Keratocystic odontogenic tumor arising at the mandibular ramus with an impacted tooth: a case report and mimic lesions.

OBJECTIVES: To consider the biologic behaviors of keratocystic odontogenic tumors (KCOTs) and ameloblastomas and dentigerous cysts. METHODS: A 63-year-old Japanese man presented with swelling and discomfort in the left cheek during jaw movement. Examination revealed a multilocular lesion within the mandible extending from the left second premolar to the left mandibular ramus and coronoid process; the lesion contained a deviated impacted tooth. The tumor had expanded beyond the bone and was invading the masseter and medial pterygoid muscles. Marginal mandibulectomy with a free iliac bone graft

Syndecan-1 (CD138) and Ki-67 expression in odontogenic cystic lesions.

The aim of this study was to assess the immunohistochemical expression of syndecan-1 (CD138) and Ki-67 in radicular cysts (RC), dentigerous cysts (DC) and keratocystic odontogenic tumors (KOT). Thirty-five RC, 22 DC and 17 KOT were used in the study and immunohistochemical reactions using anti-syndecan-1 and anti-Ki-67 antibodies were performed by the streptavidin-biotin-peroxidase method. Fisher's exact test and Spearman's correlation coefficient were used for statistical analysis of data. Among the studied lesions, no differences in the syndecan-1 expression were observed, but the suprabasal

New method for analysis of facial growth in a pediatric reconstructed mandible.

INTRODUCTION: The aim of this article was to present a new method of analysis for the assessment of facial growth and morphology after surgical resection of the mandible in a growing patient. METHODS: This was a 2-year longitudinal study of facial growth in a child who had undergone segmental resection of the mandible with immediate reconstruction as a treatment for juvenile aggressive fibromatosis. Three-dimensional digital stereo-photogrammteric cameras were used for image acquisition at several follow-up intervals: immediate, 6 months, and 2 years postresection. After processing and superi

CBCT features of florid cemento-osseous dysplasia with coexisting lesions at different maturation stages: a case report and literature review.

BACKGROUND: Florid cemento-osseous dysplasia (FLCOD) is a begin fibro-osseous lesion occurring in the tooth-bearing regions of the jaws and is characterized by replacement of normal bone with fibrous connective tissue and cementum-like mineralized material. Most patients are asymptomatic and are diagnosed incidentally during radiographic examinations, and the condition may be misdiagnosed because of overlapping imaging features with other jaw lesions. Because the disease is usually asymptomatic and exhibits imaging features that overlap with those of several odontogenic and inflammatory lesion

Cemento-osseous dysplasia with concomitant cemento-ossifying fibroma: a rare case report.

Despite rare reports of hybrid tumors containing cemento-ossifying fibroma (COF), its association with florid cemento-osseous dysplasia (FCOD) is very rare. A 38-year-old female presented for treatment of an expansive, tumoral lesion extending from the right mandibular lateral incisor to the first molar. Clinical examination revealed a significant bulging of the vestibular and lingual mandible cortices. CT scans revealed a hypodense, unilocular lesion that was relatively well-demarcated. Also, multiple mixed hyperdense/hypodense lesions were observed in the alveolar process of the jaws. An inc

Use of a surgical drain for continuous postoperative trigeminal nerve block in a dog.

A male, neutered, mixed-breed dog, aged 12 years, was referred for a left mandibulectomy resulting from the recurrence of a squamous cell carcinoma in the vertical ramus of the left mandible. Preoperative pain management consisted of methadone 0.2 mg kg-1 intravenously and an ultrasound-guided trigeminal nerve block with bupivacaine. Intraoperatively, a Jackson-Pratt drain connected to a continuous suction bulb was placed in the vicinity of the trigeminal nerve for postoperative bupivacaine delivery and exudate drainage. Bupivacaine 0.13% was delivered every 4 hours along with subcutaneous mel

Intraosseous mandibular clear cell odontogenic carcinoma with predominant small round blue cells: a potential diagnostic pitfall.

OBJECTIVE: Clear cell odontogenic carcinoma (CCOC) is a rare malignancy of the jaw, presenting significant diagnostic challenges. This report aims to highlight the complexities associated with biopsy-based diagnoses of oral and maxillofacial lesions, as demonstrated in a case of intraosseous mandibular CCOC initially suggestive of Ewing's sarcoma due to its presentation with small round blue cells. RESULTS: The patient, a 37-year-old male, presented with a mandibular lesion that on incisional biopsy was suggestive of Ewing's sarcoma. Subsequent, comprehensive histologic evaluation after defin

Regulatory role of human fibrocartilage stem cells in condyle osteochondroma.

OBJECTIVE: Osteochondroma is a common benign skeletal disorder for which different molecular and histological features of long bones have been reported. We investigated cell-of-origin and molecular mechanisms of a rare condylar osteochondroma (CO). METHODS: Human fibrocartilage stem cells (hFCSCs) isolated from CO and normal condyle tissue were used for RNA sequencing, real-time PCR, Western Blotting, immunohistology, flowcytometry, as well as for chondrogenic differentiation, proliferation, and apoptosis detection assays. RESULTS: HFCSCs were fewer in number with weaker proliferative capaci

Ipsilateral sagittal split ramus osteotomy to facilitate reconstruction of the temporomandibular joint after resection of condylar osteochondroma.

