phobia reactions, applicable to different stimuli. In Study 2, participants (N = 959) completed…

Psychometric properties of the Fear of Cancer Recurrence Inventory: an item response theory approach.

OBJECTIVE: Classical psychometric methods have been used to demonstrate the validity and reliability of the 42-item Fear of Cancer Recurrence Inventory (FCRI). Our aim was to expand on this evidence with information on the discriminative value of the individual items when administered to people with a personal history of melanoma, using an item response theory (IRT) approach. METHODS: We used a two-parameter IRT model to examine all items of the FCRI, primarily regarding whether people with a personal history of melanoma use the response scale as expected (as indicated by item characteristic …

Alterations in skin temperature and sleep in the fear of harm phenotype of pediatric bipolar disorder.

In children diagnosed with pediatric bipolar disorder (PBD), disturbances in the quality of sleep and wakefulness are prominent. A novel phenotype of PBD called Fear of Harm (FOH) associated with separation anxiety and aggressive obsessions is associated with sleep onset insomnia, parasomnias (nightmares, night-terrors, enuresis), REM sleep-related problems, and morning sleep inertia. Children with FOH often experience thermal discomfort (e.g. feeling hot, excessive sweating) in neutral ambient temperature conditions, as well as no discomfort during exposure to the extreme cold, and alternate …

Living with advanced breast cancer: perceptions of Pakistani women on life expectations and fears.

BACKGROUND: Pakistan has the highest rate of breast cancer in Asia. Many women delay seeking heath advice and treatment for breast cancer, and between 50% and 70% of women will present when the disease in is in an advanced, aggressive form, particularly young women. The psychological impact of breast cancer is challenged by the negative public perception of cancer. OBJECTIVES: This qualitative study explored the psychological impact of advanced breast cancer in women receiving follow-up care in an oncology hospital in Pakistan. METHODS: This qualitative study used semistructured interviews t

Fearful attachment mediates the association of childhood trauma with schizotypy and psychotic-like experiences.

We examined whether insecure attachment styles mediate the association between childhood trauma and nonclinical psychotic phenomena in 546 young adults. Fearful attachment mediated the associations of physical/emotional trauma with schizotypy, suspiciousness, and psychotic-like experiences. Results support theoretical accounts implicating attachment disruptions in the pathway from childhood adversity to psychosis.

Hippocampal molecular mechanisms involved in the enhancement of fear extinction caused by exposure to novelty.

Exposure to a novel environment enhances the extinction of contextual fear. This has been explained by tagging of the hippocampal synapses used in extinction, followed by capture of proteins from the synapses that process novelty. The effect is blocked by the inhibition of hippocampal protein synthesis following the novelty or the extinction. Here, we show that it can also be blocked by the postextinction or postnovelty intrahippocampal infusion of the NMDA receptor antagonist 2-amino-5-phosphono pentanoic acid; the inhibitor of calcium/calmodulin-dependent protein kinase II (CaMKII), autocamt…

Quantitative genetic analysis of traits related to fear and feather pecking in laying hens.

Feather pecking is a well known problem in flocks of laying hens. It is partially controlled by genetics. Fear is frequently reported to be related with feather pecking. The present study reports the result from a quantitative genetic analysis of feather pecking and three fear test traits in laying hens. Fear was recorded by the tonic immobility test, the open field activity and the emergence box test. These were recorded at a juvenile and adult age of the hens. The heritability of feather pecking was 0.16, and in the range between 0.07 and 0.14 for the fear test traits. Genetic correlations b…

Fear of falling and gait variability in older adults: a systematic review and meta-analysis.

BACKGROUND: Fear of falling (FOF) and increased gait variability are both independent markers of gait instability. There is a complex interplay between both entities. The purposes of this study were (1) to perform a qualitative analysis of all published studies on FOF-related changes in gait variability through a systematic review, and (2) to quantitatively synthesize FOF-related changes in gait variability. METHODS: A systematic Medline literature search was conducted in May 2014 using the Medical Subject Heading (MeSH) terms "Fear" OR "fear of falling" combined with "Accidental Falls" AND "…

Treatment of severe fear of childbirth with haptotherapy: design of a multicenter randomized controlled trial.

BACKGROUND: About six percent of pregnant women suffer from severe fear of childbirth. These women are at increased risk of obstetric labour and delivery interventions and pre- and postpartum complications, e.g., preterm delivery, emergency caesarean section, caesarean section at maternal request, severe postpartum fear of childbirth and trauma anxiety. During the last decade, there is increasing clinical evidence suggesting that haptotherapy might be an effective intervention to reduce fear of childbirth in pregnant women. The present study has been designed to evaluate the effects of such in…

Open-label treatment with escitalopram in patients with social anxiety disorder and fear of blushing.

Fear of blushing (FB) is a form of social anxiety disorder (SAD) characterized by an intense and obsessive threat of blushing in front of other people. No data are available on the specific efficacy of antidepressants on FB. This open-label pilot study investigated whether the selective serotonin reuptake inhibitor escitalopram specifically improves symptoms of FB in SAD patients. Thirty-nine patients meeting Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition criteria for SAD and presenting a significant FB according to the Salpêtrière Erythrophobia Questionnaire (SEQ) were …

Orexins (hypocretins) contribute to fear and avoidance in rats exposed to a single episode of footshocks.

Orexins (hypocretins) are peptides that have been shown to regulate behavioral arousal and wakefulness. Recent evidence indicates that orexin neurons are activated by stress and that orexins play a role in anxiety. The present paper describes a series of experiments that examined whether orexins are involved in the anxiety that resulted from exposing rats to an acute episode of footshocks (5 × 2 s of 1.5 mA shocks). We found that prepro-orexin (ppOX) mRNA was elevated in rats at 6 and 14 days after exposure to footshock and that ppOX mRNA levels were correlated with fear at 14 days post-shock.…

The impact of domestication on fearfulness: a comparison of tonic immobility reactions in wild and domesticated finches.

We examined differences in the fear response between Bengalese finches and their wild ancestor, the white-backed munia, to explore the evolutionary mechanisms of behavioural changes due to domestication. The Bengalese finch (Lonchura striata var. domestica) was domesticated from the wild-living white-backed munia (L. striata) approximately 250 years ago. A previous study indicated that Bengalese finches sing much more complex songs than white-backed munias. We hypothesised that Bengalese finches are likely able to allocate more resources to reproduction in exchange for reduced survival effort.…

'Living each week as unique': maternal fears in assisted reproductive technology pregnancies.

OBJECTIVE: to explore women's fears during pregnancy following conception via assisted reproductive technology (ART). METHODS: 19 expectant first-time mothers were interviewed during the third trimester of pregnancy using a semi-structured schedule. Perceptions of and feelings about pregnancy were assessed. Content analysis was used to identify themes and subthemes. FINDINGS: four overarching themes emerged: the baby's survival, the health of the baby, the efficacy of the mother and childbirth. Of these, the most commonly reported fears were related to miscarriage or fetal death, and the bab…

Neuropeptide regulation of fear and anxiety: Implications of cholecystokinin, endogenous opioids, and neuropeptide Y.

The neural circuitry of fear likely underlies anxiety and fear-related disorders such as specific and social phobia, panic disorder, and posttraumatic stress disorder. The primary pharmacological treatments currently utilized for these disorders include benzodiazepines, which act on the GABAergic receptor system, and antidepressants, which modulate the monamine systems. However, recent work on the regulation of fear neural circuitry suggests that specific neuropeptide modulation of this system is of critical importance. Recent reviews have examined the roles of the hypothalamic-pituitary-adren…
