Etiology of Onychomycosis in Patients in Turkey.

BACKGROUND: Onychomycosis is a chronic nail infection caused by dermatophytes, Candida, nondermatophyte molds, and Trichosporon. The purpose of this study was to identify the underlying pathogen in patients with onychomycosis in our region. METHODS: A retrospective analysis of 225 cases with onychomycosis, diagnosed over a 27-month period at the Department of Dermatoveneorology, Bezmialem Vakif University, Istanbul, Turkey, and confirmed with culture, was performed. RESULTS: Patient age ranged from 2 to 87 years (mean ± SD, 41.59 ± 17.61), and female patients were more commonly affected (120

Effect of litter quality on foot pad dermatitis, hock burns and breast blisters in broiler breeders during the production period.

Foot pad dermatitis and hock burn lesions are a form of contact dermatitis, a condition affecting skin areas in contact with unsuitable or irritating material. Contact dermatitis is a common problem, reducing the welfare of broilers, and is believed to also affect broiler breeders. However, there is very little research on contact dermatitis in breeders. This study followed the severity of foot pad lesions in broiler breeders throughout the production period. At slaughter the presence of hock burns and breast blisters was also determined. In addition, changes in litter condition over time and

[Visit a swimming pool in Moscow as a risk of skin diseases of fungal etiology].

450 persons visited swimming pools in Moscow out of 900 patients seen by a dermatologist, have been questioned. Skin and toenail mycoses plates (18.7% and 21.8% respectively) were shown to prevail in the overall structure of the skin disease in persons who regularly visited swimming pool for a long time. In the structure of the species composition of fungal pathogens of diseases in such persons Trichophyton rubrum (51%) takes the first place, Candida albcans (25.5%) takes the second place.

Prevalence of toenail onychomycosis in patients with type 2 diabetes mellitus and evaluation of risk factors.

BACKGROUND: we sought to determine the frequency of toenail onychomycosis in diabetic patients, to identify the causative agents, and to evaluate the epidemiologic risk factors. METHODS: data regarding patients' diabetic characteristics were recorded by the attending internal medicine clinician. Clinical examinations of patients' toenails were performed by a dermatologist, and specimens were collected from the nails to establish the onycomycotic abnormality. All of the specimens were analyzed by direct microscopy and culture. RESULTS: of 321 patients with type 2 diabetes mellitus, clinical o

Acremonium mycetoma in a heart transplant recipient.

Mycetomas are chronic, progressive infections caused either by fungi (eumycetoma) or filamentous bacteria (actinomycetoma) and are characterized by the triad of draining sinuses, tumefaction, and the presence of macroscopic grains. We describe a case of eumycetoma in a cardiac transplant recipient caused by the soil saprophyte Acremonium species. This represents only the fifth case of eumycetoma reported in a solid organ transplant recipient. With the population of immunosuppressed patients and the incidence of invasive fungal infections increasing, consideration should be given to unusual sap

Successful treatment of pyoderma gangrenosum with topical 0.5% nicotine cream.

Pyoderma gangrenosum is potentially a devastating and destructive disorder. There is no uniformly effective or specific therapy for pyoderma gangrenosum. Previous reports of nicotine therapy for pyoderma gangrenosum have suggested it to be efficacious. Unfortunately, previous reports were restricted by the use of commercially available preparations of nicotine, either as a gum or patch formulation. We have used topical nicotine 0.5% w/w cetamacrogol formula A cream that enables direct application onto the lesion, as well as dose and concentration variation. Two patients with pyoderma gangrenos

Itraconazole and terbinafine treatment of some nondermatophyte molds causing onychomycosis of the toes and a review of the literature.

