[Risk factors of elbow stiffness after open reduction and internal fixation of intercondylar fracture of humerus].

OBJECTIVE: To investigate the risk factors of elbow stiffness after open reduction and internal fixation of intercondylar fracture of humerus. METHODS: From March 2015 to February 2019, 120 patients with humeral intercondylar fractures were treated with open fixation including 59 males and 61 females, aged from 25 to 77 years with an average of(53.5±3.2) years. According to the occurrence of elbow stiffness after operation, 120 patients were divided into stiffness group(37 cases) and control group(83 cases). The related factors of elbow stiffness were analyzed by single factor analysis, and t

Plate fixation for management of humerus fractures.

Proximal humerus, humeral shaft, and distal humerus fractures are all common adult fractures, and often occur in older patients. While the treatment of proximal humerus fractures remains controversial, certain fractures benefit from plate fixation such as fracture-dislocations and head-split fractures. When plate fixation is chosen, anatomic reduction and restoration of the medial calcar are important for successful results. Further research is required to minimize complications and determine the optimal surgical candidates for plate fixation. Humeral shaft fractures are generally treated non-

Long bone asymmetry and hemihypertrophy as a skeletal marker of child abuse.

Diaphyseal overgrowth is a well-documented complication of long bone fracture in orthopedic studies, but it is not a condition commonly mentioned in the forensic literature as a possible indicator of child abuse. Here we present an occurrence of humeral hypertrophy associated with a physeal fracture to the left distal humerus in a case of infant child abuse. Humeral overgrowth was present in this infant along with superficial bruising, other humeral fractures, rib fractures, and vertebral injuries consistent with battered child syndrome. In 2002, the humeri, lower ribs, and vertebrae of this i…

[Fractures of the proximal humerus].

The Neer four-part classification for displaced proximal humeral fractures is the most useful classification so far devised for consistent diagnosis and treatment. Nevertheless, there are several points in the Neer classification that need changing. Group II fractures, i.e., displaced fractures of the anatomical neck have a severe prognosis, excluding a mild graduation as in group II. Displaced four-segment fractures can be classified in group IV as well as in group V, which means the classification less than sharp. Without disregarding Neer's criteria, we use our own simplified classification…

Lessons learned from treating patients with unstable multifragmentary fractures of the proximal humerus by minimal invasive plate osteosynthesis.

INTRODUCTION: The authors present clinical and radiographic results of minimal invasive plate osteosynthesis (MIPO) for three- or four-part fractures of the proximal humerus. PATIENTS AND METHODS: Twenty-six patients with three- or four-part proximal humeral fractures treated with the MIPO technique through the deltoid splitting approach were clinically and radiographically evaluated at a minimum of 12 months with an average of 20.1 months. The valgus-impacted type of three-part fracture was excluded to verify the results of the MIPO with unstable multifragmentary fractures of the proximal hu…

Functional Outcome after Simultaneous Bilateral Four-Part Proximal Humerus Fracture: A Comparison of ORIF and Hemiarthroplasty in an Individual Patient.

Simultaneous bilateral four-part proximal humeral fractures are rare. Four-part fractures of the proximal humerus are difficult for the patient and technically demanding for the surgeon. Our surgical tactic is to attempt open reduction and internal fixation where possible. We report the functional outcome in a 56-year-old female who sustained simultaneous bilateral four-part proximal humeral fractures after falling down a flight of stairs. Open reduction and internal fixation using threaded pins and tension band suture was performed on one side, and shoulder replacement hemiarthroplasty was re…

Acute prosthetic replacement for severe fractures of the proximal humerus.

Nineteen patients with 20 severe proximal humeral fractures were treated early with a Neer humeral prosthesis from 1979 to 1986. There were 18 four-part and two three-part fractures. The average age of the patients at the time of injury was 64 years. Follow-up evaluation was performed at an average of 40 months. Pain relief was satisfactory in 18 of 20 fractures. Patients were satisfied with the result in 16 of the 20 shoulders. Functional evaluation was good or fair in 15 of 20 patients. Active forward elevation averaged 72 degrees and active external rotation 16 degrees. Complications consis…

Primary Monoblock Inset Reverse Shoulder Arthroplasty Resulted in Decreased Pain and Improved Function.

