Patency of the ductus arteriosus is one of the more common problems of the neonate. Although the ductus arteriosus usually closes within the first days of life, persistent patency can complicate the clinical status of a newborn. The ductus arteriosus also may play a role in the pathophysiology of persistent pulmonary hypertension of the newborn and in some forms of congenital heart disease. Patients with paroxysmal supraventricular tachycardia (PSVT) who are either not adequately controlled with chronic suppressive therapy or who have infrequent attacks may be treated with oral diltiazem with a high probability of conversion to sinus rhythm within 30 minutes of ingestion of the calcium channel blocker. Due to its rapid absorption, the diltiazem appears to be effective in converting PSVT and may eliminate the need for emergency department evaluation and parenteral drug therapy.

Periodic paralyses are uncommon disorders characterized by episodic muscle weakness, often with hypokalemia. Thyrotoxic periodic paralysis (TPP) is the most common and is rarely seen in the Caucasian population; the relative unfamiliarity of TPP among physicians in the United States may lead to initial errors in diagnosis. Last year's introduction of clozapine, an atypical antipsychotic, in the United States was the most significant advance in schizoprenia treatment since the advent of chlopromazine in the 1950s. However, clozapine has a disturbingly high incidence of agranulocytosis. The drug manufacturer designed a complex and expensive mandatory blood monitoring system that was dismantled after public pressure earlier this year. There is hope that clozapine will become more widely available to schizoprenic patients.

Serious cellulitis, a common acute illness, frequently is associated with underlying venous and lymphatic compromise. The process is easily treated but, when recurrent, is associated with significant morbidity. Local measures, such as support hose and elimination of tinea pedis, have a role in its therapy but do not uniformly prevent recurrence. Prophylactic antibiotic therapy modeled after rheumatic fever prophylaxis can be effective in the prevention of this process.

Hereditary nephritis with deafness, or Alport's syndrome, is a familial disorder characterized by progressive renal insufficiency, sensorineural hearing loss and ocular abnormalities. A 37-year-old white woman presented with bilateral symmetrical ascending motor weakness. Subsequent evaluation revealed she was suffering from infectious mononucleosis. This case demonstrates that one should consider mononucleosis when presented with an idiopathic disease of the nervous system. The classic features of mononucleosis, such as pharyngitis, lymphadenopathy or splenomegaly, need not be present.
