Artificial intelligence-assisted speaking in Spanish as a foreign language: effects on foreign language emotions and willingness to communicate.

INTRODUCTION: Although AI-powered conversational tools are increasingly used in foreign language education, little is known about their longitudinal effects on learners' emotional experiences and willingness to communicate (WTC), particularly among beginner learners of languages other than English. Grounded in Positive Psychology, Control-Value Theory, and the WTC framework, this study examined the effects of AI-assisted speaking practice using TalkPal AI on foreign language enjoyment (FLE), foreign language anxiety (FLA), foreign language boredom (FLB), and WTC in Spanish as a foreign languag

Clinical practice guideline on the treatment of axial spondyloarthritis and psoriatic arthritis. ESPOGUÍA 2024.

The important advances in the area of therapeutic interventions and the time elapsed have justified the complete update of the Clinical practice guideline on the treatment of axial spondyloarthritis and psoriatic arthritis (ESPOGUIA2017). Methodologically, the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) system has been incorporated, which allows the quality or certainty of the evidence to be assessed for each outcome of interest, previously prioritized by the drafting group and which structures the process of formulating recommendations explicitly. Thus, an updat

Haemophobia.

I recently finished an introductory session with a group of new start undergraduate nursing students about their responsibilities in clinical practice and was finishing up the session by checking if anyone had any questions. The usual questions were asked such as "why is it called off duty and not on duty?" Good question but once you have worked in clinical practice for a while the question of when you are off duty becomes very important more so than on duty! However, I was not expecting the following question which was, "I faint when I see blood do I need to tell the mentor when I am out in p

Canadian Society of Nephrology Commentary on the 2012 KDIGO clinical practice guideline for glomerulonephritis: management of nephrotic syndrome in children.

The KDIGO (Kidney Disease: Improving Global Outcomes) clinical practice guideline for management of glomerulonephritis was recently released. The Canadian Society of Nephrology convened a working group to review the recommendations and comment on their relevancy and applicability to the Canadian context. A subgroup of pediatric nephrologists reviewed the guideline statements for management of childhood nephrotic syndrome and agreed with most of the guideline statements developed by KDIGO. This commentary highlights areas in which there is lack of evidence and areas in need of translation of ev

Management of temporomandibular disorders in a restorative practice.

Various disorders related to the masticatory muscles or to the temporomandibular joints may be encountered in a restorative dental practice. Most of them can be managed by the restorative dentist whose primary interest may not lie in the area of temporomandibular disorders. The term temporomandibular disorders is used in this discussion to denote the group of diseases of the masticatory system that involves primarily the temporomandibular joints, the masticatory muscles, and the occlusion of the teeth. The learning objective of this article is to describe temporomandibular disorders encountere

Do family physicians need medical assistants to detect and manage hypertension?

To test a new approach to detecting and managing hypertension, 34 family practices in southwestern Ontario that comprised 32 124 patients aged 20 to 65 years were randomly assigned in a 5-year study to either undertake a system of care in which a medical assistant oversaw screening and attended to education, compliance and follow-up (experimental group) or continue their usual practices (control group). The 17 physicians in the experimental practices (15 659 patients) were matched with the 17 in the control practices (16 465 patients) according to size of the community, sex, level of practice

Best practice guidelines for management of spinal disorders in skeletal dysplasia.

BACKGROUND: Disorders of the spine present a common and difficult management concern in patients with skeletal dysplasia. Due to the rarity of these conditions however, the literature, largely consisting of small, single institution case series, is sparse in regard to well-designed studies to support clinical decision making in these situations. METHODS: Using the Delphi method, an international, multi-disciplinary group of individuals, with significant experience in the care of patients with skeletal dysplasia, convened to develop multi-disciplinary, "best practice" guidelines in the care of

Person-centred care - radiographers' perceptions of the framework and its performance in radiography. A phenomenographic study.

INTRODUCTION: Person-centred care is being implemented in healthcare to facilitate co-created care and patients' participation in their own care. This is a new practice, also among radiographers. The aim of the study was to explore radiographers' perceptions of the framework of person-centred care and its performance in radiography. METHODS: Phenomenography was the study design. Focus group discussions and interviews were performed within a period of three months with a group of five radiographers in clinical practice. The discussions were part of a training programme on the theory and practi

Involving practice nurse and other assistant clinical staff members in the management of low back pain: A qualitative interview study from Danish general practice.

OBJECTIVES: Involving practice nurse and other assistant clinical staff members in providing information and education to patients with low back pain at follow-up appointments may release more time and improve care in general practice. However, this requires a shift in the division of tasks, and general practitioners' barriers and facilitators for this are currently unknown. The objectives were to explore general practitioners' experiences and perceptions of including assistant clinical staff members in the management of low back pain. METHODS: This is a semi-structured interview study in Dan

Benzodiazepine dependence: a general practice survey.

This study aims to evaluate the dependence potential of benzodiazepines in general practice. Amongst a sample of 1079 current general practice patients 87 (8.1 percent) were using benzodiazepines. Most of the users were middle to old age females and had chronic physical illnesses. The drug history was extensive (mean--3 years 7 months) and about 39 percent (34/87) of users were currently on a higher dose than the first prescribed dose. Interview of a selected group (40) of benzodiazepine users showed 75 percent (30/40) to be from a lower socio-economic class and 17 percent (7/40) of these repo

Differences in the quality of oral anticoagulation therapy with vitamin K antagonists in German GP practices - results of the cluster-randomized PICANT trial (Primary Care Management for Optimized Antithrombotic Treatment).

BACKGROUND: In Germany, patients receiving oral anticoagulation (OAC) are often treated by general practitioners (GPs), and large proportions of patients receive vitamin K antagonists (VKAs). The quality of OAC in German GP practices, differences between various practices, and improvement potential through implementation of case management, have not yet been investigated satisfactorily. Based on results of a cluster-randomized controlled trial, we aimed to assess whether OAC quality can be improved, any variations between practices exist and determine practice- and patient-level factors. Impact of limb ischemic preconditioning on the incidence of vein thrombosis in patients with peripherally inserted central catheter.

BACKGROUND: Peripherally inserted central catheters (PICC) are increasingly used in clinical practice, which also leads to an increased incidence of PICC-related thrombosis. Local thrombus formation could be prevented by limb ischemic preconditioning (IPC). This study aimed to determine whether IPC can prevent deep vein thrombosis in patients with PICC. METHODS: A total of 600 breast cancer patients receiving PICC were randomized into two groups between July 2016 and July 2018 at the Department of Radiation Oncology. In the preconditioning group, 5 min of ischemic preconditioning was performe
