Interval cancers as an indicator of performance in breast screening.

BACKGROUND: The literature shows that breast screening performance varies between centers. Those who monitor the performance of screening programs must use appropriate methods to demonstrate quality. Analysis of interval cancer rates provides one such measure. This study analyses interval cancers over a ten-year period in a single center of the UK National Breast Screening Program, offering screening to a population of 45,000 women. METHODS: 108,948 screening examinations were undertaken between 1987 and 1996. Sequential interval cancer rates were studied and compared with the results of the

[Late myelopathy after chemonucleolysis. Case report and review of the literature].

Chemonucleolysis is a debated therapeutic method for herniated lumbar disc. We report a patient who suffered a sequence of characteristic sequels cumulating in late-onset myelopathy with persistent spastic paraplegia, sensory loss below T8 and bladder incontinence. Complications of chemonucleolysis are less frequent as compared to herniated disc surgery, but may cause severe impediment. Serious complications are anaphylactic shock, intracranial or spinal hemorrhage and transverse myelitis. This has to be taken into account for indication and patient information.

[Plunging ranula presenting as a giant retroauricular mass: A case report].

Plunging ranula is rare and manifests as the submandibular or submental cystic mass, without intra-oral abnormality. It usually causes misdiagnosis and malpractice. This article reported a case of plunging ranula that appeared as a massive mass located behind the left ear. A 6-year-old child presented with recurrent left retroauricular swelling over six months without obvious inducement. The mass showed repeatedly swelling and persistent skin ulcers. Physical examinations described a fluctuant, nontender mass behind the left ear whose size was about 5 cm×5 cm×3 cm. The skin upon the surface of…

The anatomy of missed breast cancers.

Missed breast cancer continues to account for the highest percentage of medical malpractice cases in the United States. A retrospective, computer-aided study was performed to investigate the mechanisms of missed breast carcinomas, missed either by mammography or by clinical exam. In a consecutive series of 509 breast cancers found in patients registered at a University Comprehensive Breast Cancer Clinic, no tumour that was 5 mm or less in maximal diameter was clinically palpable. This subgroup consisted of seven in situ and 32 invasive carcinomas. The incidence of palpable tumours in 5 mm incr…

Prevalence of traditional uvulectomy and its associated factors among parents who had less than 6 months of infants in Gondar City, north-west Ethiopia: Mixed study design.

BACKGROUND: Traditional uvulectomy is widely practiced in Africa especially in sub-Saharan countries including Ethiopia. Limited Studies conducted in different times and areas of the world have shown that the prevalence of uvulectomy and its associated factors are varied from country to country. There is limited evidence to determine the prevalence and reasons of malpractice in Ethiopia. Therefore, this study aimed to assess the prevalence of traditional uvulectomy, and its associated factors among parents who had children aged less than 6 months. METHODS: A community-based cross-sectional mi…

Learning from the Law. A review of 21 years of litigation for pain during caesarean section.

The large majority of caesarean sections in the UK are now carried out under neuraxial anaesthesia. Although this technique is widely accepted as being the safest option in most circumstances, the use of regional anaesthesia increases the risk of patients experiencing intra-operative discomfort or pain. Pain during operative obstetric delivery is the commonest successful negligence claim relating to regional anaesthesia against obstetric anaesthetists in the UK. In the following article, using a database of over 360 cases spanning 21 years, we break down and examine the recurrent components of

Do maternal characteristics influence maternal-fetal medicine physicians' willingness to intervene when managing periviable deliveries?

OBJECTIVE: Determine the relative influence of patient characteristics on Maternal-Fetal Medicine (MFM) physicians' willingness to intervene when managing 23-week preterm premature rupture of membranes. STUDY DESIGN: Surveyed 750 randomly sampled US members of the Society of Maternal-Fetal Medicine. Physicians rated their willingness to offer induction, order steroids and perform cesarean across eight vignettes; then completed a questionnaire querying expectations about neonatal outcomes and demographics. RESULTS: Three hundred and twenty-five (43%) MFMs responded. Patient characteristics on

[Injury to the median nerve after minimally invasive decompression: discrepancy between the surgical report and actual course of surgery].

A 72-year-old man suffering from carpal tunnel syndrome had undergone minimally invasive decompression by using a SafeGuard® Mini-Open Carpal Tunnel Release System. After the operation the patient presented with a paraesthesia in the median nerve distribution. Two months later an operative revision was performed in another hospital. A partial transection of the median nerve and an incomplete release was seen. The surgeon of the first operation stated that detailed informed consent including the risk of iatrogenic nerve injury had been obtained. Furthermore he referred to the operative report,

What patient attributes are associated with thoughts of suing a physician?

OBJECTIVE: To address a neglected research area: the attributes of rehabilitation patients associated with "thoughts of suing a physician" (S-MD). DESIGN: The S-MD statement "I am thinking about suing one of my doctors" was administered to 2264 people, along with the Battery for Health Improvement (BHI 2). Items predictive of S-MD were identified. SETTING: Acute physical therapy, work hardening programs, chronic pain programs, physician offices, and vocational rehabilitation programs. PARTICIPANTS: Participants included 777 rehabilitation patients and 1487 nonpatient community-dwellers. IN

Medicolegal aspects of automobile injuries.

We have tried to acquaint you with the basic elements of law relevant to the treatment of vehicle-related injury, i.e., the Good Samaritan clause, the definition of liability in surgery, warranty and informed consent. We have listed prophylactic measures to reduce medicolegal problems to a minimum. We have outlined the stepwise development of a court case and your duties to the court. Finally we have detailed special situations that occur in a trial and how to cope with them. In summary, bear in mind that it is natural to be edgy about court appearances. Few of us in medicine, after all, enjoy