BACKGROUND: Onychomycosis may be caused by dermatophytes (which form the majority of organisms), Candida species, and nondermatophyte molds. OBJECTIVE: To evaluate the efficacy and safety of itraconazole and terbinafine in the treatment of some nondermatophyte molds that cause toe onychomycosis and to review the literature on the treatment of nondermatophyte mold toe onychomycosis using the oral antifungal agents. PATIENTS AND METHODS: Patients with nondermatophyte mold toe onychomycosis were treated in an open, prospective manner with either itraconazole (pulse) or terbinafine therapy. In e

Dermatophyte test medium culture versus mycology laboratory analysis for suspected onychomycosis. A study of 100 cases in a geriatric population.

Onychomycosis is the most frequently encountered condition in podiatric practice in the United States. A variety of modalities are available to confirm the presumptive diagnosis of onychomycosis. This study was conducted to compare the results of in-office dermatophyte test medium cultures with those of mycology laboratory analysis for 100 cases of suspected onychomycosis in a geriatric population. The results demonstrated that 20% of the patients had dermatophyte involvement, 56% had saprophyte involvement, and 19% had yeast involvement. Only 50% of positive dermatophyte test medium cultures

Toenail onychomycosis in patients with acquired immune deficiency syndrome: treatment with terbinafine.

Skin infections caused by dermatophytes are one of the most frequent dermatological complications in patients with acquired immunodeficiency syndrome (AIDS) resulting from infection with human immunodeficiency virus (HIV). Tinea unguium associated with AIDS is characterized by being clinically more aggressive and therapeutically more difficult to treat than in the general population. Terbinafine is considered to be a first-choice option for the treatment of dermatophyte onychomycosis in immunocompetent individuals. This drug has been used in a series of 21 HIV-positive patients diagnosed with

Foot infections due to Hendersonula toruloidea and Scytalidium hyalinum in coal miners.

A total of 250 coal miners were screened for mycotic skin infections. Sixty-six (34.8%) miners had clinical lesions on their feet which proved to be of mycotic etiology by direct microscopy and culture. Hendersonula toruloidea, the commonest etiological agent, was the sole agent recovered from 23 (34.8%) of the positive cases and was also isolated from three (4.5%) cases of mixed infection with dermatophytes (two cases with Trichophyton rubrum and one with Trichophyton mentagrophytes var. mentagrophytes). Scytalidium hyalinum was recovered as the sole causal agent in four (6.1%) patients and f

Contact dermatitis of the feet.

For a period of 12 years, 539 patients with shoe and dye contact dermatitis, were studied in 2 out-patient clinics in Lisbon. 75% of the patients had a bilateral recurrent dermatitis confined to the dorsa of the feet. PTBP resin and chromium proved to be the 2 principal allergens, but when lesions were localized to the soles, rubber additives, chiefly mercaptobenzothiazole, were the predominant ones. Allergy to dyes was found in 54 patients, most patients being allergic to PPD, p-aminoazobenzene and disperse orange 3. No patient was found to be allergic to the anthraquinone dye, disperse blue

Infection and diabetes mellitus.

Most physicians believe that diabetic individuals are predisposed to infections and that infection complicates the control of the diabetes. However, only bacteriuria can be documented to occur with increased frequency in diabetic compared with nondiabetic patients. Although most bacteriuric diabetic patients are asymptomatic, severe infections such as emphysematous pyelonephritis, papillary necrosis, perinephric abscess, and candida pyelonephritis may occur. Tuberculosis, once a proven threat to diabetic individuals, is a less serious problem now that effective screening and chemoprophylaxis p

Laser therapy for onychomycosis in patients with diabetes at risk for foot complications: study protocol for a randomized, double-blind, controlled trial (LASER-1).

BACKGROUND: In a sham-controlled double-blind trial, we aim to establish the efficacy and safety of the local application of laser therapy in patients with diabetes, onychomycosis and risk factors for diabetes-related foot complications. Onychomycosis leads to thickened and distorted nails, which in turn lead to increased local pressure. The combination of onychomycosis and neuropathy or peripheral arterial disease (PAD) increases the risk of developing diabetes-related foot complications. Usual care for high-risk patients with diabetes and onychomycosis is completely symptomatic with frequent