BACKGROUND: The first-generation, lateral-center-of-rotation reverse shoulder arthroplasty (RSA) modular design has demonstrated durable early-, mid-, and long-term outcomes. The second-generation monoblock implant shares a similar design but eliminates the modular junction and facilitates inset placement within the metaphysis to avoid humeral-sided junctional failures and facilitate metaphyseal press-fit. However, no paper has specifically examined the radiographic findings and improvements in pain and function after the use of this next generation design. QUESTIONS/PURPOSES: (1) After secon

Radiographic and anatomic variations on postoperative acromion fractures after inlay and lateralized reverse shoulder arthroplasty.

INTRODUCTION: Postoperative fracture of the acromion is a complication uniquely more common after reverse shoulder arthroplasty (RSA) than other forms of shoulder arthroplasty. There is limited knowledge regarding the etiology of these fractures or the anatomic risk factors. The purpose of this study is to identify associations of the acromioclavicular (AC) joint and relative humeral and glenoid positioning on the occurrence of acromial fractures after RSA. METHODS: A retrospective case-controlled study was performed on primary RSA patients treated by a single surgeon from September 2009 to S

Using Teriparatide to Augment Healing in a Humeral Shaft Nonunion: A Case Report.

The occurrence of complications of fracture healing, such as delayed union and nonunion, is well known, but the use of pharmacotherapy for these delayed unions and nonunions has not been explored in detail. The authors describe a case of traumatic humeral shaft fracture successfully treated with once-daily administration of 20mcg of teriparatide for six months. The patient was a 22-year-old male who had been through a road traffic accident. The radiograph of the humerus shaft showed a fracture line and the displaced distal portion of the shaft of the humerus. Based on these features, the patie

Combined radial and median nerve injury in diaphyseal fracture of humerus: a case report.

Radial nerve palsy is the most common neurological involvement in humeral shaft fractures. But combined radial and median nerve injury in a closed diaphyseal fracture of the humerus is rare. Combined injury to both radial and median nerve can cause significant disability. A detailed clinical examination is therefore necessary following humeral shaft fractures. We report a patient with closed diaphyseal humeral fracture (AO 12A-2.3) together with radial and median nerve palsy, its management and review of the literature. As the patient had two nerves involved, surgical exploration was planned.

[Closed flexible medullary nailing in pediatric traumatology].

Flexible Medullary Nailing is an excellent method in children when operative management is indicated. It is a new biomechanical concept using prebent flexible rods introduced in a metaphyseal area. In femoral shaft fractures, closed percutaneous nailing permits stabilization using wires inserted by a medial and a lateral approach. In severely displaced humeral fractures of the upper end, two Kirschner wires are inserted under X-ray control through a epicondyl hole. Pinning can be indicated in others fractures : tibia, radial neck (original procedure), fore arm, humeral supracondylar original p…

Ultrasonographic evaluation of birth injury to the shoulder.

Injuries of the proximal humerus in infants are often missed or misinterpreted because the humeral epiphysis is frequently nonossified. Ultrasonography enables direct visualization of the proximal humeral epiphysis, metaphysis, joint space, and relationship of the epiphysis with the glenoid cavity. We describe an infant with unexplained agitation and ventilatory difficulty related to a fracture of the proximal right humerus that was not appreciable on plain film radiographs. Sonographic imaging of the shoulder, with use of the opposite side for comparison, clearly demonstrated a Salter I proxi

Morphological Analysis of Fractures of the Proximal Humerus by the Fracture Mapping Technique.

OBJECTIVE: Fractures of different parts of the proximal humerus may lead to different postoperative functional deficits, but there are few studies on the morphology and related functions of the proximal humerus. The purpose of this study was to analyze the fracture pattern of the proximal humerus by the three-dimensional (3-D) fracture mapping technique and to further evaluate its clinical utility. METHODS: Patients with proximal humeral fractures admitted to Pudong Hospital, Fudan University, from January 2018 to December 2020, were analyzed. Three surgeons divided the fractures into groups …

Traumatic Bilateral Luxatio Erecta from a Sliding Injury Down a Ladder; A Rare Case Report and Literature Review.

Bilateral inferior shoulder dislocations also known as luxatio erecta is an extremely rare injury that is commonly complicated with injuries to the humeral head, glenoid, clavicle, scapula, rotator cuff, capsule, ligaments, brachial plexus, axillary artery and vein.  Our patient is a 66-year-old man who presented with both upper extremities above his head in a fixed abducted position after sliding down a ladder approximately 6-meters. Initial radiographs revealed both humeral heads to be located below the glenoid fossa with each humeral shaft parallel to the scapular spines.  Computed tomograp