We report the outcomes of 12 patients with osteochondroma of the mandibular condyle who were treated by condylectomy with sagittal split ramus osteotomy (SSRO) between January 2011 and October 2015. Variables assessed before and after operation were imaging, appearance, maximum mouth opening, maximum mandibular protrusion, lateral excursion, and function of the temporomandibular joint (TMJ). Patients were followed up for a mean (range) of 21 (13 - 30) months. Outcomes were satisfactory with no complications or recurrence. Patients regained good occlusion and facial symmetry, and satisfactory f

The efficacy of hyperbaric oxygen therapy related to the clinical stage of osteoradionecrosis of the mandible.

This study aimed to evaluate the success of hyperbaric oxygen therapy (HBOT) and surgery in the treatment of mandibular osteoradionecrosis (ORN) in relation to the extent of the ORN. Twenty-seven patients with ORN were identified from a total of 509 patients with a history of primary oral or base of the tongue cancer; these patients had been treated with radiation therapy with curative intent between 1992 and 2006, with a radiation dose to the mandible of ≥50Gy. The ORN was staged according to the classification of Notani et al. The time from completion of radiation therapy to the development

Juxta-articular Myxoma of the Temporomandibular Joint.

The juxta-articular myxoma represents a benign mesenchymal neoplasm that arises from tissue within or adjacent to a joint space. There have been a number of reported cases involving myxomas of the knee, shoulder, elbow, wrist, and hip. To our knowledge there, however, have been no reported cases of juxta-articular myxomas of the temporomandibular joint (TMJ). This report describes the case of a 57-year-old woman with a juxta-articular myxoma of the left TMJ extending into the infratemporal fossa (ITF). Access to the tumor was accomplished via a preauricular incision and low condylar osteotomy

Osteonecrosis of the jaw in a patient receiving cabozantinib.

Since the discovery of bisphosphonate-related osteonecrosis of the jaw, there has been increasing evidence in recent years of osteonecrosis induced by drugs other than bisphosphonates, mainly agents with antiangiogenic and antiosteoclastic activity. Mandibular osteonecrosis was observed in a 51-year-old female with medullary thyroid cancer receiving cabozantinib, a new tyrosine kinase inhibitor having antiangiogenic activity. The bone necrosis appeared after a dental extraction. The clinical, radiographic and histologic picture of a chronic non-healing extraction socket was consistent with dru

Differential Diagnosis of a Periapical Radiolucent Lesion. A Case Report and Review of the Literature.

This article demonstrates a methodological approach to diagnosing a periapical radiolucency that could not be diagnosed using only basic clinical and radiographic findings. The patient was a 59-year-old Hispanic female with a small tender mass on the lower gingiva associated with tooth #25. Radiographic appearance demonstrated a well-defined radiolucent lesion at the apices of the mandibular incisors. The patient had no significant medical history. Cone-beam computed tomography (CBCT) showed bony expansion of the buccal plate. Differential diagnosis included non-endodontic unilocular radioluce

Para-articular osteochondroma of the temporomandibular joint.

A para-articular osteochondroma is a rare osteocartilaginous tumour that arises in the soft tissue adjacent to a joint but with no attachment to the bone. To our knowledge, this is the first case of one reported in the region of the temporomandibular joint (TMJ). The patient presented with severe preauricular pain caused by a para-auricular osteochondroma, which arose in the preauricular region just external to the TMJ.

Acute vasculitis resulting in free flap failure: the importance of early recognition and options for management.

Unusual or unexpected medical causes for free flap failure do occur but are uncommon. We present a rare case of a fibula free flap failure due to an acute vasculitis which was undiagnosed until after the flap had failed. In addition to two successful flap salvages and intravenous heparin, an epoprostenol infusion was commenced but a third salvage was not successful. The vasculitis resulted in marked blood vessel wall thickening, and cutaneous manifestations which presented as late signs. High peri-nuclear anti nuclear cytoplasmic antibody (pANCA) and myeloperoxidase (MOP) titres were subsequen

Diffusion-weighted imaging in the evaluation of odontogenic cysts and tumours.

OBJECTIVE: The differentiation between keratocystic odontogenic tumour (KCOT) and other cystic/predominantly cystic odontogenic tumours is difficult on conventional CT and MR sequences as there is overlap in the imaging characteristics of these lesions. The purpose of this study was to evaluate the role of diffusion-weighted imaging (DWI) and to assess the performance of apparent diffusion coefficients (ADCs) in the differential diagnosis of odontogenic cysts and tumours. METHODS: 20 patients with odontogenic cysts and tumours of the maxillomandibular region were examined with DWI. Diffusion-

Prosthodontic treatment of a patient with bisphosphonate-induced osteonecrosis of the jaw using a removable dental prosthesis with a heat-polymerized resilient liner: a clinical report.

Bisphosphonate-induced osteonecrosis of the jaws (BONJ) is reported frequently in patients receiving oral or intravenous bisphosphonates. To minimize potential complications, dental treatment must be performed with care. There are invasive and noninvasive treatment options for patients with active BONJ or a history of this complication. This clinical report describes the prosthodontic treatment of a patient who developed BONJ after receiving intravenous bisphosphonates (pamidronate). Because of the recurrent incidence of BONJ, noninvasive prosthetic therapy with telescopic overdentures and a h

Avascular osteonecrosis of the mandible following bisphosphonate therapy.

Cancer patients receiving bisphosphonate therapy, both intravenously and orally, are predisposed to developing osteonecrosis of the jaw. We present a case of a 66-year-old man with metastatic prostate cancer who spontaneously developed avascular necrosis of the mandible after being on intravenous zoledronic acid for 1 year. Jaw osteonecrosis commonly presents with mild to severe jaw pain. Although most of the reported cases give a history of dental procedures, there have been cases of osteonecrosis developing spontaneously. In cases of established osteonecrosis of the jaw, there is little evid
